        TGF-α(Transforming Growth Factor-α)가 표피화에 미치는 효과에 대한 연구

        In the split thickness wound or burn, the wound is healed by epithelization. The two growth factors, epidermal growth factor(EGF) and transforming growth factor-α(TGF-α)play important roles in the epothelization. But the growth factor which has a more profound effect on the epithelization and the concentration of TGF-αthat has the most profound effect is unknown. So the authors observe the growth promoting activity and colony forming activity of EGF and different concetrations (0.1ng/ml, 1.0ng/ml, 10.0ng/ml, 100.0ng/ml) of TGF-α in human epithelial cell culture. And after the topical application of TGF-α and EGF to the 2nd degree burn on 30 Wistar rats, the rates of epithelization were observed. The results were as follows; 1. In epithelial cell culture, TGF-α has the most profound effect on the colony forming activity in the concetration of 0.1ng/ml. 2. In epithelial cel culture, TGF-αhas the most profound effect on the growth promoting activity in the concetration of 1.0ng/ml. 3. In vivo the TGF-αtreated wound showed faster epithelization than the EGF treated and nontreated wound. 4. In histologic examination of the epithelization, the granulation tissue proliferation and the neovascularization are markedly increased in TGF-αtreated wounds. In conclusion TGF-αhas a significant effect on epithelial cell proliferation and epithelization in vivo and vitro. And TGF-α has the most profound activity in the concentration of 0.1-1.0 mg/ml.

        Earlobe 결합 교정의 치험례

        Congenital earlobe defects fall into two categories : a simple cleft due to developmental malunion or a hypoplastic cleft with significant deficiency of the soft tissues. The location and size of congenital defects vary depending on embryology. The degree of defects vary from slight indentation of periphery to complete division of lobule including the cartilagenous skeleton. Traumatic defects that result from ear piercing are the most commonly acquired defects of the earlobe. For the methods of repair, accurate evaluation of the deformity with regard to the depth and width of the cleft and tissue deficiency must be carried out. Orientals have thought that the earlobes are significant cosmetic or physiognomic symbols so we reconstructed the earlobes considering all its aspects. We experiedced five cases of earlobe defects and managed them by Z-plasty, double Z-plasty, dermofat graft and composite graft methods. The resultts are as follows; 1. If there is enough local tissue for repair, Z-plasty and double Z-plasty methods are good enough to correct the ear lobe defect. 2. If the residual earlobe is hypopastic with significant deficiency of the soft tissues, composite graft and dermofat graft methods are requred for reconstruction.

        실리콘 겔이 흰쥐의 좌골 신경 및 복강내 장기에 미치는 영향에 대한 실험적 연구

        Silicone gel has been used for mammary implants contained in a silicone elastomer envelope. Following rupture of a silicone prosthesis, silicone gel can migrate along the soft tissue to the upper extremity or distant sites, probably resulting in symptoms of nerve dysfunction. The purpose of this ecperiment was to study the effects of silicone gel on the peripheral nerve and intraperitoneal organs. Silicone gel was placed extraneurally (n=16) adjacent to and injected directly in the left sciatic nerve (n=16) and in the peritoneal cavity (n=32) of male Sprague-Dawley rats over a 22-week period. For comparison, saline was injected into the contralateral site. For the study of functional changes in the nerve, sciatic function index(SFI) was calculated from gait analysis. Extraneural silicone gel elicited an intense inflammatory response initially by predominantly histiocytes with a few eosinophils, lymphocytes, and foreign-body giant cells. The cellular response peaked at 6 weeks, and then the thickness of the cellular infiltration surrounding the gel decreased gradually followed by collagen deposition after 10 weeks. The silicone gel had migrated and broken into smaller droplets with time. Each droplet appeared to initiate the inflam mation-fibrosis cycle anew. Perineural fibrosis was marked by 14 weeks and the thickness of epineurium was tripled at 22 weeks. No silicone gel penetrated the epineurium and there was no demyelination in the sciatic nerve. Intraneurally injected silicone gel also caused a delayed inflammatory response around the silicone gel which was similar to that of the extraneural group, but there was no migration and breakup into smaller droplets. The nerve fibers were slightly split but had sparsely demyelinized. The intraperitoneally injected silicone gel caused a nonspecific inflammation at the omentum, peritoneum, and liver. But, the silicone gel remained in the peritoneum and omentum with time, and no gel was found in the liver. The SFI's at 18 weeks were-5.90(extraneural group) and -8.18 (intraneural group), and - 6.30 (right leg for control), which were within normal limits (-1.9± 6.3), and the indices implied no functional abnormality. This experimental study showed that silicone which was injected extraneurally and intraneurally and in the peritoneal cavity caused little inflammatory reaction and fibrosis an presented no toxic reaction. The silicone gel left in the peritoneal cavity and omentum were not systemically absorpted, but caused no histologic abnormalities during experimental period. In conclusion, the free silicone gel would cause minor problems, such as mild compressive symtom. It is recommended the free silicone gel should be removed as early as possible.
