요추 후외방 유합술에서 동종골과 국소골의 혼합 이시기술과 자가골 이식술의 결과 비교

연구계획 : 전향적 연구 연구목적 : 요추 후외방 유합술에 동종골 이식이 유용한지를 확인해 보기 위해 동종골과 국소골을 이식한 군과 자가골과 국소골을 이식한 군의 결과를 비교 분석하였다. 대상 및 방법 : 척추경 나사못을 이용한 요추 한 분절 후외방 유합술 환자 중 동결 건조 동종골과 감압술 및 피질골 박리술시 채취되는 국소골을 혼합 사용한 15명과 후방 장골에서 채취한 자가골과 국소골을 혼합 이식한 24명에 대해 방사선상 골 유합 등급과 기간, 임상 결과를 비교하였다. 결과 : 방사선상 골 유합 드급은 동종골 이식군이 1등급 7예, 2등급 5예, 3등급 3예로, 자가골 이식군이 1등급21예, 2등급 2예, 3등급 1예로 나타나 자가골 이식군의 유합 등급이 높았다. 골 유합이 완성되는 기간은 동종골 이식군이 11.1개월, 자가골 이식군이 6.7개월로 차이가 있었다. 수술후 임상 결과는 동종골 이식군이 우수 4예, 양호 9예, 보통 2예였고, 자가골 이식군은 우수 10예, 양호 11에, 보통 3예였다. 결론 : 요추 후외방 유합술에서 동종골과 국소골의 혼합 이식은 자가골 이식에 비해 골 유합 등급이 떨어지고 유합에 소요되는 기간이 길기 때문에 자가골을 이식하는 것이 바람직하다. Study Design : A prospective study Objectives : To determine the usefulness of allogeft and compare the result of allograft mixed with local bone and autograft in posterolateral lumbar fusion. Summary of Literature Review : Allograft is used for the purpose of spinal fusion. especially in the scoliosis surgery. In some studies allograft is not recommended in posterior lumbar spinal fusion alone. Materials and Methods : Fifteen patients underwent one level posterolateral lumbar fusion using freeze-dried allograft mixed with local bone. We compared them with twenty four patients using autogenous iliac bone graft mixed with local bone about radiographic fusion grading, duration of fusion and clinica outcomes. Results : Radiographic fusion grading of allograft was 7 cases of grade 1,5 cases of grade 2,3 cases of grade 3, and grading of autograft was 21 cases of grade1,2 cases of grade 2,1 cases of grade 3. Bone fusion was complete after 11.1 months in allograft and 6.7 months in autograft. Clinically, there were 4 excellent, 9 good, 2 fair cases in allograft and 10 excellent, 11 good, 3 fair cases in autograft. Conclusion : Allograft was inferior to autograft in posterolateral lumbar fusion because allograft mixed with local bone reduced radiographic fusion grading and prolonged duration of bone fusion.
