      • 구강 편평세포암종에서 E-cadherin 및 β-catenin 발현의 변화 : Correlation with Histologic Features and p53 Expression

        Objective : Altered expression of cell adheion molecules is associated with biologic behavior of tumor. The aim of this study is to evaluate the expression pattern of E-cadherin, β-catenin and p53 protein according to histologic grade and invasion pattern of squamous cell carcinoma of the oral cavity and the relationship between E-cadherin, β-catenin and p53 protein expression. Methods and Material : The materials were fifty seven cses of squamous cell carcinoma, and clinicl parameers such as age, sex, tumor location, stage and recurrence were recorded. Histologic review was done based on histologic grade and invasion pattern(nodular vs infiltrative). Immunohistochemical stains for E-cadherin and β-ca tenin were interpreted based on staining pattern as los of membranous expression and cytoplasmic expression and p53 protein expression was defined as positive if more than 10% of nuclei were reactive. Statistical analysis between E-cadherin, β-catenin and p53 protein expressions and histologic grade and invasion pattern and the relationship between E-cdherin, β-catenin and p53 protein expressions were perfomed. Results : There wa positive correlation between higher histologic grade and infiltrative pattern. Loss of membranous expression and cytoplasmic expression of E-cadherin were noted in 33.3% and 57.9%, respectively, and loss of membranous expression of E-cadherin showed increaing tendency in poorly differentiated carcinomas, however, there was no significant difference of cytoplasmic expression rate of E-cadherin according to histoogic grade. Altered expression of E-cadherin was more frequent in poorly differentiated carcinomas. Loss of membranous expression and cytoplasmic expression of E-cadherin were more frequent in carinomas with infiltrative pattern than in carcinomas with nodular pattern, but their diferences were not significant, Loss of membranous expression and cytoplasmic expression of β-catenin were observed in 19.3% and 80.7%, respectively. Loss of membranous expression of β-catenin howed no positive correlation according to histologic grade and invasion pattern, but cytoplasmic expression rate of β-catenin was higher in poorly differentiated carcinomas and in carcinomas with infiltrative pettern. p53 protein expression rate was 45.6%, and showed invreasing tendency in poorly differentiated carcinomas, but no significant relationship with invasion pattern. There was an inverse relationship of loss of membranous expression and cytoplasmic expression of E-cadherin and β-catenin. Altered expression of E-cadherin was related to cytoplasmic expression of β-catenin, however, there were no significant relationship between altered expressions of E-cadherin and β-catenin and p53 protein expression. Conclusion : In squamous cell carcinomas of the oral cavity, altered expression of E-cadherin and β-catenin was more frequent in poorly differentiated carcinomas and in carcinomas with infiltrative pattern and there was positive correlation betwween altered expression of E-cadherin and β-catenin. These findings suggest that altered expressions of E-cadherin and β-catenin may have a role in the development of squamous cell carcinomas with aggressive biologic behavior. but altered explosions of E-cadherin and β-catenin. These findings suggest that altered expressions of E-cdherin and β-catenin my have a role in the development of squmaous cell carcinomas with aggressive biologic behavior. But altered expression of E-cadherin and β-catenin might not be related to p53 protein expression. Further study on genetic mutation related to altered expression of E-cadherin and β-catenin will be needed

      • 구강에 발생한 편평상피암종의 MMP-2, MMP-9, TIMP-1, TIMP-2 발현과 EGFR 및 E-cadherin 발현의 상호연관성

        Objective : Matrix metalloproteinases(MMPs) are involved in the degradation of extracellular matrix, which is related to infiltrative growth and metastasis of tumor. Positive correlation between MMPs expression and clinico-pathologic parameters were reported, and MMPs expressions were regulated by tissue inhibitor of metalloproteinases(TIMPs) or cell adhesion molecules such as E-cadherin and epidermal growth factor receptor EGFR). The aim of this study is to evaluate the relationship between MMP-2, MMP-9 expressions and clinico-pathologic factors such as clinical stage. recurrence. histologic grade and invasion pattern, and between MMP-2 and MMP-9 Expressions and TIMP-1, TIMP-2, and E-cadherin expressions. Methods and Material : The materials were 55 cases of squamous cell carcinoma of the oral cavity, and immunohisdtochemical studies for MMP-2, MMP-9, TIMP-1, TIMP-2, E-cadherin and EGFR were performed. Clinico-pathologic factors such as clinical stage, recurrence, histologic grade and invasion pattern were recorded through microscopic examination and clinical record review. Statistical analyses between MMP-2, MMP-9 expressions and clinico-pathologic factors, TIMP-1, TIMP-2 expressions and EGFR and E-cadherin expressions were done. Results : 1. MMP-2 and MMP-9 expressions were noted in 30(54.5%) and 22(40.0%) of 55 cases, respectively. TIMP-1 and TIMP-2 expressions were observed in 21(38.2%) and 33(60.0%), and E-cadherin and EGFR expressions were found in 35(63.6%) and 26(47.3%) of 55 cases, respectively. 2. MMP-2 expression rate was slightly higher in cases without recurrence, and TIMP-2 expression rate was slightly higher in cases showing more infiltrative growth pattern. However, there were no significant differences of MMP-2, MMP-9, TIMP-1, and TIMP-2 expression rates according to other clinico-pathologic parameters were noted. 3. The expression rate of EGFR was higher in cases with well differentiation (p=0.0047), but no positive relationship between the expression rate of E-cadherin and histologic grade was found. There were no significant differences of the expression rates of E-cadherin and EGFR according to other clinico-pathologic parameters. 4. Cases with positive reaction for MMP-9 showed an increasing tendency of negative reaction for TIMP-1. However, signigicant relationships between MMP-2, MMP-9 expressions and TIMP-1, TIMP-2 expressions were not noted. 5. The expression rate of MMP-2 was higher in cases with positive reaction for Ecadherin and EGFR with no statistical significance. The expression rate of MMP-9 was significantly higher in cases with positive reaction for E-cadherin(p=0.0221), but there was no significant relationship between MMP-9 and EGFR expression. Conclusion : These results suggest that MMP-2, MMP-9, TIMP-1 and TIMP-2 expressions are involved in the development of oral squamos cell carcinomas, but MMP-2, MMP-9, TIMP-1, and TIMP-2 expressions might not seem to be a sueful prognostic factors because there were no significant relationship between clinicopathologic parameters, EGFR expression showed positive correlation with low histologic grace, so EGFR expression could be regarded as a good prognostic factor. In the progression of squamous cell carcinoma of oral cavity, MMP-9 and E-cadherin might be involved with positive correlation, but there were no significant correlations between MMP-2, MMP-9 and TIMP-1, TIMP-2 and EGFR expressions rate. Further study for other factors such as cytokines or growth factors except TIMP-1, TIMP-2 and EGFR will be needed.

      • 법랑아세포종에서의 p53 및 MIB-1 발현 : 임상 및 병리학적 요인들간의 연관성 Correlation Between Clincopathologic Parameters

        Thirty five patinets' paraffin embedded biops block , which was previously diagnosed ameloblastoma and obtained from oral and maxillofacial surgery dept. of pusan paik hospital and OMS dept of collage dentistry, pusan national university hospital, from January 1994 to march 1999, were performed immunohistochemistry with p-53 and MIB-1. On investigating the expression, correlation between clinical and histologic factor and correlation between prognosis and moleculopathologic property were examined. And we obtained the following results; 1. Age over forty, recurrence rate was 58.5%. 91.4% of the tumor was developed on mandibule, recurrence rate of maxilla area tumor was 66.7% On radiologic and histologic feature, multilocular and follicular type has high recurrence rate but no significance statistical difference was found 2. Expression of p-53 was 25.7%, When underage of forty. location on mandibule, multilocular and plexiform type, the expression of p-53 was somewhat higher and recurred cases of tumor, expression rate was 12.5%. 3. MIB-1 expression rate was 77.1%. When location on mandibule, unilocular, plexiform type tumor and previously recurred case, MIB-1 expression was 87.5% which was somewhat higher than no recurrence case of 74.1%. 4. 33.3.% of positive on MIB-1 have expression of p-53 and all p-53 positive case express MIB-1 positive. All p-53 positive case have high cell proliferation rate. From this result, age and location have closely related with the prognosis of tumor, histologic feature and MIB-1 expression has related with the prognosis of tumor. But p-53 has highly expressed on cell proliferation area, not closely related with the prognosis of tumor.
