        혈액투석실 간호사의 비판적 사고성향과 임상의사결정능력이 환자안전역량에 미치는 영향

        This descriptive research is to provide preliminary data for the development of a nursing service program that ensures the safety of hemodialysis patients by identifying the effects of nurses’ critical thinking disposition and clinical decision making ability on the patient safety competence. The research participants were recruited through convenience sampling. The survey was conducted on 202 nurses who had worked for three months or longer in a hemodialysis unit. Critical thinking disposition was measured using the instrument to measure critical thinking disposition developed by Yun Jin (2004), and clinical decision making ability was measured using the Korean version of the Clinical Decision Making in Nursing Scale (Jenkins, 1985), which was translated and revised by Baek Mi-kyung (2005). The instrument to measure patient safety competence in this study is the one developed by Lee Nam-ju (2012) and then revised and supplemented by Jang Hae-na (2012) for clinical nurses. In summary, Consequently, education programs tailored for nurses according to their years of experience and position need to be developed and implemented. In addition, policies related to patient safety accidents should be prepared and continuous education should be provided for nurses working at a hemodialysis unit. 본 연구는 서술적 조사연구로 혈액투석실에 근무하는 간호사의 비판적 사고성향과 임상의사결정능력이 환자안전에 미치는 영향을 확인하여 혈액투석환자의 안전간호를 위한 서비스 프로그램 개발의 기초자료를 제공하기 위해 수행되었다. 연구대상은 혈액투석실 근무경력 3개월 이상의 간호사 202명이며, 구조화된 설문지를 이용하여 자료를 수집하였다. 연구도구는 윤진(2004)이 개발한 비판적 사고성향 측정도구, Jenkins(1985)가 개발한 The Clinical Decision Making in Nursing Scale을 백미경이(2005)이 수정한 임상의사결정능력 측정도구, 이남주(2012)가 개발한 환자안전역량 측정도구를 장해나(2013)가 수정한 도구를 사용하였다. 연구결과 혈액투석환자의 안전간호 실현을 위해 간호사의 비판적 사고성향과 임상의사결정 능력이 주요한 영향요인임을 확인하였으므로 직급별 맞춤형 교육프로그램을 개발하여 적용하고, 환자안전을 위한 정책을 마련하여 지속적으로 환자, 보호자 및 간호사에 대한 교육이 필요할 것으로 사료되었다.

        지방 종합병원의 서비스 질이 고객의 재이용의사에 미치는 영향

        본 연구는 지방 종합병원 서비스 질이 고객의 재이용의사에 어느 정도 영향을 미치는 가를 실증적으로 검증하는데 목적을 두고 있다. 이를 위해 본 연구에서는 부산지역 200병상 이상 600병상 이하 종합병원 중 대학병원을 제외한 총 12개 의료기관 입원환자와 외래환자를 대상으로 설문조사를 실시하였고, 설문조사 결과는 SPSS21을 이용하여 신뢰도 분석, 요인분석, 상관관계분석, 평균분석 그리고 다중회귀분석을 통해 연구모형을 검증하였다. 실증적 분석결과 서비스 질의 주요 구성요소인 유형성, 신뢰성, 접근성, 대응성, 친절성은 모두 의료서비스 이용자의 재이용의사에 통계적으로 유의한 정(+)의 영향을 미치는 것으로 나타났다. 표준화 계수를 통해 서비스 질 구성요소 별 재이용의사에 미치는 영향력 정도는 대응성(β=.260)이 가장 높게 나타났으며, 그 다음으로 신뢰성(β=.217), 친절성(β=.209), 유형성(β=.162), 접근성(β=.151) 순으로 나타났다. 그러나 성별, 연령, 학력 등과 같은 인구통계학적 변수들은 통계적으로 유의한 영향을 미치지 않은 것으로 나타났다. The purpose of this study is to empirically examine the extent to which Local General Hospitals service quality influence customer revisit intention. A questionnaire survey was conducted in Busan area among General Hospitals with more than 200 to 600 sickbed capacity. With the exception of University Hospitals, admitted patients and out-patients in 12 medical institutions were targeted for the survey. The survey results were analyzed through SPSS and the research model was verified through reliability analysis, factor analysis, correlation analysis, average analysis and multiple regression analysis. The empirical results indicated that tangibility, reliability, accessibility, responsiveness and kindness being the main components of service quality, all had positive significant effect on medical service user’s revisit intention. Through a standardized coefficient, it was observed that responsiveness (β=.260) was the service quality component that had the highest effect on revisit intention. This was followed by reliability (β=.217), kindness (β=.209), tangibility (β=.162), and accessibility (β=.151) in an orderly manner. However, gender, age, educational level and similar demographic variables did not have statistically significant effect on customer revisit intention.

        Regulatory Mechanism of L-Alanine Dehydrogenase from Bacillus subtilis

        L-alanine dehydrogenase from Bacillus subtilis exhibits allosteric kinetic properties in the presence of $ZN^{2+}$. $ZN^{2+}$ induces the binding of substrate (L-alanine) to be cooperative at pH 8.0. The effect of pH variation between pH 7.0 and pH 10.0 on the inhibition by $ZN^{2+}$ correlates with the pH effect on the $K_m$ values for L-alanine within these pH range indicating that $ZN^{2+}$ and substrate compete for the same site. No such cooperativity is induced by $ZN^{2+}$ when the reaction is carried out at pH 10. At this higher pH, $ZN^{2+}$ binds with the enzyme with lower affinity and noncompetitive with respect to L-alanine. Inhibition of L-alanine dehydrogenase by $ZN^{2+}$ depends on the ionic strength. Increase in KCI concentration reduced the inhibition, but allosteric property in $ZN^{2+}$ binding is conserved. A model for the regulatory mechanism of L-alanine dehydrogenase as a noncooperative substrate-cooperative cofactor allosteric enzyme, which is compatible in both concerted and the sequential allosteric mechanism, is proposed.

        IFC 포맷을 활용한 BIM S/W의 건물정보모델 교환 방법론 연구

        In order to implement BIM (Building information Modeling) effectively, picking up and combining right softwares from available resources are required. And it is important to interchange data between applications without loosing any information. Data interchange using the international Industry Foundation Classes (IFC) for building information model has been recently recognized. Even though many softwares are supporting IFC format, there has not been enough research on the problems of practical data exchange and its implementation. The problem of BIM data exchange is mostly caused by flexibility of IFC model structure and certification which does not test roundtrips between different software products. In the presented research work the interoperability performance of three BIM S/W, which are the most widely used and IFC2x3 compatible architectural design applications, has been evaluated. As a result of the research, the directions for the interchange data of BIM S/W and mapping classes between BIM S/W were suggested.
