      • Evolution of ALPPS: The Simpler, Safer and Effective One---TELPP

        Aims: The characteristic of associating liver partition and portal vein ligation for staged hepatectomy(ALPPS) carries high mortality and morbidity. There is room for improvement. We suggest Terminal Branches Portal Vein Embolization (TBPVE) as a way to compart the liver. As a result, only a single surgical operation is required.This method is termed Terminal branches portal vein Embolization Liver Partition Planned hepatectomy (TELPP). Methods: Patients with unresectable primary or metastatic liver tumor were performed with TELPP. The procedure of TELPP was that in addition to PVE, embolization agent was infused to the terminal branches of portal vein of S5,S8 or S4. CT scan was taken one or two weeks later, and standard liver volume(SLV), FLR and FLR/SLV are calculated. Two weeks later when the FLR and liver function is appropriate, open or laparoscopic hepatectomy is performed. Results: The study included 11patients including hepatocellular carcinoma: n =8, intrahepatic cholangiocarcinoma: n = 1, hilarcholangiocarcinoma: n =1, colorectal liver metastasis: n =1. After a waiting period of 14 days, the volume of theFLR had increased from 382mlto 578ml, representing a median volume increase of 51% (range =32.5%-86.7%). Of the 11patients with hepatectomy, right hemihepatectomy (n=2), extended right hemihepatectomy (n=5), right trisecmentectomy(2), extended left hemihepatectomy (n=1) and left trisecmentectomy(1). No patient died, and no serve perioperative morbidity occurred. Conclusions: ALPPS and all modifications need two-stage operations with a high morbidity and mortality rate. It seems that TELPP is very promising. It has the merit of ALPPS as extraordinarily rapid increasement of FLRvolume, yet the morbidity and mortality is much lower, owing to the fact that unlike ALPPS, there is no two liver raw surfaces left behind in the abdominal cavity to produce bile leak, as only single surgical operation is required

        Robust Generalized Labeled Multi-Bernoulli Filter and Smoother for Multiple Target Tracking using Variational Bayesian

        Multiple target tracking mainly focuses on tracking unknown number of targets in the complex environment of clutter and missed detection. The generalized labeled multi-Bernoulli (GLMB) filter has been shown to be an effective approach and attracted extensive attention. However, in the scenarios where the clutter rate is high or measurement-outliers often occur, the performance of the GLMB filter will significantly decline due to the Gaussian-based likelihood function is sensitive to clutter. To solve this problem, this paper presents a robust GLMB filter and smoother to improve the tracking performance in the scenarios with high clutter rate, low detection probability, and measurement-outliers. Firstly, a Student-T distribution variational Bayesian (TDVB) filtering technology is employed to update targets’ states. Then, The likelihood weight in the tracking process is deduced again. Finally, a trajectory smoothing method is proposed to improve the integrative tracking performance. The proposed method are compared with recent multiple target tracking filters, and the simulation results show that the proposed method can effectively improve tracking accuracy in the scenarios with high clutter rate, low detection rate and measurement-outliers.       • Prognostic Significance of Interactions Between ER Alpha and ER Beta and Lymph Node Status in Breast Cancer Cases

        Objective: Both estrogen receptors, ER alpha ($ER{\alpha}$) and ER beta ($ER{\beta}$), are expressed in 50-70% of breast cancer cases. The role of $ER{\alpha}$ as a prognostic marker in breast cancer has been well established as its expression is negative correlated with tumor size and lymph node metastasis. $ER{\beta}$ is also a favorable prognostic predictor although this is less well documented than for $ER{\alpha}$. Materials and Methods: To explore whether ERs independently or together might influence clinical outcome in breast cancer, the correlation between the ERs with the clinicopathological features was analyzed in 84 patients. Results: $ER{\alpha}$ expression negatively correlated with tumor stage (r=-0.246, p=0.028) and tended to be negatively correlated with lymph node status (r=-0.156, p=0.168) and tumor size (r=-0.246, p=0.099). Also, $ER{\beta}$ was negatively correlated with nodal status (r=-0.243, p=0.028), as was coexpression of $ER{\alpha}$ and $ER{\beta}$ (p=0.043, OR=0.194, 95% CI= 0.040-0.953). Conclusion: Coexpression of ERs might serve as an indicator of good prognosis in breast cancer patients.

        Evaluation of Non-isothermal Melt Crystallization Kinetics of the Poly(lactic acid)/Graphene Composites under Variant Cooling Circumstances via Solidification Analysis Approach

        The poly(lactic acid) (PLA)/graphene (GN) composites were prepared by solution blending. An in situ measurement technique was adopted to examine the non-isothermal crystallization of PLA/GN composites, which was different from the typical circumstances at constant cooling rate in DSC. The rheological properties of the samples were investigated, and the Carreau-A model was applied to non-linearly fit the rheological data. The non-isothermal crystallization kinetics and solidification behaviors at variant cooling rates were investigated using a modified version of Avrami equation by Jeziorny coupled with three-parameter model (TPM) and four-parameter model (FPM). Our findings showed that the viscosity of the PLA/GN composites dropped with increasing shear rate or GN content. Crystallization kinetic analysis suggested that GN served as an effective nucleating agent for PLA under non-isothermal melt crystallization conditions with variant cooling rate.

        Major Determinants and Long-Term Outcomes of Successful Balloon Dilatation for the Pediatric Patients with Isolated Native Valvular Pulmonary Stenosis: A 10-Year Institutional Experience

        Purpose: We report herein major determinants and long- term outcomes of balloon dilatation (BD) for 27 pediatric patients with isolated native valvular pulmonary stenosis (VPS). Materials and Methods: From May 1997 to May 2003, 27 pediatric patients with VPS (pressure gradients≧ 40mmHg) were enrolled in this retrospective study. Single- balloon maneuver was applied in 26 patients, and double- balloon maneuver in 1. After BD, the pressure gradients were documented simultaneously by pullback maneuver by cardiac catheterization and echocardiography within 24 hours, at 1- month, 3-month, 1-year, and 4-to-10-year follow-ups. Results: Before BD, the echocardiographic gradients ranged from 40 to 101mmHg (61±19, 55), and from 40 to 144mmHg (69 ±32, 60) by pressure recordings. After BD, the gradients ranged from 12 to 70mmHg (29±13, 27) by pressure recording (p<0.001), and from 11 to 64mmHg (27±12, 26) by echocardiography within 24 hrs (p<0.001). The ratios of the systolic pressure of the right ventricle to those of the left ventricle were 55 to 157% (89±28, 79%) before BD, and 30 to 79% (47±13, 42%) after BD (p<0.001). Follow-up (7.7±5.7, 4.5 years) echocardiographic gradients ranged from 11 to 61mmHg (25±11, 24). Two patients did not have immediate success owing to infundibular spasm. Improved right ventricular compliance could be accounted for the ultimate success in these 2 patients. The ultimate successful rate was 100%. Conclusion: BD can achieve excellent long-term outcomes in the pediatric patients with isolated native VPS.

      • Association of Single Nucleotide Polymorphisms in the Prostaglandin-endoperoxide Synthase 2 (PTGS2) and Phospholipase A<sub>2</sub> Group IIA (PLA2G2A) Genes with Susceptibility to Esophageal Squamous Cell Carcinoma

        Background: The prostaglandin-endoperoxide synthase 2 (PTGS2) and phospholipase A2 group IIA (PLA2G2A) genes encode enzymes that are involved in arachidonic acid and prostaglandin biosynthesis. Dysregulation of both genes is associated with inflammation and carcinogenesis, including esophageal squamous cell carcinoma (ESCC). We therefore hypothesized that there is an association between single nucleotide polymorphisms (SNPs) in these genes and susceptibility to ESCC. Methods: We performed a gene-wide tag SNP-based association study to examine the association of SNPs in PTGS2 and PLA2G2A with ESCC in 269 patients and 269 healthy controls from Taihangshan Mountain, Henan and Hebei Provinces, the rural area of China which has the highest incidence of esophageal cancer in the world. Thirteen tag SNPs in PLA2G2A and 4 functional SNPs in PTGS2 were selected and genotyped using a high-throughput Mass Array genotyping platform. Results: We found a modest increased risk of ESCC in subjects with the PTGS2 rs12042763 AA genotype (OR=1.23; 95% CI, 1.00-3.04) compared with genotype GG. For PLA2G2A, a decreased risk of ESCC was observed in subjects with the rs11677 CT (OR=0.51, 95%CI, 0.29-0.85) or TT genotype (OR=0.51, 95%CI, 0.17-0.96) or the T carriers (CT+TT) (OR=0.52, 95%CI, 0.31-0.85) when compared with the CC genotype. Also for PLA2G2A, rs2236771 C allele carriers were more frequent in the control group (P=0.02). Subjects with the GC (OR=0.55, 95%CI, 0.33-0.93) or CC genotype (OR=0.38, 95% CI, 0.16-0.94) or the C carriers (GC+CC) (OR=0.52, 95%CI, 0.32-0.85) showed a negative association with ESCC susceptibility. Conclusions: Our results suggest that PTGS2 and PLA2G2A gene polymorphisms may modify the risk of ESCC development.

      • Curcumol Induces Apoptosis in SPC-A-1 Human Lung Adenocarcinoma Cells and Displays Anti-neoplastic Effects in Tumor Bearing Mice

        Curcumol is a sesquiterpene originally isolated from curcuma rhizomes, a component of herbal remedies commonly used in oriental medicine. Its beneficial pharmacological activities have attract significant interest recently. In this study, anti-cancer activity of curcumol was examined with both in vitro and in vivo models. It was found that curcumol exhibited time- and concentration-dependent anti-proliferative effects in SPC-A-1 human lung adenocarcinoma cells with cell cycle arrest in the G0/G1 phase while apoptosis-induction was also confirmed with flow cytometry and morphological analyses. Interestingly, curcumol did not display growth inhibition in MRC-5 human embryonic lung fibroblasts, suggesting the anti-proliferative effects of curcumol were specific to cancer cells. Anti-neoplastic effects of curcumol were also confirmed in tumor bearing mice. Curcumol (60 mg/ kg daily) significantly reduced tumor size without causing notable toxicity. In conclusion, curcumol appears a favorable anti-cancer candidate for further development.
