신경섬유종증은 상염색체 우성 유전성 질환으로,17번 염색체의 장완에 위치한 종양 억제 유전자의 변성에 의해 발생한다. 이는 두가지 형이 있으며 신경섬유종의 발생은 제1형 신경섬유종증의 임상적 진단 기준 중 하나이다. 신경섬유종증의 임상적 증상으로는 피부병소,골변형,중추신경계의 종양 등이 있으며 환자의 25%가 구강내 신경섬유종을 보인다. 악골내 신경섬유종은 드물며 방사선학적으로 하악공,하악관,이공을 포함하고 단방성으로 잘 경계된 방사선 투과성으로 나타난다. 신경섬유종은 하나 또는 그 이상의 병소에서 신경육종으로 전이된다는 점에서 임상적으로 중요하며 현재 특이한 치료법은 없으나,외과적 절제술이 좋은 치료법으로 여겨지고 있다. 본 증례는 제1형 신경섬유종증 진단을 받은 환아로,구강내 신경섬유종의 외과적 절제 후 양호한 결과를 보여 이를 보고하는 바이다. Neurofibromatosis is an autosomal dominant disorder caused by a mutation of a tumor supressor gene on the long arm of chromosome 17. There are two types of neurofibromatosis, and development of neurofibroma is one of clinical diagnostic criteria for neurofibromatosis. The clinical signs of neurofibromatosis include as skin lesions, bone deformities, and tumors involving central nervous system. About 25% of neurofibromatosis involves oral neurofibroma. Radiographically, oral neurofibroma is well-defined unilocular radiolucency, which involves mandibular canal, mandibular foramen and mental foramen. When a lesion is small and approachable, complete resection, including lcm of marginal connective tissue, is feasible. However, there are studies reporting that the recurrence rate after surgical resection is high and frequent recurrence may even increase the risk of malignant transformation. This case reports a patient with neurofibromatosis type I, accompanying oral neurofibroma, who shows a favorable result after surgical resection of the oral lesion.

유치열이 혼합치열기를 거쳐 영구치열로 교환되는, 소아 및 청소년기에서는 맹출장애가 흔히 관찰된다. 이로 인해 치아의 맹출시기를 놓치게 되면 심미적 문제 인접치아의 전위 및 치근흡수, 부정교합 등의 많은 문제를 야기한다. 치아 중 제1대구치는 특히 교합 및 발육에 중요하게 관여하며 기능적, 형태적으로 건전한 교합의 발육 및 유지에 필수적인 치아이다. 그러므로 이들 이상증례를 조기에 파악하고 문제점에 대응하는 것은 보다 적절한 교합관리에 중요한 부분이라 할 수 있다. 상악 제1대구치의 맹출장애는 크게 맹출지연, 매복, 일차정체 및 이차정체로 나누어서 생각할 수 있다. 매복의 원인 중 물리적 장애물이 존재할 경우 우선 이를 제거하고 치아발육 상태에 따라 제거 후 관찰, 외과적 노출 또는 교정적 견인 등으로 치료가능하며 이소맹출에 의 한 매복일 경우 brass wire, pendulum applinace, 제2유구치 발치후 공간 유지 및 공간확보 등으로 치료를 할 수 있다. 본 증례들은 혼합치열기의 환아로서 맹출장애를 보이는 상악제1대구치의 진단 및 그에 따른 치료를 통해 양호한 치료결과를 보여 이를 보고하는 바이다. An eruption failure can be observed for child and adolescent periods when the primary dentition is changed to the permanent dentition through the mixed dentition frequently. The eruption failure can lead to miss erupting times of the tooth, then it will cause a lot of problems including root resorption, esthetic problem, transposition of adjacent tooth, malocclusoin and etc. Especially, the maxillary first molar is importantly concerned with occlusion and growth and is an essential tooth for development and maintenance of occlusion. So, it is a momentous part of more proper occlusal management to find these abnormal cases at the early stage and solve the problems. The sorts of eruption failures of the maxillary first molars can be divided into delayed eruption, impaction and the primary retention and the secondary retention. When physical obstacles cause impaction, first of all they must be removed then we can treat the impaction with observation after removal, surgical exposure or orthodontic traction. If the source of impaction is an ectopic eruption, the treatment can be a brasswire, a pendulum appliance, a space maintainer or space regainer after the extraction of the second deciduous tooth and etc. These cases are made a diagnosis of eruption failures of the maxillary first molars in mixed dentition period and have good prognosises after my treatments. So I reported them.
