        In case of the occurrence of osteonecrosis at three or more anatomical location, it is defined as multifocal osteonecrosis, but still any king of study with references hasn't been reported in Korea. The patient who had admitted due to the pain in both knee joints, both hip joints and left ankle joint and we diagnosed as multifocal osteonecrosis by using bone scan and magnetic resonance imaging. To relieve right knee joint and left hip joint pain, we perfomed arthroplasty. Here we present one case report of multifocal osteonecrosis through analysis by patient's history and hematologic examination and provide a review of the literature.

      • 수 종의 생약제제가 human fetal osteoblasts의 염기성 인산분해 효소 활성에 미치는 영향

        Several growth factors and polypeptides are not commonly yet used for regenerators of bone tissue or alveolar bone because of the insufficiency of studies on their side effects, genetic engineering for mass production and stability for clinical application. Recently, many herbal medicines, which have advantage of less side effects and possibility of ling-term use, have been studied for their capacity and effects of anti-bacterial, anti-inflammatory and regenerative potential of periodontal tissues. Morindae Radix, Cibotium Barometz (L.), Albizziae Cortex, Cistandhis Herba have been traditionally used as medicines for treatment of bone disease in Eastern medicine. The objective of the present study is to examine the ability of alkaline phosphatase (ALP) activity of human fetal osteoblast (hFOB1) when several natural medicines were supplemented. hFOB1 were cultured with Dulbecuo's Modified Eagle's Medium Nutrient Mixture F-12 HAM ( DMEM/F-12 1:1 Mixture, Sigma, USA) and negative control, dexamethasone (positive control), and each natural medicines for 3 days. And then ALP activity was measured by spectrophotometer for enzyme activity and Alizarin red S staining for morphometry. Among the natural medicines of this study, Morindae Radix, Cibotium Barometz (L.) and Cistanchis Herba induced higher activity of ALP systhesis than negative controls in all experimental group. Albizziae Cortex showed mild increases than negative control group. According to measurement of positively stained area, all of the natural medicines of this study increased compared to negative control. Especially, Cibotium Barometz (L.) and Cistanchis Herba showed statistical significance compared to negative control (p<0.05). These results indicate that Morindae Radix, Cibotium Barometz (L.), Albizziae Cortex, Cistandhis Herba have an inducing ability of ALP synthesis on osteoblast.

      • 우골 유도 합성골이 사람 태아 골모세포의 골 광물화 과정에 미치는 영향

        The ultimate goal of periodontal therapy is to promote the regeneration of lost periodontal tissue, there have been many attempts to develop a method to achieve this goal, but none of them was completely successful. The purpose of this study is to evaluate the effects of Bio-oss on Alkaline Phosphatase(ALP) Activity in Human Fetal Osteoblasts (hFOB1). The results of this study were as follows, in ALP Activity, 100 ㎍/㎖ Bio-oss treated group showed significantly increased value than negative control group, but positive group(10^-7 M dexamethasone treated group) showed the highest ALP activity at 3 day. In mineralization assay, numerous mineralized nodules were identified as darkly stained spots in 100 ㎍/㎖ Bio-oss treated group than two control groups, whereas a small number of mineralized nodules were showed in the positive control. ALP may relate to the initial phase of bone nodule formation. On the basis of these results, this study showed Bio-oss is capable of accelerating new bone formation through hFOB1 differentiation in vitro.

      • 월견초 종자추출물 투여에 의한 생체면역 반응 억제 및 그 기전

        Recently, essential fatty acids(EPA) have started commanding clincal interest. Their vast possibilities for therapeutic use make basic knowledge of their chemical nature and physiologic significance a necessity. EPAs are important not only in themselves but also as precursors of other polyunsaturayed fatty acids. Evaing primrose extract(EPE) is a naturally occurring rich source of EPA especially linoleic and gamma-linolenic acid which are biosynthetic precursors of prostaglandins(PG). SO gamma-linolenic acid in the form of particular variety of EPE has been extensively reported of value in the treatment of various disorders resulting from the abnormalities of EPA. However, surprisingly little has been done to develope and exploit EPE’s immunomodulating properties. Moreover, the mechanisms of EPE action on ommune function has not been elucidated. This study was designed to evaluate the effect of EPE on the cellular and humoral immunr responses of mice, and to investigate the mechanisms responsible for the immunomodulating action of EPE. Mice sensitized with ?? SRBC were injected with EPE(1mg/mouse,ip) for 4 days before SRBC, and/or with cholesterol(CHO,10mg/mouse,sc) on day O of SRBC, or with indomethacin(an inhibitor of PG-synthetase, IDM, 0.15mg/mouse,ip) on day-1 and O of SRBC, and challenged with a dose of ?? SRBC on day 4 of sensitization. The effect of EPE on immune response and the effect of CHO and IDM on EPE-induced immunosuppression were evaluated by measuring the footpad sweelling reaction and antibody response to SRBC. EPE itself did not influence arthus reaction but remarkably reduced delayed type hypersesitivity(DTH) and antibody production in comparison with the corresponding control. While CHO slightly increased Arthus reaction and DTH but it somewhat decreased antibody responses. IDM significantly enhanced both cellular and humoral immune response. And when CHO or IDM was administered into mice in combination with EPE, both of them significantly blocked the immunosuppressive activities of EPE at the almost same degree. Taken together, the present study revealed that EPE iteslf has immunosuppressive principle, and that this nonspecific immune-inhibitory effect of EPE might be manifested partially via the decrement of the CHO-levels and partially via the enhancement of PG synthesis.

      • 사람태아골모세포에 대한 GBM의 골형성 유도에 관한 효과

        The purpose of the present study was to investigate the effects of GBM on the activity, differentiation and alkaline phosphatase(ALP) synthesis of hFOB1s. To examine the cellular activity, hFOB1s were cultured with DMEM/F-12 1:1 Mixture and 100 ㎍, 2 mg, 5 mg, 10 mg of GBM for 2 days and 4 days. To compare the ALP synthesis, hFOB1s were cultured with DMEM/F-12 1:1 Mixture and 100 ㎍, 2 mg, 5 mg, 10 mg, 20 mg of GBM for 3days. The cellular activity of hFOB1s treated with 100 ㎍ of GBM was increased at 2 and 4-day(p>0.05) to control. The activity of ALP in hFOB1s treated with 100 ㎍ GBM was significantly increased at 3 day(p<0.05). This study indicated that 100 ㎍ of GBM has an inductive effect on bone formation in vitro increasing with cell proliferation, ALP activity.

      • 사람태아골모세포에 대한 DLB의 골형성 유도에 관한 효과

        The purpose of this study was to investigate the effects of DLB on the cell growth and differentiation of human fetal osteoblasts. Human fetal osteoblasts(hFOB1) were cultured to examine the cellular activity for 2 days and 4 days with 100 ㎍, 2 mg, 5 mg, 10 mg of DLB, and to compare the ALP synthesis to control groups for 3 days with DMEM/F-12 1:1 Mixture and 100 ㎍, 2 mg, 5 mf, 10 mg, 20 mg of DLB. The cellular activity of hFOB1s treated with 100 ㎍ of DLB was increased at 2 and 4 day to control(p>0.05). The activity of ALP in hFOB1s treated with 100 ㎍ DLB was significantly increased at 3 day(p<0.05). This study indicated that 100 ㎍ of DLB has an inductive effect on bone formation in vitro increasing with cell proliferation and ALP activity.

These results indicate that Cortex Eucommiae, Semen Cuscutae, Eupoly phaga, Halloysitum Rubrum have an inducing ability of ALP synthesis on osteoblast.

        방사선 사진과 디지탈 영상에서 실험적 치근 이개부 병소의 감지도에 관한 연구

        The aim of this study was to evaluate clinician's detectability in the diagnosis of bone loss in the bifurcation of mandibular molars on periapical radiographs and Digital images. Periapical radiographs were obtained of the first molars in 2 dry mandibles after preparation of bony defects corresponding to degree Ⅰ, degree Ⅱ and degree Ⅲ buccal furcation involvements. The radiographs were randomly presented to 39 clinicians(1 oral radiologist, 4 periodontist, 34 general dentists) who were asked to determine the presence or absence of bone loss. Periapical films were digitized with a TV camera. Digital images were assessed by 15 clinicians(1 oral radiologist, 4 periodontist, 10 general dentists). 1.the overall diagnostic accuracy of Digital images for detection of bone loss in the bifurcation of mandibular molars was higher than that of the periapical radiographs. 2.the largest increase in diagnostic accuracy was found between lesion grade Ⅱ and Ⅲ on both radiographs and Digital images(P<0.05). 3.there was no significant difference between the standard state and the controlled contrast state on Digital images. 4.the overall diagnostic accuracy of 1 radiologist and 4 periodontists was better than that of the general dentists for detecting bifurcation involvements.

      • 탈희동결건조골과 이종골이 사람태아 골모세포의 골형성에 관한 효과

        The ultimate goal of periodontal disease therapy is to promote the regeneration of lost periodontal tissue, there have been many attempts to develop a method to achieve this goal. The purpose of this study was to evaluation the effects of BBP, DLB on bone formation in human fetal osteoblasts. We examined the effect of BBP, DLB on collagen synthesis of human fetal osteoblasts. Collagen synthesis level of 5mg of BBP(5.80±0.18㎍/mg/ml), DLB(5.51±0.57㎍/mg/ml) showed significant difference compare with control group(3.58±0.35㎍/mg/ml). We also examined expression of BMP-2 by Western blot analysis. BMP-2 was expressed in both of BBP and DLB. These results suggest that BBP, DLB have an inductive effect on bone formation in vitro increasing with collagen synthesis and expression of BMP-2.
