초고온 호기성 퇴비화 공정의 복합 악취 및 악취 물질 평가

본 연구는 정읍시에서 배출되는 하수 슬러지, 축산 분뇨, 음식물 쓰레기를 대상으로 Pilot Scale(100㎥) 초고온 호기성 퇴비화 공정에서의 복합 악취 및 12개 지정 악취물질을 평가하였다. 각각의 대상 물질은 종균과 혼합하였고 1차 50일, 2차 60일 기간동안 운전하였다. 호기성 퇴비화가 진행됨에 따라 1차, 2차 퇴비화공정에서 온도가 상승하여 약 90~95℃가 되었다. 악취 분석을 위한 시료는 1차의 경우 퇴비상부에서, 2차의 경우 Pilot Plant 경계지점에서 채취하였다. 지정 악취 물질 12개 항목의 기기 분석에서 악취의 주요 원인으로는 암모니아, 메틸머캅탄, 디메틸다이설파이드, 트리메틸아민이었다. 초기 복합악취의 농도는 퇴비화 공정이 끝난 후의 농도보다 높았지만 규제기준을 넘지 않았다. 초고온 호기성 퇴비화 공정에서는 적절한 교반시점과 발효 온도가 악취 발생의 중요한 인자라고 판단된다. This study was conducted to evaluate production of complex odor and 12 specific odorous compounds in a pilot-scale (capacity : 100 ㎥) ultra thermophilic aerobic composting. There were three types input: municipal wasted sludge, livestock manure and slurry, and food waste produced in Jung-Eb city. Each raw material was mixed with seed material and operated for two periods (1st : 50 days, 2nd : 60days). During composting, the temperature hit 90~95℃ after every mixing in both periods. Therefore, it was concluded that increasing temperature also saves the time which required for composting and high reduction of organics and water contents. The primary odorous compounds were ammonia, methyl mercaltan, dimethyl disulfide and trimethylamine. The concentration of the primary compounds and complex odor during the operation were higher than those on final day and most compounds did not exceed the allowable exhaust standard for odor. Also, it was found that optimal mixing time and control of high temperature are the most important parameters for odor control in ultra thermophilic aerobic composting.
Porokeratosis at the site of previous moxibustion: a case report

Porokeratosis is characterized by annular keratotic rims. The histopathological finding that characterizes this condition is the cornoid lamella, which is a column of parakeratotic cells. It has been suggested that external stimuli, including immunosuppressive therapies, ultraviolet light, and trauma may serve as etiopathogenetic contributors. We report a case of porokeratosis occurring at the site of previous moxibustion. A 59-year-old man visited our clinic with an annular plaque on the right buttock. The plaque showed an atrophic center with a slightly elevated keratotic border. The patient revealed a history of undergoing moxibustion of the lower back and buttocks, 8 years prior to presentation. Unfortunately, he experienced a burn at the site of moxibustion, which was located on the right buttock, and developed a brownish plaque at the site of the burn. The patient was healthy and denied a family history of similar skin lesions at his initial visit. His histopathological examination revealed hyperkeratosis, acanthosis, and cornoid lamella in the epidermis without a granular layer beneath it. Based on the clinical and histopathological findings, the patient was diagnosed with porokeratosis and was treated with 3 cycles of 0.05% ingenol mebutate gel. The lesion decreased in size following 2 weeks of treatment. The residual lesion was treated with carbon dioxide laser, which led to the disappearance of the keratotic annular plaque.

Background: The purpose of this study was to evaluate the quantitative association between the degree of reduction and the position of the blade of the proximal femoral nail antirotation (PFNA) in intertrochanteric hip fractures. Methods: From March 2009 to April 2015, 530 patients treated with PFNA for intertrochanteric hip fractures were retrospectively reviewed. Patients were divided into a valgus reduced group (group 1) and a non-valgus reduced group (group 2), and the “valgus reduced” was defined as valgus reduction over 5°. We compared the calcar referenced tip-apex distance (calTAD) and the area between the blade of PFNA and the medial cortex of the femoral neck between the two groups. Results: The calTAD was measured as 22.5 ± 4.1 mm in group 1 and 24.8 ± 3.8 mm in group 2 (p < 0.05). The area between the blade and the medial femoral neck was measured as 135.5 ± 49.8 mm2 in group 1 and 145.1 ± 54.8 mm2 in group 2 (p = 0.074). The area corrected for the length difference in the femoral neck was 0.55 ± 0.16 in group 1 and 0.79 ± 0.19 in group 2 (p < 0.05). Conclusions: Valgus reduction resulted in less calTAD and inferior position of the blade at the femoral neck in the treatment of intertrochanteric hip fractures with PFNA.
Three cases of successful treatment of resistant tinea capitis with griseofulvin

Tinea capitis is a fungal infection of the scalp and hair follicles, predominantly affecting prepubertal children. Systemic antifungal therapy is generally recommended, and griseofulvin is the drug of choice in children. The first patient was a 5-year-old female presenting with pruritic inflammatory erythematous patches with broken hair on the vertex for a month. The second patient was a 7-year-old female presenting with pruritic round erythematous scaly patches on the scalp for 3 months. The third patient was a 4-year-old male presenting with multiple yellowish inflammatory plaques with bald patches on the scalp for 3 months. Scales and broken hairs on KOH examination revealed several hyphae and spores. Deep yellow and orange pigment on the reverse of the colony was observed. Identification of M.canis infection was made by polymerase chain reaction. All three patients were resistant to oral terbinafine, and two of them were also insusceptible to itraconazole therapy. At last, griseofulvin was administered 15mg/kg/day for 2 weeks, which resulted in significant clinical improvement. Herein, we report three cases of M. canis infection that showed resistance to several antifungal agents except for griseofulvin.
A case of malar rash in angioimmunoblastic T-cell lymphoma

Angioimmunoblastic T-cell lymphoma is an aggressive subtype of peripheral T-cell non-Hodgkin lymphoma with diverse cutaneous manifestations. Because there are no specific diagnostic criteria for cutaneous clinical and histopathological findings, reaching a correct diagnosis solely based on dermatological examination is immensely challenging. A 59-year-old female presented with pruritic widespread erythematous confluent papules and plaques with edematous malar rash on both cheeks, mimicking systemic contact dermatitis, drug eruptions, and lupus erythematosus. However, there were no significant foods, allergens, and changes of drugs related to the causes. Skin biopsy revealed dermal perivascular infiltrates of lymphocytes and neutrophils with no specifically confirmed disease entity. Steroid pulse therapy was started, which seemingly showed a clinical improvement; however, generalized edematous skin eruption recurred with fever and swelling of the neck. Lymph node biopsy finally revealed angioimmunoblastic T-cell lymphoma. The patient was eventually transferred to the department of medical oncology and hematology for chemotherapy, which was ineffective to stop the fatal outcome. Herein, we report a patient, who was initially diagnosed as systemic contact dermatitis, later confirmed to be angioimmunoblastic T-cell lymphoma upon lymph node biopsy.

Granuloma annulare (GA) is a benign, idiopathic, self-limiting granulomatous dermatosis, characterized by small, firm, flesh-colored or erythematous papules with a well-demarcated ring-like borders and atrophy in the center. The most common locations of localized GA are dorsal or lateral aspects of hands and feet. To the best of our knowledge, only several cases of auricular GA have been reported in the literature. We report a unique case of bilateral auricular granuloma annulare involving both antihelices and helices, in a 68-year-old Korean male who presented with asymptomatic scattered skin-colored firm papules and with no history of trauma.

Background: It has been reported that loss of sirtuin 6 (Sirt6) exhibit impaired healing in skin wounds. Given the heterogeneity of macrophage phenotypes during wound repair and the regulation of macrophage phenotypes by sirtuins, we here hypothesized that Sirt6 deletion in myeloid cells would affect macrophages phenotypes in skin wounds and delay wound closure. Objectives: To find out the role of Sirt6 regarding macrophage phenotype in wound healing. Methods: To address this question, floxed Sirt6 mice were bred with lysozyme M-Cre mice to generate myeloid Sirt6 knockout (KO) mice with reduced Sirt6 expression. A full-thickness excisional lesion was made on the dorsal skin of the mice. Results: Compared with wild type mice, wound closure was delayed in KO mice with less collagen deposition, attenuated vWF expression and reduced expression of wound healing related genes. Using immunohistochemistry and RNA expression analyses on macrophage subpopulations from wound tissues, we identified an increased infiltration of M1 type macrophages in KO mice compared with wild type mice. Consistent with the in vivo defects in wound closure, the in vitro study demonstrated that keratinocytes and fibroblasts treated with KO macrophage conditioned medium migrated slower than those of wild type. Conclusion: These results suggest that an imbalance of macrophage phenotypes by Sirt6 deletion contributes to impaired wound healing in mice.

<B>ABSTRACT</B><P>A point mutation (E115K) resulting in slower growth of <I>E</I><I>scherichia</I><I>coli</I> DH5α and XL1-Blue in minimal media was identified in the <I>purB</I> gene, coding for adenylosuccinate lyase (ASL), through complementation with an <I>E. coli</I> K-12 genomic library and serial subcultures. Chromosomal modification reversing the mutation to the wild type restored growth phenotypes in minimal media.</P>
