        Benztropine Mesylate가 Haloperidol 타액농도에 미치는 영향에 대한 연구

        Nineteen patients with schizophrenia by the DSM-III-R were administrated with only haloperidol in the first period. And the second period began after extrapyramidal symptoms developed, in this period they were administrated with haloperidol and benztropine concomittantly. At the third period, after withdrawal of benztropine, they were administrated with only haloperidol. For the purpose of evaluating the effect of benztropine mesylate on the haloperidol saliva level, the authors measured the haloperidol saliva level, the reduced haloperidol saliva level and the ratio of the reduced haloperidol saliva level/the haloperidol saliva level at the seperated administration period. The results are as follows : 1) Though the haloperidol saliva level of the single administration period of haloperidol was higher than that of other two periods, there was no statistically significant change. 2) The reduced haloperidol saliva level had no statistically significant change among three periods. 3) The reduced haloperidol saliva level/the haloperidol saliva level ratio had no statistically signifiant change among three periods. The authors concluded that the benztropine mesylate had no significant effect on the haloperidol saliva level in this study.

      • 은행경영지배구조 개선에 관한 연구

        경영진견제 측면만이 부각된 현 사외이사제도 중심의 은행경영지배구조는 주주가치의 극대화 및 경영효율성 제고와 투명성 확보차원에서 보면 다음과 같은 문제점이 있다. 첫째로 이사회 및 경영진의 역할이 불명확하여 경영의 성과제고를 어렵게 하고 있으며, 둘째로 대부분 위원회 의결기능이 결여되어 제 기능을 발휘하지 못하고 있고, 셋째로 개별이사 및 이사회를 평가할 수 있는 구체적인 기준이나 절차가 거의 없어 이사의 교체방안이 없다. 마지막으로 대부분 은행의 경우 경영진의 성과평가 절차 및 기준이 없거나 명확하지 못하여 경영진에 대한 보상이 제대로 이루어지지 않고 있다. 이와 같은 문제점에 대한 개선책으로는 첫째로 은행은 은행장과 이사회의장 및 이사회와 위원회의 역할과 의무를 구체적으로 명시하는 문서를 구비할 필요가 있으며 이사회 지원기능을 강화하고 경영진과 이사회간의 창구를 단일화시킬 필요가 있다. 둘째로 각 위원회에 권한을 대폭 이양하여 위원회의 기능을 활성화시킴과 동시에 이사회로의 보고체계를 확립시킬 필요가 있고, 셋째로 각 은행들의 이사회는 이사회 및 이사에 대한 성과평가를 매년 의무적으로 실시하고 평가 결과를 바탕으로 이사회의 재구성이 가능하도록 해야 할 필요가 있으며, 마지막으로 각 은행들은 은행장과 임원의 경영성과가 충분히 보상될 수 있도록 합리적인 성과평가 절차를 구축하고 보상수준과 구조를 개선할 필요가 있다.

        A synchronous motor started as an induction motor must be synchronized as soon as possible by the application of field at any point. Unfortunately, the dynamic motion equations, describing the synchronized process, that is, describing the pulling-into-step process, are represented by complicate and nonlinear differential equations which have no analytical techniques. However, practical criteria for predetermining with adquate accuracy whether a given motor will or will not pull into step under specified conditions, must be necessarily given. Futhermore, it is impossible to obtain the accurate solution of the closed form by conventional techniques. Even though they are obtained, they does not show the stable regions over 4 quadrants. Specially, the accurate observations about the continuous variations of slip and load angle are very important for the stablized control of synchronous motor and all systems with them. The abnormal and unstable operations such as hunting pulling out and etc. are fatal to the life of machine itself and all systems with them. Therefore, in this paper, the stable regions in the sense of strictness over 4 quadrants, the analytical and the experimental techniques, for cylindrical synchronous motor are introduced in detail. These techniques used in this paper are based on the construction and analysis of Liapunov function, derived from the Lagrange-Char-pit method which is one of the solution techniques for partial differential equations. The theory of the various stable regions and optimal points for applications of field, which can be predicted the success or the failure of pulling-into-step under the variations of load ratio and damping ratio is presented and also is proved by experimental results.

        It is well known that some of piles are subjected to lateral earth pressures due to lateral soil movements, such piles are called as passive piles. Because the lateral behavior of pile in a group is developed by many factors involved in their deformation and strength characteristics, the mechanisms of the passive pile is not completely clarified yet. This paper describes a series of model tests on instrumented pile groups embedded in HAP-CHEN sand undergoing lateral movement, the extent of the group effect on the lateral response of a pile in a group was found to be dependent on a number of factors, including the position of the pile in a group, the pile spacing, and the pile arrangement.

        자동차 탑승자 사고에서 외상계수를 이용한 구급대원의 중증도 분류 평가

        Purpose: The assessment of trauma patients in the prehospital setting is difficult, but appropriate field triage is critical to the prognosis of trauma patients. We sought to evaluate the triage given by the emergency medical technicians (EMTs) using the trauma score to patients injured in motor vehicle collisions (MVCs). Methods: From June 2012 to July 2012, questionnaires were distributed to EMTs, who had transported injured patients to the study hospital. Scene records, photos of the damaged vehicle, and ambulance run sheets were used to provide physiologic, physical, and mechanistic information about the MVC. To evaluate the appropriateness of the injury assessment by EMTs, we compared their impressions with the hospital`s final diagnosis within a 3 level triage system comprising both the maximum abbreviated injury scale (MAIS) and the injury severity score (ISS). Kappa (k) was calculated to evaluate the agreement between the triage by EMTs and the triage based on hospital`s final diagnosis. Results: A total of 91 patients were analyzed by 31 EMTs. The percentage of males was 57.1%, the mean age was 44.5, and the mean MAIS and ISS were 2.7 and 16.6 respectively. While EMTs correctly diagnosed patient injuries to the extremities in 35.7%, and to the neck in 32.1%, pelvic injuries were missed in 80.0%. The agreement between the triage by the EMTs and the triage based on the hospital`s final diagnosis was 62.6%(k=0.366) by the MAIS and 50.5%(k=0.234) by the ISS. The kappa value was higher in EMT-I than in EMT-II. Conclusion: In MVC, the assessment of injured patients by EMT-I was more appropriate, and the 3-level triage method based on the MAIS could contribute to a more accurate triage. Prospective studies to search for appropriate methods of field triage are required for programming practical education for EMTs.

        Incorporating Erlotinib or Irinotecan Plus Cisplatin into Chemoradiotherapy for Stage III Non-small Cell Lung Cancer According to <i>EGFR</i> Mutation Status

        <P><B>Purpose</B></P><P>Concurrent chemoradiotherapy (CCRT) is the standard care for stage III non-small cell lung cancer (NSCLC) patients; however, a more effective regimen is needed to improve the outcome by better controlling occult metastases. We conducted two parallel randomized phase II studies to incorporate erlotinib or irinotecan-cisplatin (IP) into CCRT for stage III NSCLC depending on epidermal growth factor receptor (<I>EGFR</I>) mutation status.</P><P><B>Materials and Methods</B></P><P>Patients with <I>EGFR</I>-mutant tumors were randomized to receive three cycles of erlotinib first and then either CCRT with erlotinib followed by erlotinib (arm A) or CCRT with IP only (arm B). Patients with <I>EGFR</I> unknown or wild-type tumors were randomized to receive either three cycles of IP before (arm C) or after CCRT with IP (arm D).</P><P><B>Results</B></P><P>Seventy-three patients were screened and the study was closed early because of slow accrual after 59 patients were randomized. Overall, there were seven patients in arm A, five in arm B, 22 in arm C, and 25 in arm D. The response rate was 71.4% and 80.0% for arm A and B, and 70.0% and 73.9% for arm C and D. The median overall survival (OS) was 39.3 months versus 31.2 months for arm A and B (p=0.442), and 16.3 months versus 25.3 months for arm C and D (p=0.050). Patients with sensitive <I>EGFR</I> mutations had significantly longer OS than <I>EGFR</I>-wild patients (74.8 months vs. 25.3 months, p=0.034). There were no unexpected toxicities.</P><P><B>Conclusion</B></P><P>Combined-modality treatment by molecular diagnostics is feasible in stage III NSCLC. <I>EGFR</I>-mutant patients appear to be a distinct subset with longer survival.</P>

        A Blind Calibration Scheme Exploiting Mutual Calibration Relationships for a Dense Mobile Sensor Network

        <P>In sensor-based measurement systems, the sensor calibrations are crucial to obtain the correct measured values. In this paper, we propose a new blind calibration scheme for a massive mobile sensor network. The mutual calibration relationships between colocated sensors are utilized in a densely deployed sensor network. We model the calibration relationships in a linear algebraic model, and derive an equivalent Laplacian matrix. By just solving the Laplacian linear equation, we can determine how much each sensor has to be calibrated. The proposed calibration scheme is explained as a generalized extension of the simple mean model scheme to the mobile sensor network. Theoretical analysis and simulation results show that the proposed linear algebraic model improves the accuracy of the simple mean model by nearly σ<SUB>drift</SUB><SUP>2</SUP>/σ<SUB>noise</SUB><SUP>2</SUP> times at maximum, in terms of mean square error, where usually σ<SUB>drift</SUB> ≫ σ<SUB>noise</SUB>. In addition, we show the feasibility of our scheme on human movements, by applying the scheme to the Levy-walk model in the simulation. The most important advantages of our scheme are that the calibration model is straightforward and that the solution is simply obtained with a matrix inversion.</P>

      • Remote Fault Detection Method for Time-Slot-Violated Terminal Using Periodic Probing Signal in TDM-PON

        <P>Remote faulty terminal detection method in the time-division-multiplexing passive optical networks is crucial, especially for the time-slot-violated faulty terminals. We previously demonstrated a detection method using a periodic probing signal. In this paper, we explain the method in details and analyze its performance limitation in multiple terminal cases to evaluate the validity scope of this method. Using a random process model, we show that the proposed method can detect the probing signal even though there is one 15-dB-stronger pseudorandom-binary-sequence bit-stream (PRBS)-type faulty signal using a receiver with the bandpass filter's bandwidth below 0.47 MHz (<I>Q</I> = 21.3). This result shows that the proposed method can be useful for the current 1 Gbps gigabit-capable passive optical network (G-PON) and ethernet PON (E-PON) standards.</P>
