대퇴 간부 협부하 골절의 초기치료에서 Poller 나사의 유용성

목 적: 본 연구의 목적은 대퇴골 간부 협부하 골절에서 원위 골절편에 Poller 나사 삽입을 통해 치료한 결과를 살펴보고자 함이다. 대상 및 방법: 대퇴간부 협부 이하 골절로 전향적 골수강내 금속정 삽입과 원위 골편에 Poller 나사 삽입을 통해 치료하였던 환자 중 술 후 최소 1년 이상 추시가 가능하였던 7명의 환자를 후향적으로 분석하였다. 남자는 4명 여자 3명, 평균 연령은 46.1세(20-72세)였다. 수술시간, 수술 후의 골 유합 시기, 전위 및 각변형 여부와 관절운동 범위 등을 평가하였다. 결 과: 7명의 전체 예에서 평균 19.1주(16-24주)에 방사선적 골 유합을 얻었다. 1예에서 5도의 외반 변형이 있었으며 1예에서 술 후 건측에 비해 15 mm 단축이 있어 보행 시 깔창 보조를 필요로 하였다. 전 예에서 고관절과 무릎의 정상범위의 관절운동이 가능하였다. 결 론: 대퇴골 간부의 협부 이하의 골절 또는 협부의 분쇄가 심한 환자의 초기 치료에서 Poller 나사를 이용한 전향적 골수강내 금속정의 치료는 원위 골편의 추가적인 안정성을 제공할 수 있어 유용한 방법이라 판단한다. Poller 나사 삽입 시 추가적인 20분이 소모되기는 하지만 대퇴골절의 해부학적 위치에 따라 그 사용을 고려하여야 할 것이다. Purpose: The purpose of this study was to evaluate the radiologic and clinical outcomes after intramedullary nailing with Poller screw insertion at initial stage in infraisthmal femur shaft fractures. Materials and Methods: Seven consecutive patients (7 femurs) treated with antegrade intramedullary nailing with Poller screw insertion for the infraisthmal femur shaft fracture were reviewed retrospectively. There were 4 male and 3 female patients. Mean age was 46.1 years (20-72 years). Operative time including Poller screw insertion, time for union, malalignment, and range of motion were evaluated. Results: All 7 cases had primarily healed successfully. Mean time for radiologic union was 19.1 weeks (16-24 weeks) postoperatively. One case had 5 degree valgus malalignment. One case of 15 mm shortening was reported and he required shoe lift orthosis. All cases had a full range of motion in hip and knee joint. Conclusion: Antegrade intramedullary nailing with Poller screw insertion is useful in the initial treatment of infraisthmal femur shaft fracture, because it could provide additional stability. An additional 20 minutes were required but a Poller screw should be considered according to the anatomic location of a femur shaft fracture.

The prevalence of metastatic tumorous lesion at medial clavicle or sternoclavicular joint is low. Herein, we presented a 45-year-old female patient with left sternoclavicular joint pain followed a weight-lifting exercise. The patient was initially diagnosed as traumatic sternoclavicular arthrosis, however, on later computed tomography scan demonstrated the bony destruction of medial clavicle which was diagnosed as metastasized non-small cell lung carcinoma after a fine needle biopsy.
