        한국 아동의 측모두부 수평 기준선에 관한 연구

        환자의 진단이나 치료계획 수립 및 치료의 결과를 평가하기 위해서 현재까지 여러 가지 수평 기준들이 사용되고 있으나 그 정확성과 재현성이 문제가 되고 있으며 기존의 연구도 서양인을 대상으로 한 수평 기준선을 사용한 경우가 대부분이다. 특히 성장이 완료된 후의 상태를 평가하는 성인 환자와는 다르게 성장기 아동에서는 발육단계에 따라 수평 기준선이 변화될 수 있으므로 이를 고려한 수평 기준선의 설정이 필요할 것 이다. 이에 본 연구에서는 성장기 아동의 골성숙도와 부정교합군 및 성별에 따른Sella-Nasion (SN) 평면과 Frankfort-Horizontal(FH)평면이 이루는 각도와 F H평면과 다른 수평면 간의 관계를 조사하기 위해 교정치료를 받은 경험이 없는 성장기 남녀 아동 540명을 대상으로 수완부골 방사선 사진과 측모두부 방사선 사진을 촬영하였으며. SMA(Skeletal Maturity Assessment)를 이용하여 골성숙도에 따라 SMI 1-4를 A군, SMI 5-7을 B군, SMI 8-11을 C군으로 분류하고, 측모두부규격 방사선 사진 계측결과 부정교합 분류에 따라 골격성 I급 부정교합군, 골격성 II급 부정교합군, 골격성 III급 부정교합군의 세 군으로 분류한 후 10개의 항목을 평가하여 다음과 같은 결론을 얻었다. 1. SN 평면과 FH평면이 이루는 각도는 골성숙도와 부정교합 및 성별에 따른 차이를 보이지 않았다. 2. SN 평면과 FH평면이 이루는 각도는 남자는 8.27°±2.31°, 여자는 8.59°±2.24°, 전체에서는 8.42°±2.28° 였다. 3. FH평면과 구개평면이 이루는 각은 골성숙도, 부정교합 및 성별에 따른 차이를 보이지 않고 비교적 일정하였다.(1.09°±3.21°). Various types of horizontal reference planes are used for diagnosis,treatment planning and evaluation of treatment results. But these reference planes lack accuracy and repro ducibility, and are mainly for Caucasian. Unlike the adult patients who have completed growth, the horizontal reference planes for growing children may change continuously during growth. Therefore this must be considered in selecting the horizontal reference plane. The purpose of this study was to investigate the angle formed by the Sella-Nasion(SN) plane and Frankfort-Horizontal(FH) plane and evaluate the angle formed by F H plane and other horizontal reference planes in relation to different skeletal maturity and malocclusion types. 540 subjects with no orthodontic treatment history were chosen, and hand-wrist X-rays and lateral cephalometric X-rays were taken. According to SM A(Skeletal Maturity Assessment) of hand-wrist X-rays, the subjects were classified into 3 skeletal maturity groups: SMI 1-4 for group A, SMI 5-7 for group B and SMI 8-11 for group C. A second classification was made according to cephalometric analysis of lateral cephalograms.The subjects were classified into 3 malocclusion groups : Skeletal Class I,II and III malocclusion group. 10 measurements were evaluated. The results were as follows. 1. The angle formed by the SN plane and FH plane showed no difference among skeletal maturity groups, malocclusion groups, and between sexes. 2. The angles formed by the SN plane and FH plane were 8.27°±2.31°for males and 8.59°±2.24°for females. The average value for females and males was 8.42°±2.28°. 3. The angle formed by the FH plane and palatal plane was almost constant showing no difference among skeletal maturity groups, malocclusion groups, and between sexes(1.09°±3.21°).

        <P>We have successfully intercalated gadolinium (III) diethylenetriamine pentaacetic acid (Gd-DTPA) into layered double hydroxide (LDH). X-ray diffractometry, Infrared spectroscopy, and electron microscopic studies together with T1-weighed contrast image analyses revealed the possiblity of Gd-DTPA/LDH as in vivo magnetic resonance (MR) contrast nanoparticles.</P>

        상악골 전방견인 장치를 이용한 골격성 III급 부정교합 환자의 치험예

        골격성 III급 부정교합은 동양에서 발생 빈도가 높으며 그 양상과 유형도 다양하며 치료가 까다로운 것으로 알려져 있다. 성장기 아동의 경우 조지에 골격 부조화를 발견하고 성장을 통해 골격의 부조화를 개선하여 수술의 가능성을 감소시키고 환자의 심리적 위축감을 감소시킬 수 있다면 그 치료는 매우 중요한 의미를 가질 것이다. 본 증례는 골격성 III급 부정교합 환자를 상악골 전방견인을 장치를 이용하여 비교적 짧은 전방 견인기간으로 안모의 개선을 얻을 수 있었던 임상적 증례이다. 비교적 어린 나이에 시도한 경우에 더 짧은 전방견인 기간 동안에 만족할 만의 결과를 얻었으나 성장잠재력이 미약한 연령에서도 골격적 개선을 얻을 수 있었다. 그러나, 상악골 전방견인 장치의 효과에 대한 많은 선학들의 연구가 있었음에도 불구하고 아직 진단에 대한 crtieria나 재발에 관한 연구는 매우 미약한 실정이고, 본 보고에서도 재발에 관한 관찰을 하지 못하였음이 아쉬움으로 남아있으며 향후에 본 증례들에 대한 retention과 relapse에 관하여 계속적인 관찰이 필요할 것이다. The clinical cases presented here involve skeletal Class III malocclusion cases treated with maxillary protraction in a relaticely short period of time with good results. When used on young patients, satisfactory results were obtained in a short period of time, but even for those with less growth potential remainig, skeletal enhancement was still evident. However, data on the criteria of diagnosis or relapse following maxillary protraction is limited despite the number of studies on the subiect. The present study could not incude the observations on retention and relapse, and further studies in the future may include such observations.

        Orthodontic Traction of Impacted Tooth

        매복된 치아는 부정교합과 인접치아의 치근 흡수, 낭종 등의 병적 변화를 유발할 수 있으므로 임상적 검사와 방사선학적 검사를 통해 정학한 위치를 확인하도록 하고, 적절한 치료계획을 통해 제 위치를 찾아주어야 한다. 외과적 수술시 치주적 문제점들을 고려하여 연조직과 골조직의 보존을 최대화할 수 있어야 하며 환자의 구강 위생상태에 대한 철저한 교육을 통해 좋은 구강 위생상태를 유지할 수 있도록 해야 한다. 교정적 견인시에는 매복치를 배열할 공간의 확보와 고정원에 대한 고려가 선행되어야 하며, 교정적 견인시 적절한 힘과 mechanics의 구사로 치근의 흡수 등의 부작용이 일어나지 않도록 해야 하고 상태에 따라 다양한 방법을 이용한 교정적 견인을 시도할 수 있다. 매복된 치아는 병적 변화를 일으킨 경우나 반드시 발치를 해야하는 경우가 아니라면 교정적 견인을 통해 기능과 심미성을 회복시켜 줄 수 있으므로 조기 발견과 발견시의 올바른 진단과 치료계획을 수립하는 것이 중요하다. Tooth impaction may lead to malocclusion, root resorption, cyst or aberrant changes in adjacent teeth. Clinical and radiographic examinations are used to locate the impaction, and appropriate treatment plans must be made to relocate the impacted tooth. When surgically exposing the impacted tooth, periodontal considerations to conserve maximum amount of soft and hard tissue are used. Oral hygiene instructions are emphasized to maintain sound periodontal health. Securing enough space for the impacted tooth and proper anchorage is important. Proper use of force and mechanics is crucial to prevent such complications as root resorption. Various patterns of orthodontic traction may be employed as situation permits. Most impaction cases can be managed with orthodontic traction to restore function and esthetics, provided that early detection and proper diagnosis and treatment planning are made.

        Hanwoo (Korean cattle) is the native, taurine type of cattle breed of Korea and its history as a draft animal dates back to 5000 Years. In earlier times Hanwoo was used extensively for farming, transportation. Over the period of time, Hanwoo has changed to be meat type cattle. Full-scale production of Hanwoo as meat-type cattle has occurred since 1960s with the rapid growth of the Korean economy. Hanwoo is one of the most economically important species in Korea as it is a significant source of nutrition to the Korean people. Hanwoo beef is the most cherished food of Korea. One of the main goals of researchers is to increase the meat quality, quantity and taste of the beef. In this review we describe the origin, domestication of Hanwoo cattle and breeding program initiated from 1980’s. Moreover the advent of technological advancement had provided us a platform to perform genome wide selection on economic traits and its implementation into traditional breeding programs.

      • A nanohybrid system for taste masking of sildenafil

        <P>A nanohybrid was prepared with an inorganic clay material, montmorillonite (MMT), for taste masking of sildenafil (SDN). To further improve the taste-masking efficiency and enhance the drug-release rate, we coated the nanohybrid of SDN–MMT with a basic polymer, polyvinylacetal diethylaminoacetate (AEA). Powder X-ray diffraction and Fourier transform infrared experiments showed that SDN was successfully intercalated into the interlayer space of MMT. The AEA-coated SDN–MMT nanohybrid showed drug release was much suppressed at neutral pH (release rate, 4.70 ± 0.53%), suggesting a potential for drug taste masking at the buccal cavity. We also performed in vitro drug release experiments in a simulated gastric fluid (pH = 1.2) and compared the drug-release profiles of AEA-coated SDN–MMT and Viagra<SUP>®</SUP>, an approved dosage form of SDN. As a result, about 90% of SDN was released from the AEA-coated SDN–MMT during the first 2 hours while almost 100% of drug was released from Viagra<SUP>®</SUP>. However, an in vivo experiment showed that the AEA-coated SDN–MMT exhibited higher drug exposure than Viagra<SUP>®</SUP>. For the AEA-coated SDN–MMT, the area under the plasma concentration– time curve from 0 hours to infinity (AUC<SUB>0-∞</SUB>) and maximum concentration (C<SUB>max</SUB>) were 78.8 ± 2.32 μg · hour/mL and 12.4 ± 0.673 μg/mL, respectively, both of which were larger than those obtained with Viagra<SUP>®</SUP> (AUC<SUB>0-∞</SUB> = 69.2 ± 3.19 μg · hour/mL; C<SUB>max</SUB> = 10.5 ± 0.641 μg/mL). Therefore, we concluded that the MMT-based nanohybrid is a promising delivery system for taste masking of SDN with possibly improved drug exposure.</P>

        Heterostructured layered aluminosilicate-itraconazole nanohybrid for drug delivery system.

        <P>A nanohybrid, consisting of layered aluminosilicate as a host material and itraconazole as a guest molecule, was successfully synthesized through the interfacial intercalation reaction across the boundary between water and water-immiscible liquid at the various pH. According to the powder X-ray diffraction pattern, the basal spacing of the intraconazole-layered aluminosilicate nanohybrid increased from 14.7 to 22.7 A depending on the pH of the aqueous suspension. The total amounts of itraconazole in the hybrids were determined to be 2.3-25.4 wt% by HPLC analysis. The in vivo pharmacokinetics study was performed in rats in order to compare the absorptions of itraconazole for the itraconazole-layered aluminosilicate nanohybrid and a commercial product, Sporanox. The pharmacokinetic data for the nanohybrid and Sporanox showed that the mean area under the plasma concentration-time curve (AUC, 2477 +/- 898 ng x hr/mL and 2630 +/- 953 ng x hr/mL, respectively) and maximum concentration (Cmax, 225.4 +/- 77.4 ng x hr/mL and 223.6 +/- 51.9 ng x hr/mL, respectively), were within the bioequivalence (BE) range. Therefore, we concluded that this drug-layered aluminosilicate nanohybrid system has a great potential for its application in formulation of poorly soluble drugs.</P>

        Anticancer drug encapsulated in inorganic lattice can overcome drug resistance

        <P>A methotrexate (MTX)-layered double hydroxide (LDH) hybrid have been developed as a drug delivery system, in which an anticancer drug, MTX, was intercalated into a 2-dimensional LDH nanovehicle to form a nanohybrid. According to the comparative cell viability studies between MTX only and its LDH nanohybrid on MTX sensitive and resistant cell culture lines, it was found that the MTX-LDH nanohybrid could bypass the MTX resistance and eventually inhibit cancer cell proliferation very effectively compared to free MTX, due to an enhanced permeability and retention effect of MTX-LDH nanoparticles even in dihydrofolate reductase-overexpressing MTX-resistant cells. This is definitely associated with the uptake mechanism <I>via</I> a clathrin-mediated endocytic pathway for the MTX-LDH nanohybrid particles, the same as for the LDH nanocarrier only, which is completely different from the cellular uptake mechanism for MTX only, the reduced folate carrier (RFC) and/or the folate receptor entries.</P> <P>Graphic Abstract</P><P>A methotrexate (MTX)-layered double hydroxide (LDH) hybrid has been developed as a delivery system that can overcome drug resistance.
