        E test 를 이용한 Candida 균종의 Azole계 항진균제 감수성 검사

        N/A Background: Because of recently increasing reports of azole-resistant Candida species, it is necessary to perform antifungal susceptibility test. E test(AB Biodisk, Sweden) is a rapid, easy-to- perform in-vitro antifungal susceptibility test. Objective : The purpose of the study was to investigate the minimal inhibitory concentration(MIC) of azole antifungal agents against Candida sp. isolated from skin lesion using E test. Methods: Twenty-seven clinical isolates of Candida sp.(including 24 C. albicans, 2 C. parapsilosis, and 1 C. guilliermondii) were tested. MIC endpoints of E test for itraconazole(ICZ) and fluconazole (FCZ) susceptibility were read after 24 hours for each isolates on Casitone agar. Results : MIC of azole antifungal agents against Candida sp. : MIC of ICZ was 0.008-1.0pg/mL on C. albicans, 0.023-0.125pg/mL on C. parapsilosis, and 0.064pg/mL on C. guilliermondii. MIC of FCZ was 0.5-4.0pg/mL on C. albicans, 1.5-2.0pg/mL on C. parapsilosis, and 0.5pg/mL on C. guilliermondii. According to NCCLS(National Committee for Clinical Laboratory Standards) guidelines, two strains of C. albicans isolated from patients with Candidal intertrigo showed ICZ-resistant. Conclusion : We found that E test represented a simple and valuable method for susceptibility test for Candida sp. (Korean J Dermatol 2001;39(6): 654~659)

        Cutaneous fungal infection is very common and important in dermatologic field. As Korean society has undergone socioeconomic and cultural changes, etiologic agents and clinical characteristics have changed substantially in cutaneous mycology as well. Furthermore, the diagnostic techniques and methods of treatment has progressed in the filed of mycology. Fungal infection of the skin is very common, and it is frequently misdiagnosed as dermatitis, bacterial infection, or even herpes viral infection. Unless dermatologists keep a keen eye and high index of suspicion, fungal infections will go undiagnosed or misdiagnosed, leading to various complications including tinea incognito, secondary bacterial infections, etc. The topic of this cutaneous fungal disease symposium is "Test your knowledge of cutaneous mycoses: fun and helpful quiz cases by experts". I hope this symposium is helpful to dermatologists for understanding and broadening the knowledge about differential diagnosis and therapy of cutaneous mycoses.

        Recently tinea incognito or steroid modified tinea is increasing by using topical and systemic steroid or calcineurin inhibitors. Despite the well described clinical features, fungal infections are sometimes misdiagnosed as dermatitis, bacterial infection, or even herpes viral infection. Tinea incognito is a dermatophytic infection have modified the clinical appearance of the fungal infection, transforming the typical ringworm and mimicking other skin diseases as a result of dermatological misdiagnosis or preexisting pathologies, The clinical appearance of the fungal infection was lupus erythematosus discoid-like, eczema-like, rosacea-like, especially on the face, impetigo-like and eczema-like on trunk and limbs. Less often the dermatophytosis resembled psoriasis, purpura, seborrhoic dermatitis and lichen planus. Therefore accurate differential diagnosis is essentially needed nowdays. The topic of this cutaneous fungal disease symposium is "Test your knowledge of cutaneous mycoses: fun and helpful quiz cases by experts". I will present you an interesting cases of fungal infection mimicking various skin disorders. I hope this lecture is helpful to dermatologists for understanding and broadening the knowledge about differential diagnosis and therapy of cutaneous mycoses.         경주지방의 백선증

        Background : Because of the differences in dermatophytoses by ographic distribution in Korea, we studied the incidence of dermatophytoses in the Kyongju area. Objective : The purpose of the study was to investigate the recant clinical and mycological features of dermatophytoses in the Kyongju area. Methods : We evaluated the clinical and mycological aspects flermatophytoses in 431 patients from September 1992 to August 1994 in the Kyongju area. Results : 1) The incidence of dermatophytoses was 7.8% out of a total of 5,549 out patients. 2) The incidence of dermatophytoses was Tinea(T.) pedis (36%), T. unguium (30.2%), T. corporis (12.0%), T. cruris, (11.5%), T. manus (7.3%), T. faic (2.2%), T. capitis (0.6%) in order of decreasing frequency. 3) Among the age groups, the in idence rate was highest in the third decade (21.3%). 4) The ratio of male to female patients was 1.5: l.5) Coexisting fungal infections were found in 1 patients (41.0%), and the cases of T. pedis with T. unguium was the most common. 6) In dercimatophytoses, the positive rate of KOH mount examination was 95.7 %, and the positive rate of culture was 77.6%. 7) Trichophytone T. rubrum was the most common causative organism of dermatophytoses (91.8%), T. mentagrophages (4.3%), T. verrucosum (2,3%), Microsporum(M.) canis (1.0%), M. gypseum (0.3%). Epidermoplyte floccosum (0.3%), presenting in order of decreasing freqiency. Conclusion : We find that the incidence rate of T. veriosum infection & coexisting dermatophytosis in the Kyongju area are higher than previous studies in Korea (Kor J Dermatol 1995;33(2): 294-302)

        Trichophyton mentagrophytes의 경구용 항진균제에 대한 감수성

        In vitro sensitivity of T. mentagrophytes against antifungal agents was investigated. The 20 strains of T. mentagrophytes were tested. They were 5 strains of granilar form, 5 of powdery form, 5 of persicolor form, and 5 of downy for m. The tested antifungal agents were griseofulvin, ket,oconazole and it,raconazole. The results were as follows : 1. Minimal inhibitory concintration(MIC) of antifungal agents against, T. mentagrophytes : MIC of griseofulvin was 3.13-25 ug/ml, the highest level, that of ketoconazole was 0.05-12 ug/ml, and that of itraconazole was 0.025 6.25 ug/ml, the lowest one. 2. MIC of antifungal agents against T. mentagrophytes strains. MlC of griseofulvin was 12-25 ug/ml on granular form and 3.13-25 ug/ml on the other forms. MIC of ketoconazole was 0.39-12 ug/ml on granular form, 0,05 0.78 ug/ml on powdery form, 0.05 12 ug/ml on persicolor form, and 0.1-12 ug/ml on downy form. MIC of itraconazole was 0.39-6.25 ug/ml on granular form, 0.05-0.39 ug/ml on powdevy form, 0.025-0.05 ug/ml on persicolor form, and 0.05-0.1 ug/ml on downy form. Granular form showed the highest level of MIC among antifungal agents. These findings suggesed that, itraconazole was the most arctive drug against T. mentagrophytes and the pranular form showed the lowest, sensirivity against antifungal agents.         포항 및 경주지역의 토양내 keratinophilic Fungi의 분포상태

        Background: Because saprophytes found in soil rarely cause dermatomycosis, we studied the distribution of soil keratinophilie fungi(KPF) in the Pohang & Kyongju areas. Objective : The purpose of the study was to investigate the distribution of soil KPF isolated in various sites in the Pohang & Kyongju areas. Methods : From 150 soil samples, collected at 50 sites in the Pohang & Kyongju areas, KPF were isolated using the hair baiting technique. Results : 1) Of the number of 150 sol samples KPF were isolated from 103 samples(68.7%). The total number of strains isolated were 115 consisting of 45 Trichophyton(T.) ajelloi, 33 Chrysasporium sp 24 Microsporum(M) gyseum, 7 Sepedonium, and 6 M. cookei. 2) The number of KPF isolated at varius sites was high in the cowshed, and low at the riverside. T. ajelloi was frequent in the field in the Pohang area, M. gypseum in the cowshed in the Kyongju area, Chrysosporium sp. on the hill, by the river and at the roadside in the Pohang area, M. cookei at the roadside in the Pohang area, and Sepedonium on the hill. Conclusion : We find that the distribution of KPF in the Pohang & Kyongju areas is similar to that in previous studies which have been undertaken in Korea. However, particular attention shoud be paid to the presence of KPF isolated near the sea. (Kor J Dermatol 1996;34(3):,381-385)

        소아 족부 및 수부백선에 관한 고찰

        Background : Because of concern about the infrequent occurre icc of tinea pedis & tinea manus, we studied the incidence of them. Objective : The purpose of the study was to investigate clinical and mycological features of tinea pedis & tinea manus. Methods . We reviewed outpatients with tinea pedis & tinea minus at the Catholic Skin Disease Clinic from 1976 to 1991. Results : The incidence was 0.08% out of 1,106,246 outpatients in tinea pedis, 0.02% in tinea manus. Interdigital lesions were the most common in tinea pedis. The ratio of male to female patients was 1.46: 1 in tinea pedis, 1.34:1 in tinea manus. The seasonaI pevalance was highest in summer. The species isolated were, in order of decreasing frequency, Trihohyton(T.) rubrum(89.9%), T. mentagrophytes(8.5%), Microgorum(M.) canis (1.6%) in tinea pedis, T. rubrum(77.8%), T. mentagrophytes(18.2%), M. Canis(2.0%), Epidermophyton(E.) floccosam(2.0% in tinea manus. Conclusion : We find that, the incidence of tinea pedis has been inc easing in recent years. (Kor J Dermatol 1993;31(5):713-720)
