This report presents a case of metasric calcinosis cutis resembling pseudoxanthoma elasticum in clinical appearance. The patient, 19-year-old man, has dull yellowish firm folded lesion on both axillae, and immobility of lower extremities because of bony osteoporosis, knee fracture, impaired renal function, Skin biopsy, bone marrow biopsy, bone X-ray, and laboratory findings showed metastatic calcinosis cutis due to histiocytosis X and chronic renal failure.

Red lunula, dusky red erythema of nailbed in the area of lunula, has ben reported in patients with alopecia areata and rheumatuid arthritis. We report a case of red lunula in a 63-year-old man. The patient showed dusky red, blanchable erythema of the lunula area in nailbecl of total fingernail, and toer.iails in association with hypertension due to nephrotic syndromc.

Chromomycosis is a chronic mycosis of the skin and subcutaneous tissue characterized by a brown, thick-walled, round, nonbudding form of causative fungi in the tissue. We report a case of chromomycosis caused by Fonsecaea pedrosoi in a 56-year-old Korean woman who showed 3 × 4cm sized deep ulcerations on the dorsa of both hands. Histopathologically, chronic granulomatous inflammation and sclerotic cells were observed. Fonsecaea pedrosoi was grown into typical black colonies in fungus culture. She was treated with pedicle flap graft followed by oral itraconazole, and showed clinical improvement.

Woringer-Kolopp disease, so called pagetoid reticulosis, is most likely represent a chronic localized from of reticulosis. A 24-year old male patient visited with asymptomatic slowly enlarging, scaly single patch on his left thigh for 10 years duration. Histopathologic studies were taken with H-E stain, D-PAS, transmission electronmicroscopy and monoclonal antibody(OK6) tissue stain. Infiltration of atypical cells showed in the lower portion of epidermis like pagetoid patterned, none in dermis. It seems to be that the cell in this case probably originated from the stimuIated T-lymphocyte.

나환자에서 Dapsone과 Clofazimine 병용치료법

Seventy-seven patients who were treated regularly for more than 5 years in the Taegu Leprosy Mission were investigated with regard to the change of the bacillary index(BI) after treatment of either dapsone(DDS) alone or a combination of DDS and clofazimine. The results were as follows: 1) In the group that took only DDS 400-500 mg per week, the BI conversion to negative took average 51 months. 2) In the group that took only 600-700 mg per week, the BI conversion to negative took average 34 months. 3) In the group that took only DDS 400 mg per week initially and 600-700mg per week secondarily, the BI conversion to negative took average 64 months, the last 33 months of which marked the time period that 600-700 mg were taken per week. 4) In the group that took only DDS 400-500mg per week initially and a combination of DDS gpp 700 mg per week and clofazimine. 3pp-4pp mg per week secondarily, the BI conversion to negative took average 63 months, the last 35 months of which marked the time period for the combined therapy. 5) In the group that took a combination of DDS 600- 700 mg per week and clofazimine 400 mg per week, the BI conversion to negative took average 42 months.

수종 악성종양 환자 및 정상인 혈청내의 표피세포질 항체에 관한 연구

For the purpose of studying the compiarson of normal persons with patients with malignant tumors, the author investigated the incidence of epidermal cytoplasmic antibodies (EC-Ab) with sera taken from 15 normal persons, 38 patients with malignant tumors, and 2 patients with miscellaneous disorders by method of indirect immunofluorescence. The results were as follow: 1) .Positive reactions of EC-Ab were found in 2(13%) of 15 normal persons and 9 (24%) of 38 patients with malignant tumors. 2)Incidence of positive reactions of EC-Ab in various types of malignant turnors were found in 3(75%) of 4 patients with lung cancer, 2(40%) of 5 patients with rectal cancer, 3(13%) of 24 patients with stomach cancer, and 1(100%) patient with Hodgkins lymphoma. 3) Among 9 patients with malignant tumors of positive EC-Ab, 5 patients yielded the upper epiderrnaI cytoplasrnic (UEC) antibodies, 3 patients the general epidermal cytoplasmic (GEC) antibodies, and 1 patient basal cell layer (BCL) antibody. 4) A.mong 9 patients with malignant tumors of positive, the deposition of IgG was discovered in 7 patients, IgA in 1 patient and C, in 1 patient.

대상포진(herpes zoster)은 지각신경절에 잠복해 있던 수 두대상포진 바이러스(varicella-zoster virus, 이하VZV)가 면역력이 약화되었을 때 재활성화되어 발생하는 질환으로 알려져 있다. 편측의 신경분절을 따라 통증을 동반한 수포성 병변을 보이는 것이 특징으로, 병변은 흉부신경절, 뇌신경절, 허리신경절, 천골신경절의 순으로 흔히 발생하며, 천골신경절에 대상포진이 발생하는 경우는 5%로 추정된다1. 천골신경절의 피부 영역은 생식기, 항문주위, 엉덩이, 하지의 후측으로 이 중 남성 생식기인 음경에 병변이 발생하는 경우는 흔히 볼 수 없다2-5. 환자는 33세 남자로 내원 4일 전부터 좌측 음경 고환 및 엉덩이에 국소적으로 발생한 통증을 동반한 군집성 소수포를 주소로 내원하였다. 내원 당시 피부 병변은 좌측 천골 2-3번 신경절(S2-3)을 따라 음경과 엉덩이에 편측성으로 홍반성 군집성 소수포와 가피가 관찰되었다(Fig. 1). 동반증상으로 배뇨통을 호소하였다. 소아기에 수두에 이환된 과거력이 있었으며 가족력에서는 특이사항은 없었다. 내원시 시행한 일반혈액검사, 일반화학검사 및 소변검사는 정상 소견 보였으며, VZV에 대한 혈청 검사에서 IgM (1.45,1.0 초과시 양성)과 IgG (9.58, 1.09 이상시 양성) 모두 양성 소견을 보였다. 병변부에서 시행한 조직 생검에서 표피내 풍선형 변성과 함께 국소적인 수포 형성과 다핵거대세포가 관찰되었으며(Fig. 2A), 면역조직화학 검사에서 VZV에 양성 소견을 나타내었다(Fig. 2B). 이상의 소견을 바탕으로 음경에 발생한 대상포진으로 진단하였고 acyclovir를 하루 750 mg씩 7일간 정맥 투여하여 통증 및 피부 병변의 호전을 보였으며 이후 합병증도 발생하지 않았다. 대상포진은 특징적인 피부소견과 함께 통증이 동반되기 때문에 임상적으로 진단할 수 있지만 흔히 발생하는 부위가 아닐 때는 확진을 위해 추가적인 검사가 필요하다. 특히 음경과 같은 생식기 부위에 병변이 발생한 경우는 단순 포진을 반드시 감별하여야 하며, 이를 위해 혈청학적 검사, 면역조직화학 검사, 중합효소 연쇄 반응(PCR), 바이러스 배양 등이 이용될 수 있다2,3. 본 증례는 음경에 발생한 대상포진을 피부생검 이외의 혈청학적 검사 및 면역조직화학적 검사로 단순포진과 감별하여 확진한 국내 피부과 문헌 상 첫 증례로 임상적 가치가 있을 것으로 생각된다. 대상포진이 음경에 발생하였을 경우 급성기 통증이 심 하고 대상포진 후 신경통에 이환 될 가능성이 높다4. 또한 운동신경을 침범하여 방광의 기능이상을 초래해 요폐증이나 신경성방광이 발생 할 수 있고 대변 장애도 유발할 수 있다4,5. 본 증례에서는 배뇨통이 있었으나 치료 후 호전되었으며 요폐증이나 신경성방광은 관찰되지 않았고, 대상포진 후 신경통의 발생도 없었다. 저자들은 좌측 음경에 군집성 수포가 발생한 환자를 임상적 소견과 함께 피부 생검, 혈청학적, 면역조직화학적 검사로 대상포진으로 확진하고 드물고 흥미로운 예로 생각되어 문헌 고찰과 함께 보고한다.
