Intracranial epidermoid tumors usually show homogeneous hypodensity on CT scans and hypointensity on T1-weighted and hyperintensity on T2-weighted MR images. Epidermoid tumors are most often located in the cerebellopontine angle and in the parasellar region. We report an unusual case of middle cranial fossa epidermoid tumor. The tumor showed inhomogeneous low density on CT scan and heterogeneous signal intensity on both T1- and T2-weighted MR images. Epidermoid tumors are usually slow growing tumor and are located in adjacent region of cranial nerve, so we must consider age and postoperative neurological defici of patient before operation.

Objectives : Although posterior lumbar fusion operations had been reported to accelerate spinal degeneration, there have been only a few studies for their biomechanical effects. We have studied the change of motion range at the vertebral joint one level above the fusion(UVJ) in pedicle screw fixation group(PSF)(n=13) where facet capsule was destroyed and in posterior lumbar interbody fusion group(PLIF)(n=8) where it was spared. Patients and Methods : The patients were divided into early(3 to 6 months) and late(over 12 months) according to postoperative follow-up period. The flexion, extension and flexion-extension angles(FA, EA, FEA) were measured at the UVJ with pre-operative, early and late post-operative films. Results : Mean age and male to female ratio were $52.7{\pm}9.3$ and 1 : 3.2. Mean follow-up periods were $144.1{\pm}30.0$ and $528.8{\pm}160.3$ days in early and late groups, respectively. The FEA and FA in the late PSF($11.8{\pm}3.1$, $8.5{\pm}2.9$) were significantly greater than pre-operative angles($7.8{\pm}3.9$, $5.1{\pm}3.7$)(p<0.01, p<0.05). All angles in the PLIF showed no significant changes with time. The FEA and FA in the late PSF($11.8{\pm}3.1$, $8.5{\pm}2.9$) were significantly greater than those of the late PLIF($7.6{\pm}2.3$, $3.4{\pm}2.0$)(p<0.01, p<0.001). All angles at early follow-up period were similar between PSF and PLIF. The EA showed no significant change in relation with follow-up period or fusion method. Conclusion : As a result, the facet capsule injury in pedicle screw fixation seems to be related with increased flexion angle or degeneration of the adjacent joint above the fused vertebra in the late phase.

Optic nerve sheath meningioma is a rare tumor originating from meningothelial cap cells of the arachnoid membrane surrounding the intraorbital optic nerve. Because optic nerve sheath meningioma grows slowly and presents benign clinical behaviors which do not cause death, observation with regular evaluation is usually considered prior to interventional treatment. However, some reports indicated that progressive visual loss in one eye and gradual progression into bilateral involvement with intracranial extension confer blindness of both eyes. Therefore, optimal management should be properly selected to produce superior clinical outcomes with the consideration of the visual function and tumor extent.

Although brain metastasis of renal cell carcinoma is a major cause of death in Von Hippel-Lindau disease(VHL), it is not easy to distinguish local recurrence of hemangioblastoma from distant metastasis. In addition, hemangioblastoma has several characteristics suitable for recipient of "tumor-to-tumor metastasis". Authors report a case of Von Hippel-Lindau disease that had metastases of renal cell carcinomas.

Objective : We report a case of primary intracranial leiomyoma in 43-year-old woman. The radiologic examination was consistent with an extradural meningioma. After surgery, the tumor was diagnosed as leiomyoma. Case presentation : A 43-year-old woman presented complaining of headache and tinnitus for few months. On examination, there were no focal neurologic deficits. Magnetic resonance image(MRI) of the brain showed a well enhancing solid tumor in right posterior parietal convexity. Surgical resection was performed and histologic was leiomyoma. Conclusion : Primary intracranial leiomyoma is rare intracranial tumor, rarely have neurologic symptoms. The radiologic diagnosis was similar with meningioma. Leiomyoma should be considered in the differential diagnosis of well-circumscribed intracranial extraaxial lesions.

Dumbell shaped schwannomas of hypoglossal nerve extending intra-and extracranially are very rare. The one case reports on a 54-year-old patient who presented with hoarseness and un a ware tongue atrophy. MRI showed inhomogeneous con-trast agent enhancement in a tumor in the cerebellomedullary cistern. The tumor extended midine and severely compressed brainstem. And it extended extracranially thorough the hypoglossal canal. The tumor was nearly total removed via suboc-cipital approach and the patient was discharged with improved hoarseness and no additional neurologic deficit.

Falcotentorial meningiomas are rare. The infratentorial supracerebellar and the occipital transtentorial approaches are frequently used to reach the tumor. A 56-year-old female was admitted to the hospital because of headache. Magnetic resonance imaging (MRI) of her brain showed a 35×38×20 mm-sized tumor, suspected as a meningioma, with heterogeneous signal intensity in the pineal region. The patient was operated using the occipital bi transtentorial/transfalcine approach. The observed tumor was a fibrotic mass with calcification and was not pulled to one side after internal decompression. It was totally removed through the bilateral approach. Immediately after surgery, however, the patient complained of total blindness. Following steroid therapy, the patient showed remarkable improvement. The occipital bi transtentorial/transfalcine approach provides a well-exposed operation field and an optimized visibility of anatomical structures. However, the potential visual cortex injury should be considered due to undue, prolonged retraction.

Meningiomas constitute about 15 to 18% of all intracranial tumors and are usally located in the subdural compartment. Intradural meningiomas may secondarily involve the overlying calvarial bone and provoke osteoblastic or, less commonly, an osteolytic reaction. Meningiomas developing outside the dura mater, without an intradural counterpart, are rare and have been reported in only 1 to 2% of all intracranial meningiomas. We present a rare case of coexisted subdural and extradural meningioma.

Carotid-cavernous fistulaes are traumatic in origin in over two thirds of all cases. Occlusion of these lesions by endovascular techniques or open surgical approaches is usually indicated. The current treatment of choice is occlusion by use of the intraarterial detachable balloon technique. Pseudoaneurysms. one of the complications after employment of the detachable balloon technique. develop at the balloon placement site after balloon deflation. Its incidence is approximately 44% in the literature. But peudoaneurysms are difficult to treat using endovascular techniques with detachable balloon or coils. The authors report that one case with a pseudoaneurysm who had abducens nerve palsy was treated successfully using Guglielmi detachable coils(GDC). His 6th cranial nerve palsy was improved within 6 weeks.
