        일 종합병원 신규간호사의 예술치료 프로그램 효과

        Purpose: This study was done to evaluate the effects of group art and music therapy on decreasing job stress and increasing resilience of newly hired nurses. Methods: A mixed method design was used. Quantitative data (N=35) were collected through questionnaires from November 2 to December, 5, 2016 and were analyzed using a one-tailed paired t-test by SPSS/WIN 22.0 program. Qualitative data (N=18) were collected through group and individual interviews from November 10, 2016 to February 3, 2017 and were analyzed using a qualitative thematic analysis. Results: The first hypothesis “participants will have lower job stress after the art and music therapy” was not statistically supported (t=-1.12, p=.270). The second hypothesis, “participants will have higher resilience after the art and music therapy” was supported (t=-2.13, p=.041). Four main themes were derived from the interviews: ‘looking into myself’, ‘feeling a camaraderie’, ‘healing of my mind and body’, ‘change in the intimidated self,’ Conclusion: The findings indicate that art and music therapy for newly hired nurses may positively influence resilience. Hospital organizations should utilize the therapy for newly hired nurses to improve their resilience so that they can realize their own values and increase positive emotion despite job stress.

        기혼직장여성의 부부특성, 근로특성, 건강특성이 건강 관련 삶의 질에 미치는 영향요인

        본 연구는 기혼직장여성의 일반적 특성, 부부특성, 근로특성 및 건강특성이 건강관련 삶의 질에 미치는 영향을 파악하기 위하여 여성가족패널조사(KLoWF)자료를 활용하여 2차 분석을 수행한 서술적 조사연구이다. 본 연구는 7차년도(2017- 2018)조사에 참여한 기혼직장여성 총 2,060명을 대상으로 SPSS 25.0를 활용하여 분석하였다. 기혼직장여성의 일반적 특성, 부부특성, 근로특성 및 건강특성을 모두 투입한 결과, 가족의 경제상태, 진단받은 질환 유무, 스트레스 인지가 기혼직장여성의 건강 관련 삶의 질에 영향을 미치는 요인으로 나타났다. 이러한 연구결과를 토대로 기혼직장여성의 건강관련 삶의 질을 높이기 위해서는 가족의 경제 상태와 건강특성이 중요하고 이를 강화해야 할 방안 마련이 필요하다. 따라서 기혼직장여성의 건강 관련 삶의 질을 향상시키기 위하여 기혼직장여성 개개인의 건강특성을 고려한 맞춤형 건강 증진 프로그램 및 경제적 안정을 위한 다양한 정부 지원정책을 마련해야 함을 제언한다. The study aims to determine the factors associated with married working women’s health-related quality of life (HRQOL) in terms of general, marital, work, and health characteristics. In view of this, data was gathered from a total of 2,060 married working women who participated in the 7th wave (2017-2018) of the Korean Longitudinal Survey of Women and Family (KLoWF), which was conducted by the Korean Women’s Development Institute. The data was analyzed using SPSS 25.0. As a result of inputting the general characteristics, marital characteristics, working characteristics, and health characteristics of married women, family’s economic status, diagnosed disease, perceived stress were found to have a significant effect on married working women’s HRQOL. Based on the study results, educational programs and various policies to help married working women better manage their family-work conflict were proposed as a means to improve their HRQOL. This study is significant as it identified a wide range of factors that affect married working women’s HRQOL and discussed health and welfare measures to improve it.

        간호조직에서의 억압에 대한 개념분석

        본 연구의 목적은 간호조직에서의 억압에 대한 개념의 속성을 파악함으로써 억압에서 벗어나기 위한 교육개발에 기초자료를 제공하고자 함이다. 간호조직에서의 억압에 대한 속성, 선행요인, 결과요인을 밝히고자 Walker와 Avant의 방법을 이용하였고 2006년부터 2020년까지 발표된 국내 및 국외 선행연구 최종 17편을 선정하여 분석에 이용하였다. 개념의 속성은 낮은 자아존중감, 권한의 부족, 수평적 폭력이었다. 선행요인은 억압자, 위계적인 조직구조, 주변화이었으며, 결과는 직무만족도, 조직침묵, 환자안전 및 간호의 질로 나타났다. 연구 결과를 고려해보면, 간호조직 내의 억압을 벗어나기 위해서 간호사는 병원의 위계적인 근무환경 개선과 전문 간호사로서의 자기 인식이 중요하다. 따라서 간호사의 근무환경과 자기인식을 개선하기 위한 효과적인 교육 프로그램 개발이 필요하다. This study was to identify the concept of oppression in nursing organization thereby providing basic data on nursing education to improve oppression in nursing organization. Using Walker and Avant’s criteria to identify attributes, antecedents, and consequences of oppression in nursing organization, 17 research articles published from 2006 to 2020 were reviewed. The defining attributes of oppression in nursing identified in this study were low self-esteem, lack of authority, and horizontal violence. The antecedents to oppression in nursing organization were oppressors, hierarchical organizational structure, and marginalization. The consequences of oppression in nursing organization included job satisfaction, organizational silence, patient safety and quality of nursing. Considering the findings, improvement of the hierarchical working environment in hospitals and self-awareness as professional nurses are important for nurses to overcome oppression in nursing organization. Therefore, effective educational programs should be developed to ameliorate working environment and self-awareness of nurses.

        병원간호사의 정보활용능력과 개인정보보호에 대한 인식이 윤리적 가치관에 미치는 영향

        Purpose: The aim of this study was to elucidate clinical nurses’ ethics germane to information literacy and perception of patient data privacy and thus help nurses to develop more positive and consolidated ethical values. Methods: For this study a descriptive survey design was used. Participants were 142 nurses who worked in a hospital and completed self-report questionnaires. Data were collected from August 1 to 5, 2016 and were analyzed using independent t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and stepwise multiple regression with SPSS 22.0. Results: Ethical value had a positive correlation with information needs (r=.25, p=.002) in information literacy as well as in direct patient care (r=.27, p=.001), shift work (r=.20, p=.016), patient information management (r=.39, p<.001), and communication (r=.24, p=.004) in perception of patient data privacy. Patient information management, educational background, and age were significant variables predicting the level of ethical values and accounted for 21% of the variance. Conclusion: Ethical values education with particular emphasize on managing patient information should be encouraged for nurses who are younger and have a lower education level. Findings indicate a need for education programs to guide clinical nurses to utilize appropriate information when solving ethical challenges in every day nursing practice.

        한국 청소년의 건강행태 및 정신건강과 흡연의 관련성

        본 연구는 2015년 청소년 건강행태온라인 조사의 원시자료를 이용한 2차 분석연구로, 청소년의 건강행태 및 정신건강 특성과 흡연과의 관련성을 파악하여, 효과적인 흡연예방 교육 및 프로그램 개발을 위한 기초자료를 제공하기 위해 시행되었다. 연구결과, 일반적 특성에 따른 흡연은 성별, 학년, 학교유형, 학업성적, 평균용돈, 경제적 상태, 아버지 학력, 어머니 학력에서 유의한 차이가 있었고(p<0.001), 건강행태 특성에 따른 흡연은 평생 음주 경험, 가족흡연 여부, 친구흡연 여부, 신체활동 여부, 주관적 건강상태, BMI에서 유의한 차이가 있었다(p<0.001). 정신건강 특성에 따른 흡연은 스트레스 인지 여부, 주관적 수면 충족, 우울감 경험, 자살생각, 주관적 행복에서 유의한 차이가 있었다(p<0.001). 따라서 향후 흡연 관련 요인을 반영한 청소년의 효과적인 흡연 예방교육과 금연지도 프로그램의 개발 및 올바른 건강행태와 정신건강 증진을 흡연예방이나 금연교육과 통합하여 제공하는 방안이 필요하다. This study, as a secondary analysis research, bases its source upon on raw data from the on-line survey of the Korean adolescents health behaviors in 2015. This study was conducted to clarify the relevance of Korean adolescents smoking with their health behaviors and mental health, and thus further to provide basic sources of effective education for their smoking prevention and for program development. The results of the study showed that Korean adolescents’ smoking was significantly affected by the adolescents sex, grades, school achievements, types of schools, allowances, economic status, parents educational background (p<0.001). Meanwhile, smoking based on the characteristics of health behaviors was significantly different according to alcohol consuming experience, families and friends smoking, physical activity and subjective health status, and BMI (p<0.001). Smoking characterized by mental health showed the significant difference in their stress recognition, subjective sleep patterns, depressed experiences, suicidal ideation, and subjective sense of happiness (p<0.001). Based on all the factors regarding youth smoking, there should be development and provision of educational program for smoking prevention and cessation, including healthy behavior and mental health improvement for Korean adolescent.

        간호사가 지각한 수간호사의 윤리적 리더십이 간호업무성과에 미치는 영향

        Purpose: The purpose of this study was to elucidate nursing performance to superior’s ethical leadership as perceived by nurses and the mediating effect of faith in supervisor. Methods: For this study a descriptive survey design was used. Participants were 258 nurses who worked in general hospital and completed the self-report questionnaire. Data were collected from May 17 to May 31, 2017 and were analyzed using independent t-test, ANOVA, Scheffé test, and Path analysis with SPSS 22.0 and AMOS 21.0. Results: Superior’s ethical leadership as perceived by the nurses and faith in the supervisor were significant variables predicting the level of nursing performance and accounted for 48.1% of the variance. Faith in the supervisor as a mediator was found to have a partial mediating effect. Conclusion: In order to improve the nurses" performance, it is important to establish a work environment so that hospital nurses can perform their work with faith in their supervisors, as well as having superiors who are ethical leaders.

        만성질환자의 e-헬스 리터러시가 의료인-환자 커뮤니케이션에 미치는 영향

        Purpose: The purpose of this study was to investigate the influence of e-health literacy in patients with chronic diseases on their healthcare provider-patient communication. Methods: The participants were 184 patients with chronic illnesses admitted to a general hospital. Using a structured-questionnaire, data were collected from December 1 to 31, 2018. Data were analyzed using descriptive statistics, t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and multiple linear regression with IBM SPSS/WIN 22.0. Results: Healthcare provider-patient communication was positively correlated with e-health literacy (r=.87, p<.001). Factors influencing healthcare provider-patient communication were e-health literacy (β=.87, p<.001), subjective health statues (β=.17, p=.011), health concerns (β=-.15, p<.001), number of questions during the hospital treatment (β=.14, p=.003), and education (β=-.14, p=.048). The input variables explained 70.8% of healthcare provider-patient communication. Conclusion: The results of this study suggest that healthcare providers should provide reliable online health information to patients and encourage health-oriented attitudes in order to communicate about health information that patients obtained from online.

        잠재감염에 의한 HBsAg 음성 B형 간염 급성 악화 1예

        Many studies have recently reported reactivation in hepatitis B surface antigen (HBsAg)-negative subjects with the use of biologic agents such as rituximab. However, occult hepatitis B virus infection itself has little clinical impact. We experienced a case of acute exacerbation caused by occult hepatitis B infection without HBsAg seroconversion. No mutation was found on the major hydrophilic loop of the S protein. The patient recovered from acute exacerbation after antiviral therapy. In conclusion, acute exacerbation can be induced by occult hepatitis B virus infection itself without reactivation. In such a case, antiviral therapy should be considered.         간호사가 지각하는 내부마케팅활동 정도가 간호사의 이직의도, 간호업무수행 및 간호업무생산성에 미치는 영향

        The purpose was to analyze the effects of internal marketing activity factors on nurse's turnover intention, nursing task performance and nursing productivity. Methods: The subjects were 355 nurses who were working at the 3 universities hospital over 1 year. The instruments were used of internal marketing activity factors(Lee, 2001), turnover intension(Lee, 1995), nursing task performance(Park, 1988) and nursing productivity(McNeese-Smith, 1996). Results: The mean score of internal marketing activity factors was 2.79, education and training 2.97, individualization 2.93, communication 2.87, promotion 2.76, work environment 2.63, reward system 2.62, and management vision for employee 2.61. The turnover intention was 3.12, nursing task performance 3.49, and nursing productivity 3.38. The internal marketing activity factors were negatively correlated with turnover intention(r=-0.37, p<0.0001), and positively correlated with nursing task performance(r=0.29, p<0.0001) and nursing productivity(r=0.30, p<0.0001). The key predictor of turnover intension was reward system, education and training, communication, and salary. They explained 35.0% of the total variance. In nursing task performance, communication, management vision for employee, salary and unit explained 26.0% of the total variance. In nursing productivity, communication, reward, education and training, salary, and position explained 24.0%. Conclusions: To increase nurse's nursing task performance and nursing productivity and to decrease turnover intention, it is necessary to concentrate on improving communication and reward system in the internal marketing activity factors. Through these activities, the effectiveness of internal marketing strategies will be enhanced and finally, nursing organizational outcome will be increased.
