        TheSūtraofGreatDhāraṇīoftheUṣṇīṣa-cittāisoneofthemostwidelypublishedscripturesofEsotericBuddhism duringtheGoryeo andJoseondynasty.ThesutraisoftenreferredtoasaBuddhistmedicalscripturewhoserecitationisbelievedtoallaydiseasessuch asdifficultbirth,chestpain,andinfectiousandincurablediseases.Itutilizestwomajormethodsforhealing:recitingandcopyingthe sutraorcopyingthedharaniandtalismanwhichappearattheendofthetextwithfinecinnabarandingestingthem witharomatic water.In ordertounderstand the medicaland historicalvalue ofthissutra,thisstudyexaminesthehistoricalbackground ofits establishmentinearlyChinaanditsmigrationtoKoreaintheGoryeoperiod.Thisstudyalsoinvestigatestheidentityandvalueofthe scriptureinBuddhistmedicalhistory.TheoldestknownprintedtextsofthesutrainKoreaaredatedtothelate11thcentury.Because ofthematerialbenefitsofthetext,thetraditionofprintinganddistributingthesutrawasverypopularbetweenthe13thand19th centuries.Therefore,examples ofthe sutra having been used formedicinalpurposes during rites are found in historicalrecords. Finally,thestudytriestoidentifytherelationshipbetweenthetreatmentmethodsoftheSutraandthoseofKoreantraditionalmedicine focusingonseveralKoreantraditionalmedicaltextssuchastheHyangyak-Jibseongbang(CompilationofNativeKoreanPrescriptions), theDonguibogam,andtheEuibang-Hapbu.

        현행 중학교 과학 실험·실습교육에 대한 교사들의 견해와 개선방안

        The status of facilities, management of the experiment, practices, and Science education teaching method in middle school have been investigated. The present status and reasonable management of middle school science education have been grasped from the questionaires. The results are as follows : The reasonable management for expeiments and practices of science education were scanty in the middle school around the urban and rural schools owing to the shortage of facilities and equipments, crowded class, excessive classes, work for teachers, excessive contents of present textbook and insufficience of administrative support etc. The current teaching method of middle school science has emphasized knowledge. This fact does not satisfy the objective of learning due to lack of teaching method. Desirable directions for the improvement of present status of middle school science education were proposed in this paper.

        고등학교 지구과학 실험·실습 요목 비교 분석

        The purpose of this study is to investigate the status of the experimental and practical education in high school earth. The present status and reasonable management of the experimental and practical education in high school earth science have been grasped from the questionaires. To do this, eighty eight earth science teachers in Korea are administered questionaires. The frequency of the experimental and practical items in the seven kind of earth science text books were investigated. The problems and the reasonable management for experimental and practical education were proposed in this paper.

      • Irradiation and additive combinations on the pathogen reduction and quality of poultry meat.

        <P>Reduction of foodborne illnesses and deaths by improving the safety of poultry products is one of the priority areas in the United States, and developing and implementing effective food processing technologies can be very effective to accomplish that goal. Irradiation is an effective processing technology for eliminating pathogens in poultry meat. Addition of antimicrobial agents during processing can be another approach to control pathogens in poultry products. However, the adoption of irradiation technology by the meat industry is limited because of quality and health concerns about irradiated meat products. Irradiation produces a characteristic aroma as well as alters meat flavor and color that significantly affect consumer acceptance. The generation of a pink color in cooked poultry and off-odor in poultry by irradiation is a critical issue because consumers associate the presence of a pink color in cooked poultry breast meat as contaminated or undercooked, and off-odor in raw meat and off-flavor in cooked meat with undesirable chemical reactions. As a result, the meat industry has difficulties in using irradiation to achieve its food safety benefits. Antimicrobials such as sodium lactate, sodium diacetate, and potassium benzoate are extensively used to extend the shelf-life and ensure the safety of meat products. However, the use of these antimicrobial agents alone cannot guarantee the safety of poultry products. It is known that some of the herbs, spices, and antimicrobials commonly used in meat processing can have synergistic effects with irradiation in controlling pathogens in meat. Also, the addition of spices or herbs in irradiated meat improves the quality of irradiated poultry by reducing lipid oxidation and production of off-odor volatiles or masking off-flavor. Therefore, combinations of irradiation with these additives can accomplish better pathogen reduction in meat products than using them alone even at lower levels of antimicrobials/herbs and irradiation doses. Effects of irradiation and additive combinations on the pathogen reduction and quality of poultry meat will be discussed in detail.</P>

        Patterns and Outcomes of the Top of the Basilar Artery Syndrome: The Role of the Posterior Communicating Artery

        <P><B><I>Background:</I></B> Clinical syndromes secondary to infarcts in the distal basilar artery (BA) area have been described as “top of the basilar” (TOB) syndrome. However, in the era of advanced imaging technology, it has been realized that the clinical and imaging features are quite diverse in patients with distal BA occlusion. The aim of the present study was to investigate the patterns and clinical outcomes of TOB assessed with modern images and categorize TOBs accordingly. Additionally, we examined the possible influence of the posterior communicating artery (PcoA) on the patterns of TOB. <B><I>Methods:</I></B> Patients with distal BA occlusion on magnetic resonance angiography were categorized as TOB-A, and those with multiple lesions in the distal BA territory on diffusion-weighted magnetic resonance imaging as TOB-L. Patients with angiographically and lesion distribution-defined TOB were classified as having TOB-A&amp;L; those with angiographically defined TOB as having TOB-A without TOB-L; and those with lesion distribution-defined TOB as having TOB-L without TOB-A. The PcoA was categorized as “textbook-type” (good P1) and “fetal-type” (absent P1). Factors associated with unfavorable short-term outcomes (modified Rankin Scale 5–6 at discharge), and 1-year and long-term mortalities, were assessed. <B><I>Results:</I></B> Of 1,466 patients with ischemic stroke in the posterior circulation who were admitted to Asan Medical Center within 24 h of symptom onset, 124 (8.5%) had TOB, including 45 with TOB-A&amp;L, 44 with TOB-A, and 35 with TOB-L. NIHSS scores (21 [9.5–26] vs. 6 [3–11.5] vs. 6 [3–9]; <I>p</I> < 0.01) and rates of motor deficit (75.6 vs. 54.5 vs. 34.4%; <I>p</I> < 0.01), concomitant pontine lesions (17.8 vs. 25.0 vs. 2.9%; <I>p</I> < 0.01), PcoA presence (44.4 vs. 68.2 vs. 25.7%; <I>p</I> < 0.01), and unfavorable short-term outcomes (62.2 vs. 25.0 vs. 14.3%; <I>p</I> < 0.01) differed significantly in the 3 patient groups. Multivariate analysis showed that textbook-type PcoA was independently associated with a lower frequency of unfavorable short-term outcomes (OR 0.15, 95% CI 0.03–0.70). Reperfusion therapy (hazard ratio [HR] 0.25, 95% CI 0.07–0.89) and the presence of textbook-type PcoA (HR 0.20, 95% CI 0.05–0.90) were associated with a lower 1-year mortality rate after stroke. <B><I>Conclusion:</I></B> Patterns and clinical outcomes of TOB vary and are affected by the hemodynamic status of the arterial system, such as BA recanalization and the presence of textbook-type PcoA.</P>

        Isolated MCA Disease in Patients Without Significant Atherosclerotic Risk Factors : A High-Resolution Magnetic Resonance Imaging Study

        <P><B>Background and Purpose—</B></P><P>Diagnosis of intracranial artery atherosclerosis remains often uncertain. The high-resolution magnetic resonance imaging (HR-MRI) enables vessel wall assessment for more precise diagnoses. The aim of the present study was to investigate the etiologies of middle cerebral artery steno-occlusive disease in young adult patients with few atherosclerotic risk factors using HR-MRI.</P><P><B>Methods—</B></P><P>We prospectively studied patients who visited a tertiary hospital in Seoul, Korea, and had (1) unilateral middle cerebral artery disease (≥50% stenosis or occlusion), (2) were ≤55 years old and had no or minimal (≤1) atherosclerotic risk factors. We excluded patients with a confirmed diagnosis of Moyamoya disease, vasculitis, or dissection and those having emboligenic sources. A presumptive diagnosis was made based on HR-MRI findings, and patients were categorized as HR-athero (atherosclerotic disease), HR-MMD (Moyamoya disease), HR-dissection, or HR-vasculitis.</P><P><B>Results—</B></P><P>Among 95 patients analyzed, 26 (27.4%) had HR-athero who were more often male (<I>P</I>=0.004), smokers (<I>P</I>=0.018), and had focal stenosis (<I>P</I>=0.003) than others. As compared with the HR-athero patients, 29 hours-MMD patients were more often female (<I>P</I><0.001) and more often had occlusive lesions (<I>P</I>=0.001) and nonfocal stenosis (<I>P</I><0.001). The 22 hours-dissection patients tended to have hypertension less often, and the 13 hours-vasculitis patients were younger (<I>P</I>=0.004) and tended to have nonfocal stenosis.</P><P><B>Conclusions—</B></P><P>In our cohort of young patients with minimal risk factors, atherosclerosis seems to be an uncommon pathology of middle cerebral artery stenosis. HR-MRI aids us to make a more reliable diagnosis.</P>

        Prognostic Significance of Troponin Elevation for Long-Term Mortality after Ischemic Stroke

        <P><B>Background and Purpose</B></P><P> Troponin, a marker of myocardial injury, frequently increases and is related with poor outcome in ischemic stroke patients. However, the long-term outcome of this elevation remains uncertain. We, therefore, investigated the prognostic significance of troponin elevation for long-term mortality, and explored factors affecting troponin elevation after ischemic stroke.</P><P><B>Methods</B></P><P> We retrospectively analyzed the medical data of stroke patients who were admitted within 24 hours of symptom onset and underwent a serum cardiac troponin I test at admission during a five-year period. Information on mortality as the outcome was obtained from the National Death Certificate system.</P><P><B>Results</B></P><P> A total of 1,692 patients were eligible for inclusion with 33 months of median follow-up. Troponin elevation that exceeded the 99th percentile (>0.04 ng/mL) of values was detected in 166 patients (9.8%). After adjusting for baseline characteristics, troponin elevation was associated with previous ischemic heart disease and congestive heart failure, comorbid atrial fibrillation and active cancer, and increased National Institutes of Health Stroke Scale score. Patients with troponin elevation had a high risk of overall death (hazard ratio [HR] 1.83, 95% confidence interval [CI] 1.40–2.40), including stroke-related (HR 1.71, 95% CI 1.14–2.55), cardiac-related (HR 3.17, 95% CI 1.49–6.74), and cancer-related (HR 1.98, 95% CI 1.14–3.45) death than those without troponin elevation.</P><P><B>Conclusions</B></P><P> Troponin elevation in the acute stage of ischemic stroke was associated with long-term mortality, mainly due to increased stroke- and cancer-related death in the first year and cardiacrelated death in the later period.</P>

        Effect of Packaging and Antioxidant Combinations on Physicochemical Properties of Irradiated Restructured Chicken Rolls

        Effects of double packaging (combinational use of aerobic and vacuum conditions) and antioxidants on physicochemical properties in irradiated restructured chicken rolls were determined. Chicken breast treated with antioxidants (none, sesamol+a-tocopherol) was used to process restructured chicken breast rolls. The sliced rolls were vacuum, aerobic, or double packaged (vacuum for 7 d then aerobic for 3 d) and electron beam irradiated at 2.5 kGy. Color, 2-thiobarbituric acid reactive substances (TBARS), oxidation reduction potentials (ORP), and volatile profiles of the samples were determined at 0 and 10 d. Irradiation made restructured chicken rolls redder (p<0.05), and the increased redness was more distinct in irradiated vacuum-packaged than irradiated aerobic or double packaged meats. TBARS values of antioxidant-treated double packaged rolls were lower than even nonirradiated vacuum-packaged meat, and those were distinct at 10 d (p<0.05). ORP and lipid oxidation values were lower in irradiated vacuum and double packaged samples than those in irradiated aerobic packaged ones at 0 d (p<0.05). Irradiation of restructured chicken rolls increased the amount of total volatiles. Considerable amounts of off-odor volatiles were reduced or not detected by double packaging and antioxidant treatment at 10 d. Therefore, the combined use of antioxidants and double packaging would be useful to reduce redness and control the oxidative quality changes of irradiated restructured chicken rolls
