미국의 지역확장센터 프로그램을 통한 전자건강기록 도입 확산 정책

이 글에서는 미국의 지역확장센터 프로그램을 통한 전자건강기록 도입 확산 정책을 살펴보고 국내 상황에 대한 시사점을 제시했다. 미국은 전국에 62개의 지역확장센터 프로그램을 신설하여 1차진료의사와 소규모 병원, 농어촌 지역 의료기관들의 전자건강기록의 도입과 인센티브 프로그램의 조건 달성을 지원했다. 구체적인 서비스 영역으로 전자건강기록 도입과 관리 지원, 의료 정보통신 관련 교육·훈련, 제품 업체 선정과 재정 상담, 개인정보보호 및 보안, 지속적인 기술지원 등이 포함되었다. 이러한 지원 활동의 결과, 미국 내 소규모 병원과 농어촌 지역 병원들의 전자건강기록 도입률이 크게 증가했으며, 인센티브 프로그램을 통해 재정 지원을 받는 비율도 높아졌다. 이러한 정책 내용과 성과는 현재 한국의 전자의무기록 인증제에서, 참여가 매우 저조한 소규모 병원과 의원들을 위한 맞춤형 지원 프로그램이 필요함을 시사한다. This article examines how the US has facilitated electronic health record (EHR) adoption through its EHR Incentive Program and Regional Extension Centers (RECs). Established in 62 different locations throughout the US, the RECs have helped primary care physicians, small hospitals, and rural health clinics adopt EHRs and meet the requirements for incentives. This program consisted of assistance in EHR adoption and maintenance, education and training on health information communication, assistance in the protection and safety of private information, and continued technical assistance. The result of such support was a large increase in the adoption of EHRs among small hospitals and rural health clinics. The US case suggests that Korea needs customized support programs for small hospitals and clinics, for whom the rate of participation in the national electronic medical record certification system remains extremely low.
대기오염이 만성질환자에게 미치는 영향과 시사점: 미국 천식 아동의 의료이용 관련 연구 결과를 중심으로

기후변화와 대기오염은 상호 영향을 주고받으며, 전 세계 사람들의 건강에 위협이 되는 주요 원인 중 하나이다. 대기오염의 영향에 더 취약한 민감계층들이 존재하는데, 최근에는 이러한 민감계층의 만성질환 문제가 미래의 상당한 질병 부담으로 예측되고 있다. 이 글에서는 기후변화와 대기오염의 건강 영향에 대해 고찰해 보고, 대기오염이 만성질환을 가진 민감계층의 건강에 어떠한 영향을 주는지에 대해 살펴보았다. 특히 미국 텍사스 남부의 사회경제적으로 열악한 지역에 거주하는 천식 아동들에 초점을 맞춰 미세먼지와 오존을 포함하는 대기오염이 아동 천식 환자의 의료서비스 이용에 미치는 영향을 분석한 세 개의 연구를 각각 고찰하고 우리나라에 주는 시사점을 제시하였다. Climate change and air pollution are interrelated and pose significant challenges to the health of people worldwide. There are vulnerable populations that are more susceptible to the effects of air pollution, and recently, the chronic disease burden among these vulnerable groups has been predicted to become a significant health issue in the future. This article examines the impacts of climate change and air pollution on human health and how air pollution affects the health of vulnerable populations with chronic diseases. Specifically, this article reviews three studies which, focusing on children living in socioeconomically disadvantaged areas of South Texas, investigated the impacts of air pollutants, including particulate matter and ozone, on the utilization of healthcare services by pediatric asthma patients, and draws out some implications for Korea.
