Purpose: Medical thoracoscopy (MT) is generally considered a safe and effective method, not only in the diagnosis of undiagnosed pleural effusion, but also in the management of pleural disease. MT allows both tissue diagnosis and pleurodesis to be achieved reliably in one procedure. But there is not enough study about the effectiveness of frozen sections during procedure. The aim of this study was to assess the feasibility and accuracy of using frozen section analysis of samples taken during MT to assist the operator’s decision to complete the procedure with a thoracoscopic talc poudrage. Method: 138 patients with undiagnosed pleural effusions underwent MT and biopsy. Frozen and paraffin sections were performed by flex-rigid MT. Talc poudrage was performed when frozen section was suspicious to malignancy. The final diagnoses based on paraffin sections were compared to results of frozen sections. Result: Frozen sections were performed during MT in 138 patients. The diagnosis based on frozen sections was malignant in 62 cases and benign in 76 cases. 62 patients of malignancy cases on frozen sections underwent talc poudrage. The final diagnosis based on paraffin sections was malignant in 74 cases, benign in 62 cases and undiagnosed in 2cases. In case of 62 malignant frozen sections, 61 cases (98.4%) were correctly identified by malignancy and 1 case (1.6%) was benign through the final diagnoses of paraffin sections. In case of 76 benign frozen sections, 61 cases (80.3%) were benign, 13 cases (17.1%) were malignancy, and 2 cases (2.6%) were undiagnosed. Conclusion: The diagnoses between frozen sections and paraffin sections are strongly correlated. Frozen sections taken during semi-rigid MT have the potential to facilitate decision-making prior to talc poudrage, particularly in malignant histology.

This study is to investigate cardiotonic effect of acupuncture on heart rate variability(HRV) analyzed by a nonlinear way(DFA, Detrended Fluctuation Analysis). It was designed as a randomized, single-blind, waiting list-controlled, cross-over study. We assessed heart rate and R-R intervals in Circadian electrocardiography with a Holter monitoring device for twelve hospitalized participants. The compatible analytical program, Zymed, was used for generating the signals of R-R intervals from 24 hour-ECG. In DFA analysis, we produced DFA alpha 1, alpha 2 parameters according to the process of Cygwin module of Linux server. We tested if there was any difference between HRV parameters using SPSS, a statistical package. There was no difference between acupuncture and no treatment group in DFA alpha 2 parameter {95% Confidence Interval (-)0.058 - 0.037, P = .565}. Two group all showed large intra-individual variations. Consequently, acupuncture treatment did not modulate the complexity of HRV in a DFA analysis. This study can be a rationale for acupuncture's properties on cardiovascular and autonomic systems.
