Acute generalized exanthematous pustulosis (AGEP) is a severe and rare eruption that characterized by a fever, neutrophilia and non-follicular pustules on the erythematous base with an acute onset. AGEP develops mostly from factors related to drugs, mainly antibiotics. A 65-year-old woman visited our clinic with generalized pustular exanthema on the intertriginous areas. She treated with celecoxib 200mg once for back pain 3 days ago.Laboratory studies revealed leukocytosis and increased erythrocyte sedimentation rate. The histological findings were subcorneal pustules, edema in the upper dermis and polymorphic perivascular infiltrates. Two days after using systemic corticosteroid, papulopustules on the trunk subsided and desquamation started. Three weeks later, re-epithleialization of denuded area with complete clearance of skin observed. Celecoxib is a cyclooxygenase-2 selective nonsteroidal anti-inflammatory drug widely used to treat pain, which might cause AGEP very rarely. Here, we report a case of AGEP caused by celecoxib.

Papular acantholytic dyskeratosis is a collection of papular skin lesions which occur in intertriginous and genital area. They show a characteristic histology of focal suprabasal acantholysis without the connection to a syndrome such as the Hailey-Hailey disease or the Darier disease. We describe a 50-year-old man with an asymptomatic papular eruption on the perianal area for several years. Histologically, a biopsy specimen showed diffuse hyperkeratosis and irregular acantholysis throughout the epidermis. We used the CO2 laser therapy as a therapeutic option. Despite causing a long and painful healing process, a considerable reduction of the symptoms was achieved . Although we do not know the precise nature or the incidence of this disease, papular acantholytic dyskeratosis should be included in the differential diagnosis of verrucous papules in perineal or perianal areas and CO2 laser can be a good therapeutic option.

Sudden headache onset may rarely be caused by spontaneous intracranial hypotension (SIH). Other associated symptoms in patients with SIH are nausea, vomiting, vertigo, hearing alteration, and visual disturbance. This case report describes a 43-year-old female diagnosed with SIH who developed diplopia after resolution of an abrupt-onset headache, which was managed with conservative treatments, including bed rest and hydration. She was also diagnosed with secondary right sixth cranial nerve palsy. Although conservative management relieved her headache, the diplopia was not fully relieved. Application of an autologous epidural blood patch successfully relieved her diplopia, even after 14 days from the onset of visual impairment.

<P><B>Background and Purpose—</B></P><P>Good collateral flow is an important predictor for favorable responses to recanalization therapy and successful outcomes after acute ischemic stroke. Magnetic resonance perfusion–weighted imaging (MRP) is widely used in patients with stroke. However, it is unclear whether the perfusion parameters and thresholds would predict collateral status. The present study evaluated the relationship between hypoperfusion severity and collateral status to develop a predictive model for good collaterals using MRP parameters.</P><P><B>Methods—</B></P><P>Patients who were eligible for recanalization therapy that underwent both serial diffusion-weighted imaging and serial MRP were enrolled into the study. A collateral flow map derived from MRP source data was generated through automatic postprocessing. Hypoperfusion severity, presented as proportions of every 2-s <I>T</I>max strata to the entire hypoperfusion volume (<I>T</I>max≥2 s), was compared between patients with good and poor collaterals. Prediction models for good collaterals were developed with each <I>T</I>max strata proportion and cerebral blood volumes.</P><P><B>Results—</B></P><P>Among 66 patients, 53 showed good collaterals based on MRP-based collateral grading. Although no difference was noted in delays within 16 s, more severe <I>T</I>max delays (<I>T</I>max<SUB>16–18 s</SUB>, <I>T</I>max<SUB>18–22 s</SUB>, <I>T</I>max<SUB>22–24 s</SUB>, and <I>T</I>max<SUB>>24 s</SUB>) were associated with poor collaterals. The probability equation model using <I>T</I>max strata proportion demonstrated high predictive power in a receiver operating characteristic analysis (area under the curve=0.9303; 95% confidence interval, 0.8682–0.9924). The probability score was negatively correlated with the volume of infarct growth (<I>P</I>=0.030).</P><P><B>Conclusions—</B></P><P>Collateral status is associated with more severe <I>T</I>max delays than previously defined. The present <I>T</I>max severity–weighted model can determine good collaterals and subsequent infarct growth.</P>

Aim(s): The present study aims to apply the network analysis method to identify the relationships between keywords of previous studies on metabolic syndrome intervention (MSI), while simultaneously examining the whole knowledge framework of research on this topic from a macro perspective. The aim of the study is to predict the future trends in research on interventions of metabolic syndrome by identifying the newly emerging research keywords. Method(s): The study utilized electronic academic databases, PubMed, CINAHL, EMBASE, and Cochrane. The search strategy included metabolic syndrome AND (treatment OR intervention OR implementation). As a result, 5,978 studies published from 1983 to 2018 were found. Key research topics were extracted with the assistance of a text-mining professional who utilized the Text Rank analyzer. The filtered keywords were then reexamined by three professionals in nursing informatics and management and one text-mining expert. Result(s): After analyzing the simultaneous appearance frequency of the 2,227 keywords, the study identified keywords with high frequency such as metabolic syndrome, cardiovascular disease, insulin resistance, risk factor, diabetes mellitus, blood pressure, physical activity, waist circumference, cardiovascular risk, abdominal obesity and so on. By applying relation of keywords on betweeness centrality, 7 clusters were established. Emerging topics were identified: the beginning phase (cardiovascular risk, abdominal obesity), the development phase (lifestyle modification, oxidative stress), the maturation phase (cardiometabolic disease, gut microbiota), and the expansion phase (cardiometabolic health, sleep duration). Conclusion(s): This study provides vital insights that can be based on the development of MSI research. Thus, if new researchers refer to existing trends to establish the subject of their study and the direction of the development of future research on MSI can be predicted. Also, nursing should play a critical role in MSI research.
