최근 전 인류를 위협하고 있고, 급속하게 퍼지고 있는 후천성면역결핍증후군(Acquired Immune Deficiency Syndrome:AIDS)은 아직 그 병에 대한 적극적이며 효율적인 대응책이 없는 현금, 관심의 촛점이 되는 공포의 병이다. AIDS란 CDC(Centers for Disease Control)의 정의에 의하면 뚜렷한 원인을 규명할 수 없는 세포매개면역에 결함이 진단되면서 한가지 이상의 기회 감염 또는 Kaposi 육종이 동반되는 경우를 일컫는다. 기존 알려진 AIDS의 병태는 Restrovirus일종의 HTLV-III/LVA의 감염으로 일어나며 이 virus는 우선적으로 T-helper cell을 공격하여 면역계에 치명적인 손상을 줌으로서 세포성 및 체액성면역반응에 이상을 초래할 뿐 아니라, 기타 숙주방어 기전에 중요한 기능 또한 저하된다고 한다. 전 세께적으로 문제가 되고 있는 이 질환이 우리나라에서도 이미 항체양성자가 밝혀져 AIDS virus에 폭로된 지반임이 증명되었다. AIDS는 대부분의 기타 전염병의 경우와는 달리 항체보유자가 바로 AIDS 바이러스 보균자이며, 이들이 가장 위험한 전염원이기 때문에 항체 양성자의 존재란 실제 환자 발생 이상으로 중요하다. 또한 동성연애자에서 주로 발생하던 이 병은 이제 이성관계에서도 증가하는 추세에 있고, 특히 모체로부터 태아에게로 감염되어 AIDS에 이환된 어린이가 많이 증가하고 있다는 사실은 산부인과 영역에서도 조속하게 지대한 관심을 가져야 할 병이다.

Prostaglandin E2 경구투여의 진통유발에 관한 임상적연구

It has been amply demonstrated that PGE2 and PGF2α are effective in evoking myometrial contraction whether the uterus is pregnant or not. Since 1971 when Karim and Sharma reported that labor could be induced by oral administration of PGE2, many clinical reports were published with different results. A clinical study was performed to know the clinical reaction of oral PGE2 for induction of labor. Induction of labor was carried out in 50 patients between 36 and 44 weeks of normal gestation. The results were as follows: 1. Induction succeeded in 75% of primigravidas, in 90% of multigravidas and the overall success rate was 84 per cent. 2. Failure rate in the group with pelvic score 0-4 was 36.8% while the same rate was 3.2% in the group with pelvic score 5 or more. 3. The failure rate was 19.5% in the group with intact membrance, while no failure was observed in the group with premature of membranes. 4. Lag period was 93.5 minutes in primigravidas and 61.9 minutes in multigravidas on the average. The mean induction delivery time was 9.3 hours in primigravidas and 6.6 hours in multigravidas. 5. The mean total dosage of oral PGE2 was 4.35mg in primigravidas, 3.42mg in multigravidas, and overall 3.89mg. 6. In only 4 cases, mild and transient nausea appeared, which was spontaneously subsided without specific management. 7. During introduction of labor, no significant changes occurred in blood pressure and pulse rate, and newborn babys` condition were excellent with Apgar score 7 or more.

It has more serious prognosis than either pure squamous cell carcinoma or adenocarcinoma. A case of Adenosquamous Cell carcinoma of cervix is presented with brief review of literature.

저자들은 서울대학교병원 산부인과에서 음핵부위에 발생한 Bowen`s Disease 1예를 치험하였기에 간략 한 문헌고찰과 아울러 증례를 보고하였다. Bowen`s disease is a chronic disorder with cutaneous plaque representing intraepidermal squamous cell carcinoma. This disease entity was originally described by Bowen in 1912, which 2 cases of skin lesion were reported . Hudelo et al, in 1922 first described as Bowen`s disease of the vulva. the lesion occurs at any epithelium including the cervix, vagina, vulva. perineum and anus. We experienced Bowen`s disease occuring over the clitoris, and a case of Bowen`s disease of the clitoris is presented with a brief review of the concerned literature.

임신과 동반된 Krukenberg종양에 의한 남성화 Krukenberg tumor of the ovary associated with hormonal activity are rare, especially during pregnancy. In recent years, we have experienced 1 case of bilateral Krukenberg tumor of the ovary causing virilization during pregnancy and its primary site, of course, is stomach. And so, the literatures on Krukenberg tumor of the ovary causing masculinization during pregnancy are reviewed briefly, in addition to reporting this case.

공배양이 생쥐 2-세포기 배아의 발생에 미치는 영향에 관한 연구

목적: 체외수정 프로그램의 낮은 성공률은 여러 가지로 설명될 수 있겠으나 이 중에서도 배아 이식시 이식하는 배아와 자궁내막과의 이질성이 가장 큰 한계점으로 지적되고 있다. 본 연구는 생쥐 2-세포기 배아를 Ham`s F-10과 m-KRB 배양액 내에서 Vero 세포와 인간의 양막 세포와 각각 공배양함으로써 배아의 발생과정을 관찰하고, 이를 공배양하지 않은 대조군과 비교분석하여 배아의 발생에 미치는 공배양의 효과를 알아보고자 한다. 연구 방법: 생쥐 제1대 잡종(C57 BL×CBA)의 2-세포기 배아를 단순 배양액(m-KRB)과 복합 배양액(Ham`s F-10) 내에서 체외 배양시 Vero 세포와 인간 양막세포를 함께 각각 공배양하여 포배기 배아로의 발생 및 세포수를 관찰하고 공배양하지 않은 대조군과 비교 분석하였다. 실험 결과의 통계학적 유의성은 X2-test와 Fisher`s exact test를 사용하여 P<0.05를 유의성으로 검증하였다. 결과: 단순 배양액(m-KRB)에서 2-세포기 배아로부터 포배기로의 발달 및 부화배반포로의 부화율은 대조군 및 Vero 세포 공배양군과 양막 세포 공배양군을 비교할 때 양막 세포 공배양군에서 유의하게 낮았다(P<0.01). 복합 배양액(Ham`s F-10)에서 2-세포기 배아로부터 포배기로의 발달 및 부화율은 세 군간에 유의한 차이가 없었다. 복합 배양액(Ham`s F-10)에서의 포배기 배아당 세포수는 공배양군이 공배양을 실시하지 않은 대조군에 비해 유의하게 많았다(P<0.05). 결론: 이상의 결과로 미루어 보아 공배양이 배아의 시기적 성장발달은 촉진시키지 않지만 공배양된 배아의 질은 향상시킨다고 생각된다. 따라서 체외수정 프로그램시 공배양을 이용하면 좋은 결과를 가져올 것으로 생각된다. Objective: Early transfer has led to asynchrony of the endometrium-trophectoderm interaction between the embryo and uterine environment. A culture system which could prolong culture time and increase embryonic cleavage rate and viability would improve the success rate. The purpose of this study was to evaluate the in vitro developmental promoting effect of coculture system on 2-cell stage mouse embryos. Methods: In this experiment, we have cocultured mouse embryos with Vero cells and with amniocytes in m-KRB and Ham`s F-10. Results: In m-KRB, The hatching rate of embryos cocultured with Vero cells (71%) was significantly increased compared with that of embryos cocultured with amniocytes (36%). In Ham`s F-10, the development rate of 2-cell stage mouse embryos showed no significant differences in three groups (control 89%; Vero cells 92%; amniocytes 95%). And also the cell number of expanded blastocyst in experimental groups was significantly greater as compared with that of the control group (P<0.01). Conclusion: Coculture system improve the quality of mouse embryos and might increase the implantation and pregnancy rate.
