The antimalarials have been commonly described for the treatment of several connective tissue diseases. Among adverse effects of antimalarials, skin hyperpigmentation has been easily overlooked for its slow onset, but it may be a marker at risk of ocular side effects. Moreover, most cases of hyperpigmentation have been reported from chloroquine, but rarely hydroxychloroquine (HCQ). We experienced two patients who showed skin pigmentation during administration of HCQ. The first case was a 53-year-old woman presented with dark brown macules on the neck and ashy-colored patches on the buttock. The second was a 75-year-old woman presented with bluish-gray patches on both shins. Before hyperpigmentation developed, each patient had been treated with HCQ 400 mg per day for two years and five months, respectively. Two years ago after skin hyperpigmentation the first patients was found to have retinal pigmentation under the fundoscopy, so that HCQ was discontinued. (Korean J Dermatol 2011;49(8):743∼747)
인삼 부산물로부터 β-Glucosidase 생성 효모의 분리 및 특성

β-glucosidase is one of the members of the cellulase enzyme system, β-glucosidase producing microorganisms are potential sources that can be employed for the bioconversion of cellulose. We isolated 10 strains from the ginseng byproduct. These isolates were confirmed to produce extracellular enzymes (protease, amylase, cellulase, and β-glucosidase). Phylogenetic analysis of 10 strains confirmed that one yeast belongs to the Rhodotorula, nine bacterial strains belong to Pseudomonas (7 strains), and Brachybacterium (2 strains). The β-glucosidase properties of one yeast isolate identified as Rhodotorula sp. were characterized. The β-glucosidase activity of Rhodotorula sp. GPY-1 was assessed at 30°C, and stability showed a range of 20~40°C. The optimum pH showed the highest activity at 5.0, and enzyme stability was maintained up to pH 5.0~8.0. In addition, Rhodotorula sp. demonstrated plant-growth-promoting activity (siderophores, N2-fixing ability, IAA production, and phosphorus solubilization), protease, cellulase activity, and antibacterial activity against the ginseng root rot disease caused by Botrytis. Therefore, Rhodotorula sp. GPY-1 may have some application potential for ginseng cultivation and the production of effective components.
아토피피부염 환자에서 Cyclosporine 치료용량 및 재발인자에 대한 후향적 연구

Background: The clinical efficacy of cyclosporine in the treatment of atopic dermatitis is well documented. However, the optimal dose regimen has not yet been established, and there are few published data about the factors related with relapse after cyclosporine therapy. Objective: The purpose of this study was to provide clues for the proper use of cyclosporine and identify contributing factors for relapse in atopic dermatitis. Methods: We performed a retrospective analysis of 174 patients with atopic dermatitis who had been treated with cyclosporine in our clinic between January 2007 and January 2012. Results: The mean starting dose of cyclosporine was 4.8 mg/kg, and the relief of symptoms was initially achieved with a dose of 5.0 mg/kg. Treatments were continued for 137.2 days with a cumulative dose of 388.0 mg/kg. After discontinuation of cyclosporine, 79/167 (47.3%) patients experienced a relapse, and the duration of remission was 288.6 days. In a multiple regression analysis, serum IgE (p=0.015), starting dose of cyclosporine (p=0.010), and maintenance therapy (p<0.001) were independent and significant factors related to relapse; the relapse and serum IgE, starting dose of cyclosporine showed negative relationship, and maintenance therapy considerably reduced the recurrence rate (odds ratio, 0.04). Conclusion: Our results indicate that higher starting dose of cyclosporine may decrease the relapse rate in atopic dermatitis. In the post-cyclosporine period, subsequent maintenance therapy should also be considered for sustained remission. At the beginning of cyclosporine therapy, initial serum IgE would be a good laboratory index for predicting the risk of relapse.

As China's WTO accession promises greater market access and a more predictable commercial environment, new entrants will be encouraged to enter China's market. Though companies are finding commercial opportunities in China, there are many potential pitfalls companies should be aware of, including issues related to the protection of intellectual property. Since joining the WTO, china has Strengthened its legal framework and amended its Intellectual Property, Rights and related laws and regulations to comply with the WTO Agreement(TRIPs). Despite stronger statutory protection, China continues to be a haven for counterfeiters and pirates. This paper indicates that China's current Intellectual Property Rights law systems and infringement cases are focused on patents and trademarks. In conclusion, this paper provides practical countermeasures for business on infringement of olina’s intellectual property rights.

Linear atrophoderma of Moulin is a rare acquired atrophic skin condition following the lines of Blaschko. It is characterized by asymptomatic, band-like, hypopigmented atrophoderma, without obvious induration or sclerosis. Usually, the lesions begin in childhood or adolescence and are localized on the trunk or extremities with a unilateral distribution. Extensive lesions with bilateral involvement are extremely rare, and only three cases affecting patients bilaterally have been previously described. We herein present the case of a 17-year-old boy with multiple hyperpigmented atrophic patches, arranged in a linear pattern on both sides of the trunk, right buttock and leg, and left arm. Diffuse-type giant cell tumor of tendon sheath is an uncommon subtype of giant cell tumor of tendon sheath that shows destructive proliferation of synovial-like mononuclear cells, admixed with multinucleate giant cells, foam cells, siderophages and inflammatory cells. This tumor is commonly located in the periarticular soft tissues of knee, thigh and foot, but on rare occasions it can be purely subcutaneous or intramuscular. Diffuse-type giant cell tumor of tendon sheath affecting the buttock is extremely rare and only two cases, periarticular and intramuscular, have been described in the literature. We report the case of a 63-year-old male with diffuse-type giant cell tumor on the subcutaneous tissue of buttock. (Korean J Dermatol 2011;49(6):542∼545)

Bowen`s disease, one of squamous cell carcinoma in situ, typically presents as a discrete, slowly enlarging, pink to erythematous thin plaque with well-demarcated, irregular borders and overlying scale or crust. Several clinicohistopathologic variants have been illustrated to date, but there have never been reported to Bowen`s disease manifesting with pedunculated tumor. Herein, we report an interesting presentation of pedunculated Bowen`s disease on the right groin. In the resected tumor specimen, human papillomavirus type 68, one of the mucosal high-risk human papillomavirus, was demonstrated.

Pityriasis rotunda (PR) is a rare disease characterized by persistent, sharply defined, oval, scaly patches of dry skin, localized mainly on the trunk and extremities. Its etiology remains unknown. However, several reports suggest that it is a form of acquired ichthyosis vulgaris or a skin manifestation of systemic disease, such as malnutrition, chronic illness, hepatic disease, and malignancies. Although a variety of treatment modalities, including topical lactic acid, urea, tars, emollients, and corticosteroid, have been applied to it, their efficacies are not satisfactory. Herein, we report a case of PR in a healthy man who was successfully treated with oral and topical retinoids.
