A Soft x-ray microscope operates in the 'water window' wavelength region (2.3-4.4 nm), where natural contrast between carbon (protein) and oxygen (water) allows to image a biological cell. The compact soft x-ray microscope operates at λ = 2.88 nm (430 eV) and consist of liquid-nitrogen-jet-target laser-plasma x-rays source, a tandem ellipsoidal condenser mirror for sample illumination, an objective micro zone plate, and a thinned back-illuminated charge coupled device with 1,024 × 1,024 pixels. The Liquid-nitrogen-jet-target laser-plasma source was a high brightness of 1.4×10^(12) photons/pulse·sr·line. The tandem ellipsoidal condenser mirror has 15% reflectivity. The photon flux arrived at the sample is 1.29×10^(5) photons/μ㎡ The micro zone plate was used for an objective optic, and had outermost zone width of 30 nm, zone number of 667, and diameter of 80㎛, and the focal length was 835㎛. On performance test for the compact soft x-ray microscope, the 'Siemens Star' image was obtained with a 60-nm-resolution at 380 magnification and 3 minutes exposure. I also obtained images of biological samples, such as diatom and red blood cell of a mouse with 5-10 minutes exposure.

남성 유방암 3증례에 관한 병리학적 및 임상적 고찰

We have experienced three cases of male breast cancer in our series, 586 cases of all surgically treated breast cancer from 1970 to 1987. Pathological diagnosis covered infiltrating ductal carcinoma, intraductal carcinoma, and papillary carcinoma. One case was treated on partial mastectomy and two cases on simple mastectomy with axillary dissection. In view of academic and clinical interacts, we report three cases of male breast carcinoma along with literature review.

The purpose of this study was to develop a new ^13C-urea-containing capsule for diagnosis of H.pylori. The urea-containing capsules were prepared with various diluents such as polyethylene glycol (PEG), microcrystaline cellulose, sodium lauryl sulfate and citric acid. The dissolution test, ^13C-urea breath test and stability test were then performed on the capsules. Microcrystalline cellulose and sodium lauryl sulfate retarded the initial dissolution rates of urea. However, PEG increased the initial dissolution rates of urea. Furthermore, two formulae composed of PEG,[^13C-urea/PEG (38/1.9 mg/cap)] and [^13C-urea/PEG (38/1.9/1.9 mg/cap)] had the maximum DOB value. about 16 at 20 min, while the formula composed of only 38 mg ^13C-urea had the maximum DOB value at 30 min. The results indicated that PEG improved the sensitivity of ^13C-urea in the human volunteers. The capsule [^13C-urea/PEG (38/1.9 mg/cap)] was stable for at least six months in 25 and 37℃. Thus, a PEG-containing capsule, [^13C-urea/PEG (38/1.9 mg/cap)] would be a more economical, sensitive and stable perparation for diagnosis of H. pylori.

Intimate partner violence and incidence of depression in married women: A longitudinal study of a nationally representative sample

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>Intimate partner violence (IPV) has a serious detrimental effect on mental health outcomes. We aimed to investigate the association of verbal or physical IPV with incidence of depressive symptoms in both married women and men according to the victim-perpetrator role. The potential mediating role of verbal or physical IPV in the association between satisfaction level with family relationships or childhood adversity and the incidence of depressive symptoms in married adults was also explored.</P> <P><B>Methods</B></P> <P>The Korea Welfare Panel Study (KOWEPS) in 2006 and 2007 dataset was analyzed for 9217 married respondents aged 19 years or older. Physical and verbal IPV was assessed according to victim-perpetrator role in 2006. Depressive symptoms were evaluated by the Center for Epidemiologic Studies Depression Scale, 11-item version in 2006 and 2007. Association of IPV with incidental depressive symptoms was investigated with logistic regression analysis fully-adjusted for all potential confounding factors.</P> <P><B>Results</B></P> <P>The bidirectional role of verbal IPV and victimization by physical IPV led to incidence of depressive symptoms in married women. Relationship of depression, chronic disease, self-rated health, and gender with health care utilization among community-living elderly

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>We investigated the interactive effects of depressive symptoms and chronic diseases on health care utilization among elderly people and explored the potential moderating effect of gender and the mediating effect of self-rated health (SRH) on the association between depressive symptoms and health care utilization.</P> <P><B>Method</B></P> <P>We analyzed data from 5223 people aged 60 years or older living in the community from the Korea Welfare Panel Study in 2015. Depressive symptoms were measured using an 11-item version of the Center for Epidemiologic Studies Depression (CES-D-11) Scale and morbidity within 28 disease categories was assessed. Health care utilization was evaluated as the number of outpatient visits (OV), number of hospitalizations (NH), and number of days spent in the hospital (DH) during past year. Hierarchical moderated regression analyses were applied to investigate the interactive effects. We also adopted the mediation analysis method by Hayes and Preacher.</P> <P><B>Results</B></P> <P>A significant interactive effect of CES-D-11 score and chronic disease on OV was found. A positive correlation between CES-D-11 score and OV was only observed in those with chronic disease. Gender had a moderating effect on the association of depression symptoms with OV, NH, and DH among elderly people with chronic disease. SRH had mediating effects on the association of CES-D-11 with OV, NH, and DH only among those with chronic disease.</P> <P><B>Limitations</B></P> <P>The severity or multimorbidity of chronic diseases, which could affect health care utilization among elderly were not considered.</P> <P><B>Conclusions</B></P> <P>We elucidated the complex aspects of the relationship between depressive symptoms and chronic disease and their interactive effects on health care utilization among elderly people, and identified important roles of gender and SRH.</P> <P><B>Highlights</B></P> <P> <UL> <LI> A nationally representative sample of 5223 community-living elderly (≥60 years). </LI> <LI> Significant interactive effect of depression and chronic disease on health care use. </LI> <LI> Gender moderated association of depression with health care use in chronically ill. </LI> <LI> Self-rated health mediated the association of depression with health care use. </LI> <LI> The mediation by self-rated health was moderated by gender and chronic disease. </LI> </UL> </P>

Currently, the World Health Organization classifies Langerhans cell tumors into Langerhans cell histiocytosis and Langerhans cell sarcoma (LCS). LCS is a neoplastic proliferation of Langerhans cells showing malignant cytological features and aggressive clinical behavior with grave prognosis. Only a few cases have been reported in the available literature; therefore, to date, no definitive treatment has been established. A 64-year-old woman presented with a 1-year history of an asymptomatic, slow-growing erythematous nodule measuring 0.7 cm on her scalp. The patient also reported a 3-month history of a painful swelling on the right side of her neck. Histopathological examination of a scalp biopsy specimen revealed sheets of atypical cells with hyperchromatic nucleoli and clear cytoplasm. Immunohistochemical studies revealed malignant cells positive for CD1a, CD31, CD68, and S-100 expression. Additionally, positron emission tomography-computed tomography and fine-needle aspiration revealed LCS of the cervical lymph nodes and surrounding soft tissue. We recommended surgical excision and adjunctive chemotherapy; however, the patient refused treatment and died of the disease 28 months later. (Korean J Dermatol 2019;57(9):538∼543)

Rapidly involuting congenital hemangioma (RICH), a subtype of congenital hemangioma (CH), is fully developed at birth and undergoes rapid involution by 6-14 months of age. Clinically, this vascular lesion differs from both non-involuting CH and infantile hemangiomas. It is important to distinguish between this rare condition and other vascular lesions to ensure appropriate management (active intervention vs. non-intervention). We describe 5 patients diagnosed with RICH based on clinicopathological and/or imaging studies. Three patients showed red-stippled bluish to purplish tense plaques and two showed slightly depressed red-stippled hypopigmented rubbery patches at their initial visit. Spontaneous involution commenced shortly after birth, and complete involution occurred by 16.0±9.9 months of age. Doppler ultrasonography and magnetic resonance imaging were performed for further evaluation. Histopathological examination revealed vascular lobules of various sizes without true arteriovenous microfistulae, and thickened intra- and interlobular vessels were prominent with glucose transporter isoform-1 protein negativity.
