Unreduced fracture-dislocation of the hip in a polytraumatized patient is usually the results of a traffic accident. The threatening emergencies are dealt with first, and the solution of the hip problem is sometimes postponed or even neglected. The patients come for treatment several months after the accident. Surgical reduction and acetabular reconstruction at this stage are almost impossible because of acetabular bone loss, fibrosis, and avascular ner.rosis of the femoral head. Recent concept is that total hip arthroplasty can be the solution for a patient suffering from a persistent to perform the procedure by a one-staged operation. The authors report four cases of unreduced fracture-dislocation of the hip treated by hip arthroplasty.

There has been only some guidelines for treatnents of concomitant ipsilateral fractures of the humerus and forearm both bone, so-called floating elbow. The authors reviewed 11 patients of floating elbow who were treated at Chonbuk National University Hospital between January 1984 and June 1991. The results were as follows: 1. Average age was 28 years old and the most common cause of injury was belt injury 2. The most common combined injury was nerve paralysis and we treated it with relatively good result by conservative treatment. 3. The method of internal fixation using plate & screws showed better result than other method of treatments. 4. In younger patients, the better results were obtained than older patients.

The tribial spine or intercondylar eminence is the nonarticular bony prominence between the articular surfaces of the medial and lateral plateau of the tibia and serves as an attachment for the cruciate ligaments The most significant aspect of the fracture of the tribial spine is that it might disturb the integrity of the cruciate ligament. We reviewed and analyzed 19 tibial spine fracture which had been treated at the department of orthopedic surgery. Chonbuk National University Hospital from January 1987 to December 1990.

Fracture of the tibial shafte is the most common fracture of the long bones and there is much controversy concerning the method of treatment because of many complications, such as non-union, delayed union and infection. We reviewed and analyzed 68 tibial shaft fractures which had been treated at the Department of Orthopedic Surgery, Chonbuk National university Hospital from January 1988 to December 1990. We divided the cases into three groups, plating, flexible intramedullary and rigid intramedullary nailing, in order to determine the more adequate method of treatment of tibial shaft fracturs. The results were as follow: 1. In both groups, bone healing time was shorter in the patients under thirty years old and it was not affected by the fibular fracture, but all of the delayed and nonunions occured in the cases having concomitant fibular fracture. 2. The rate of fracture union was not affected by the fracture location and the worst prognosis among the fracture types was for segmental and comminuted fracture. 3. The average union time was 15.1 weeks in the plating group, 13.6 weeks in the flexible intramedullary nailing group and 12.5 weeks in the rigid intramedullary nailing group. 4. The time for achieving full range of motion and full weight bearing and fracture healing was less in the intramedullary nailing group. We conclude that intramedullary nailing is a more suitable method for the treatment of the tibial shaft fractures.
