복강경하 전자궁절제술에서 자궁의 무게에 따른 이환율의 비교

목적: 자궁의 무게에 따른 복강경하 자궁절제술의 합병증을 비교 분석하여 복강경하 자궁절제술의 안정성을 알아보고자 하였다. 연구 방법: 2008년 1월부터 2009년 6월까지 본원 산부인과에서 양성 자궁 질환으로 복강경하 자궁절제술 시행 받은 265명을 대상으로 하였다. 자궁 무게를 180 g 미만인 60명 (Group 1), 180 g 이상 350 g 미만인 141명 (Group 2), 350 g 이상인 64명(Group 3)으로 나누어 비교하였다. 수술 후 의무기록을 이용하여 후향적 분석을 통해 자궁 무게에 따른 이환율의 상관관계를 알아보았다. 결과: 각 군간 나이, 분만력, 체질량 지수, 골반수술 병력, 내과적 병력에서는 각 군간의 유의한 차이는 없었다. 수술 중, 수술 후 합병증에서도 유의한 차이는 없었으나 자궁의 무게가 클수록 수술시간이 길었다. 결론: 복강경하 자궁절제술은 거대 자궁에서도 안정적으로 시행될 수 있으므로 단순히 자궁의 무게가 복강경하 자궁절제술시행의 기준이 되어서는 안 된다. Objective: To evaluate the effect of uterine weight on short-term outcome of total laparoscopic hysterectomy (TLH) for benign gynecological conditions. Methods: A retrospective medical records review of 265 cases of patients with TLH was performed. Patients included in this study underwent TLH as benign uterine disorders at Kangnam Sacred Heart Hostpital, Hallym University from January 2008 through June 2009. These patients were stratified into three groups; Group 1 patients with uterus weighing less than 180 g (n=60), Group2 patients with uterus weighing 180 g to 350 g (n=141), Group 3 patients with uterus weighing more than 350 g (n=64). The groups were compared as regard postoperative stay, operative time, estimated blood loss, hemoglobin change, conversion to open surgery, and postoperative complications. Results: There was no significant difference in age, gravidity, body mass index, previous pelvic surgery and past medical history. The overall complication rates were not significantly different. But operative time prolonged as uterine weight increased. Conclusion: The TLH can be performed successfully in case of enlarged uterus. Therefore the enlarged uterus is not an absolute contraindication for TLH.

Women with severe preeclampsia or eclampsia who develop pulmonary edema most often do so postpartum and some of these women have cardiac failure. Peripartum cardiomyopathy is defined as a cardiac failure occurring in the latter part of pregnancy or in the peripartum, without obvious cause and prior evidence of heart disease. It is very rare but the mortality rate is as high as 10~50%. Here, we describe a case of a woman who has undergone emergency Cesarean section due to complicated severe preeclampsia with acute pulmonary edema and peripartum cardiomyopathy at 33+1 weeks of gestation.

본 논문에서는 무인헬기에 적합한 새로운 형태의 점항법 유도법칙을 소개한다. 본 점항법 유도법칙은 미래의 예측 위치에 기반하여 자동제어부에서 사용할 속도와 방위각 명령을 생성해낸다. 무인기의 뱅크각을 통해 간접적으로 비행방향을 변경하는 기존의 점항법 유도법칙과 달리 본 유도법칙은 무인헬기의 속도백터를 직접 변경하도록 속도 명령을 생성해낸다. 제안된 유도법칙의 검증을 위해 소형 무인헬기용 비행제어시스템을 개발해 비행시험을 수행하였으며, 개발된 비행제어시스템은 비행제어 컴퓨터와 항법센서, 그리고 지상관제국으로 구성되어 있다. 비행시험을 통해 제안된 유도법칙이 성공적으로 작동함을 확인하엿으며, 향후 경로추종 비행에 확장ㆍ적용할 계획이다. This paper presents a new point navigation guidance law which is useful for unmanned helicopters. Predicting the future position, the guidance law generates velocity and heading commands, which are used as input to autopilot. This method differs from conventional guidance law in that it reorients the direction of flight velocity vector directly, not by bank angle indirectly. For flight tests, we have developed a flight control system for a R/C helicopters. The system consists of a flight control computer, navigation sensors, and a ground station. The results of the test show that the proposed law guides a unmanned helicopter along a line path within a given area. In the future, we are planning to extend the guidance law to the mission of path following, i.e., waypoint navigation.

Porcine circovirus type 2 (PCV2) is associated with porcine circovirus diseases (PCVD), of which postweaning multisystemic wasting syndrome is considered to cause considerable economic losses in pig industry worldwide. As the virus-like particle (VLP) is a highly effective type of subunit vaccine and has unique advantages in terms of safety and immunogenicity, this study aimed to develop PCV2-like particles, which matched currently circulating Korean PCV2 and were applicable as vaccines. The ORF2 genes encoding PCV2 capsid protein were amplified from the PCV2 subgroup 1A/B Korean C275 isolate and the subgroup 2E C94 isolate by PCR assay with primer pair specific to PCV2 ORF2 gene, and were cloned into baculovirus transfer vector. Recombinant baculovirus was generated by cotransfection with the transfer vector and linear baculovirus DNA into the Sf9 cells, and then by plaque purification. Expression of PCV2 capsid protein was determined by the indirect immunofluorescence and Western blotting assays, and electron microscopy. By both immunological assays, PCV2 capsid antigen was detected in the Sf9 cells infected with the recombinant baculoviruses. The formation of empty virus particles, characteristic of VLP, was detected in the lysate of Sf9 cells infected with the recombinant baculoviruses by negative electron microscopy. From these results, VLPs of two genogroups of PCV2 were successfully expressed and generated in a baculovirus expression system. It is expected that the expressed VLPs of two genotypic groups can be used for control of PCV2 infection as good vaccine candidates.
