        중국인과 일본인에 대한 가변형 치과 인상용 트레이의 적합성에 관한 연구

        연구목적: 본 연구는 가변형 트레이 개발의 일환으로 그 시제품을 제작하고 비가역성 수성 콜로이드 인상재를 사용하여 중국인과 일본인에서 인상채득 시 트레이의 구강 내 적합성를 알아보고자 하였다. 연구재료 및 방법: 가변형 트레이는 한국 성인 악궁의 크기 분석결과를 기본으로 하여 설계하였으며, 먼저 CAD-CAM 작업을 통하여 견본 모형을 제작한 후, 이를 똑같이 복제한 실리콘 간이금형을 만들었다. 그리고 이 금형에 polyurethane을 주입하여 다수의 시제품을 완성하였다. 시제품을 이용하여 중국인으로는 상해 제2의과대학교 구강의학원 학생 60명 (남자 30명, 여자 30명)을 대상으로 하였고, 일본인으로는 일본 구마모토 고등학교 졸업생 60명 (남자 30명, 여자 30명)을 대상으로 인상채득을 실시하고, 측정부위별로 인상체의 두께와 길이를 측정한 후 통계처리를 하여 그 적합성을 평가하였다. 결과: 1. 중국인과 일본인 모두 스탑과 경사면에 의해 트레이의 폭이 적절히 조절되어 대체적으로 3-6mm의 균일한 인상체의 두께를 얻을 수 있었다. 2. 중국인의 상악 트레이에서는 전치 순측 변연부에서 두께가 6.2mm, 견치 순측 변연부에서 두께가 5.9mm, 구개부 중앙은 10.5 mm, 구개부 후방은 9.7mm 높이로 비교적 인상체의 두께가 크게 측정되었다. 3. 중국인의 하악 트레이에서는 제1, 2소구치 접촉점 설측 변연부에서 인상체의 길이가 8.9mm, 전치 설측 변연부에서 7.2mm로 길게 나타났으며, 견치 순측 6.8mm, 소구치 순측 7.0mm로 인상체의 두께가 크게 측정되었다. 4. 일본인의 상악 트레이에서는 전치 순측 변연부에서 두께가 7.4mm, 견치 순측 변연부에서 7.7mm, 구개부 중앙은 9.1mm 높이로 인상체의 두께가 비교적 크게 측정되었다. 5. 일본인의 하악 트레이에서는 견치 순측 7.4mm, 제1, 2소구치 접촉점 순측 변연부 8.4mm로 인상체의 두께가 크게 측정되었다. 결론: 본 가변형 치과 인상용 트레이는 한국인 모형 계측치를 바탕으로 설계되어 한국인에서 적합성이 우수하며, 본 실험의 결과 중국인과 일본인에서도 적합성이 좋아 본 트레이를 범용하여 쉽고 정확한 인상채득을 할 수 있을 것이다. Purpose: This study was designed to investigate the fitness of adjustable dental impression trays on the Chinese and the Japanese. Material and methods: Initial design of the adjustable dental trays was developed from the results of the dental arch size of Korean adults. This design was applied to the CAD-CAM process in order to create tray model samples. Simple silicon-base molds were then replicated based on these sample models. Polyurethane injection into the silicon- base molds completed the process of creating a large number of test products. 60 Chinese dental students (male:30, female:30) from the Shanghai Second Medical University and 60 Japanese alumni from the Kumamoto high school (male:30, female:30) were selected for taking irreversible hydrocolloid impression with these trays. The width and length of the impression body were measured on several measuring points by Vernier caliper. The results were analyzed statistically to evaluate the fitness of the trays. Results: 1. Uniform impression material thickness was achieved on the Chinese and Japanese by controlling the width of the tray using stops and beveled guides. The material thickness was generally within the range of 3 mm to 6 mm. 2. In the maxillary tray of the Chinese, average thickness of the impression material of the labial vestibule of the incisal teeth was 6.2 mm, the canine was 5.9 mm and the midpalatal part 10.5 mm and the posterior palatal part 9.7 mm. These were relatively large values. 3. In the mandibular tray of the Chinese, average length of the impression material of the lingual vestibule of first, second premolar contact point was 8.9 mm, the incisal teeth was 7.8 mm and thickness of the labial part of canine was 6.8 mm and premolars 7.0 mm. These were relatively large values. 4. In the maxillary tray of the Japanese, average thickness of the impression material of the labial vestibule of the incisal teeth was 7.4 mm, the canine was 7.7 mm and the midpalatal part 9.1 mm. These were relatively large values. 5. In the mandibular tray of the Japanese, average thickness of the impression material of the labial vestibule of first, second premolar contact point was 8.4 mm, and thickness of the labial part of canine was 7.4 mm. These were relatively large values. Conclusion: This adjustable dental tray shows good accuracy to Korean because it was designed by the analysis of the dental arch size of Korean adult model. With this result, it can be applied to Chinese and Japanese, we can take more easy and accurate dental impressions.

        구치부 고정성 국소의치에서 전부도재 시스템에 따른 응력분포에 관한 연구

        Purpose. The purpose of this study was to analyze stress distribution of all ceramic posterior fixed partial denture using a three dimensional finite element method. Material and method. A three dimensional finite element model was created to demonstrate all-ceramic posterior fixed partial denture and then this computer model measured the stress distribution of the all ceramic bridges which has a ceramic core materials such as Zirconia, IPS Empress. 2. In-Ceram zirconia, Metal-Ceramic. Also the stress distribution was examined according to loading sites when force was applied to sites such as the central area of second premolar the mesial connector of pontic, the central fossa of pontic, the distal connector of pontic, and the central fossa of second molar. Results. 1. In all the materials of the core in this study, von Mises stress indicated that the stress increased as force was applied to loaded sites, just at those points, on the connector, and the margin in the area adjacent to the connectors. 2. The maximum principal stress was much higher in the lower part of the connectors than in any other region. 3. As the load was applied to the different locations, the research showed a consistent increase of stress in the lower connectors. The maximum value of the von Mises stress was two or three times greater when the load was applied directly to the connectors rather than indirectly through another stressed region. 4. In the case of In-Ceram zirconia, the stress in lower connectors was the highest of all the reference points, the stress showed 75% of all the maximum stress. Ziconia showed 72%, Metal Ceramic 67% and IPS Empress 2 50%. 5. In the case of Ziconia, the stress was well dispersed in each reference point that the stress differences were smaller when compared to In-Cream ziconia.

        IPS Empress 도재관의 파절강도 : 상악 견치에서 절단연 삭제량과 축면 경사도에 따른 영향

        Purpose. The purpose of this study was to compare the fracture strength of the IPS Empress ceramic crown according to the incisal reduction (2.0mm, 2.5mm, 3.0mm) and axial inclination ($4^{\circ}$, $8^{\circ}$, $12^{\circ}$) of the upper canine. Material and methods. After 10 metal dies were made for each group, the IPS Empress ceramic crowns were fabricated and each crown was cemented on each metal die with resin cement. The cemented crowns mounted on the testing jig were inclined 30 degrees and the universal testing machine was used to measure the fracture strength. Results. 1. The fracture strength of the ceramic crown with 3.0mm depth and $12^{\circ}$ inclination was the highest (839N) Crowns of 2.0mm depth and $12^{\circ}$ inclination had the lowest strength (559N). 2. There was no significant difference in the fracture strength by axial inclination in the same incisal reduction group. 3. The fracture mode of the crowns was similar. Most of fracture lines began at the loading area and extended through proximal surface perpendicular to the margin irrespective of incisal reduction.
