      • A Method for Diagnosing Healthcare Provision Process Based on Failure Cases

        Background: Provision of high-quality healthcare services to hospital patients IS regarded as an important social issue in Japan. One useful approach is to diagnose the healthcare provision process based on failure cases, including not only medical accidents but also medical incidents, to find the key factors related to the healthcare provision process, and to change the process for the better. Many previous related studies in healthcare are too focused on the-human factor," which is only one among many different types of factors. On the other hand, in the industry field, many useful analysis models such as the Input-Process-Output model, Process Networking, Quality Management System models have already been developed and applied in many companies. However, there are substantial differences among the analytic viewpoints used in each model, and the relationship among them are not clear. Purpose: To conduct proper work process diagnosis based on failure cases, the necessary analytic viewpoints must be sufficiently systematized for hospital medical staffs who are unfamiliar with the above analysis models to utilize them relatively quickly. The purpose of this study is to systematize the analytic viewpoints, establish the diagnosis procedure and develop the needed tool. Finally, we confirm the effectiveness of the proposed method by applying it in hospital A. Approach: First, we identify the necessary conditions of the ideal process model that can provide high-quality healthcare services by examining them from the viewpoint of process structure, contents and operational management style. After establishing the general diagnosis procedure based on the failure cases, the analytic viewpoint used in each step of the diagnosis is specified and shaped into one analytic template. Findings: We proposed a method for diagnosing the healthcare provision process based on failure cases and applied it in several real cases in hospital A. As a result, we can confirm that the proposed diagnosis method leads medical staffs to review their own healthcare provision process systematically, to find the key factors of the failure case effectively, and to devise various types of countermeasures compared with the previous studies.

        Education System to Learn the Skills of Management Decision-Making by Using Business Simulator with Speech Recognition Technology

        In this paper, we propose an educational system that involves a business game simulator and related curriculum. To develop these two elements, we examined the decision-making process related to business management and identified some significant skills thereby. In addition, we created an original simulator, named BizLator (http://bizlator.com), to help students develop these skills efficiently. Next, we developed a curriculum suitable for the simulator. We confirmed the effectiveness of the simulator and curriculum in a business-game-based class at Aoyama Gakuin University in Tokyo. On the basis of this, we compared our education system with a conventional system. This allowed us to identify advantages of and issues with our proposed system. Furthermore, we proposed a speech recognition support system named BizVoice in order to provide the teachers with more meaningful feedback, such as level of students’ understanding. Concretely, BizVocie fetches students’ speech of discussion during the game and converts the voice data to text data with speech recognition technology. Finally, teachers can grasp students’ parameters of understanding, and thereby, the students also can take more effective class using BizLator. We also confirmed the effectiveness of the system in the class of Aoyama Gakuin Universiry.

        Advances in cancer DNA methylation analysis with methPLIER: use of non-negative matrix factorization and knowledge-based constraints to enhance biological interpretability

        DNA methylation is an epigenetic modification that results in dynamic changes during ontogenesis and cell differentiation. DNA methylation patterns regulate gene expression and have been widely researched. While tools for DNA methylation analysis have been developed, most of them have focused on intergroup comparative analysis within a dataset; therefore, it is difficult to conduct cross-dataset studies, such as rare disease studies or cross-institutional studies. This study describes a novel method for DNA methylation analysis, namely, methPLIER, which enables interdataset comparative analyses. methPLIER combines Pathway Level Information Extractor (PLIER), which is a non-negative matrix factorization (NMF) method, with regularization by a knowledge matrix and transfer learning. methPLIER can be used to perform intersample and interdataset comparative analysis based on latent feature matrices, which are obtained via matrix factorization of large-scale data, and factor-loading matrices, which are obtained through matrix factorization of the data to be analyzed. We used methPLIER to analyze a lung cancer dataset and confirmed that the data decomposition reflected sample characteristics for recurrence-free survival. Moreover, methPLIER can analyze data obtained via different preprocessing methods, thereby reducing distributional bias among datasets due to preprocessing. Furthermore, methPLIER can be employed for comparative analyses of methylation data obtained from different platforms, thereby reducing bias in data distribution due to platform differences. methPLIER is expected to facilitate cross-sectional DNA methylation data analysis and enhance DNA methylation data resources.

      • A matrix of the functions and organizations that ensure continued healthcare services in a disaster

        Purpose Japan is one of the most natural disaster-prone countries in the world. In the case of a natural disaster or mass casualty incident, a large number of injured people are likely to go to hospitals, which need to provide more services at such times. Thus, it is necessary for various bodies - such as hospitals, municipalities, medical associations, pharmaceutical associations, and trade associations - to collaborate. For example, coordination among multiple hospitals is important when transporting patients to another hospital if they cannot be examined at the first one. There is a pressing need to establish an Area Disaster Resilience Management System for Healthcare (ADRMS-H) to increase medical resilience. In order to develop an ADRMS-H and ensure that continuous healthcare is provided during calamities, it is necessary to clarify the functions (which we have already explained) and coordination that organizations must perform. Since medical care needs change by the hour during disasters, so do the functions that guarantee ongoing healthcare. This study aims to create a matrix of functions ensuring continued healthcare services in a disaster and relevant organizations to understand each one``s role and how they relate to each other. Methodology/Approach We used the case of Kawaguchi city in Saitama Prefecture, Japan as an example to illustrate related organizations needed to fulfill the functions for ensuring continued healthcare services. Next, we created a matrix of the functions and organizations. Findings This study identified related organizations such as municipalities, hospitals, medical associations, and trade associations in Kawaguchi city. Since functions for ensuring ongoing health care change by the hour, so do the organizations, whose transformations we analyzed by the hour. Value The results of this paper can help people understand how related organizations work together during disasters; in light of this, it will be possible to develop a model for an ADRMS-H in terms of coordination among organizations.

      • Development of Assessment Item for Area Resilience for Healthcare with SERVQUAL

        Purpose; It is important to maintain continuity of health care services when disaster occur. Countries such as Japan that are characterized by a high risk of natural disasters should take countermeasures against healthcare service disruptions. To enhance healthcare services`` continuity in area, healthcare institutions should consider the relationship between municipalities and healthcare-related organizations. In addition, it is necessary to evaluate current performance of their system and improve towards upcoming disturbance. This study focuses on evaluation for resilience of area resilience of healthcare services with the consideration for community resilience assessment and SERVQUAL. Methodology/Approach; To clarify Business continuity management system (BCMS) that includes every stakeholder in the healthcare services community, we defined necessary functional areas in disaster base medical centers. We derived assessment items thorough structures based on the previous literature while incorporating the viewpoint of the healthcare services field. Moreover, we compared these items with the existing evaluation items thorough literature survey such as community resilience assessment system and SERVQUAL. After development of the evaluation items, we discussed with managers of health care institution and develop measurement and improvement plan. Findings; As a result, we developed evaluation items of resilience for area healthcare services including disaster base medical centers. The items are organized with the structure, process and outcome which is proposed by Donabedian. With the consideration of SERVQUAL, evaluation of area resilience should include the management of the perception among citizen since the service level during disaster is quite different from usual state. Research limitations/implications; The assessment items for area healthcare services is not applied for the evaluation of the area. With the quality management system for health care model, the outcomes of this study will be useful for business continuity in various regions since it unites the management system. In addition, the study contributes to making a safer and more secure society.

        Integrative analysis reveals early epigenetic alterations in high-grade serous ovarian carcinomas

        High-grade serous ovarian carcinoma (HGSOC) is the most lethal gynecological malignancy. To date, the profiles of gene mutations and copy number alterations in HGSOC have been well characterized. However, the patterns of epigenetic alterations and transcription factor dysregulation in HGSOC have not yet been fully elucidated. In this study, we performed integrative omics analyses of a series of stepwise HGSOC model cells originating from human fallopian tube secretory epithelial cells (HFTSECs) to investigate early epigenetic alterations in HGSOC tumorigenesis. Assay for transposase-accessible chromatin using sequencing (ATAC-seq), chromatin immunoprecipitation sequencing (ChIP-seq), and RNA sequencing (RNA-seq) methods were used to analyze HGSOC samples. Additionally, protein expression changes in target genes were confirmed using normal HFTSECs, serous tubal intraepithelial carcinomas (STICs), and HGSOC tissues. Transcription factor motif analysis revealed that the DNA-binding activity of the AP-1 complex and GATA family proteins was dysregulated during early tumorigenesis. The protein expression levels of JUN and FOSL2 were increased, and those of GATA6 and DAB2 were decreased in STIC lesions, which were associated with epithelial-mesenchymal transition (EMT) and proteasome downregulation. The genomic region around the FRA16D site, containing a cadherin cluster region, was epigenetically suppressed by oncogenic signaling. Proteasome inhibition caused the upregulation of chemokine genes, which may facilitate immune evasion during HGSOC tumorigenesis. Importantly, MEK inhibitor treatment reversed these oncogenic alterations, indicating its clinical effectiveness in a subgroup of patients with HGSOC. This result suggests that MEK inhibitor therapy may be an effective treatment option for chemotherapy-resistant HGSOC.

      • A study on the mechanism and countermeasures of medical devices incidents

        Purpose: This study will focus on the human, medical devices and human-computer interactions to analyze the medical devices incidents. Based on the analysis result, this study will suggest the mechanism of medical devices incidents and the countermeasure methods. Approach: In this study, we summarize the mechanism of medical devices incidents through the analysis and research on the cases of medical devices incidents. Mechanism of medical devices incidents will consider the error-process, error-factors and the operating environments. The consideration point is the human, medical devices and human-computer interactions. Based on the explored mechanism of medical incidents, this study will establish the countermeasures through eliminating the error-factors of medical devices incidents. Methods to establish the countermeasures will consider the characteristics of human, features of operating environment and medical devices. In order to ensure the countermeasure``s effect, the countermeasure ideal is by improving the features of operating environment and operating devices to cater human``s characteristics. Findings: Through this study we will present following two: (l) Propose the general mechanism which can summarize the incidents of medical devices, (2) Propose the countermeasures which based on the mechanism and can solve the practical problems. Value of paper: Three aspects of human, devices and the interaction caused the medical devices incidents. In the past research on the medical devices, one or two aspects are considered, but a few of research combine the three aspects. To consider the three aspects and propose the useful mechanism and countermeasure method is one of the value of this paper. Using the practical case to propose scientific result and to solve the practical medical problems is also the value of this paper.
