      • 안와파열골절의 수복에서 In Split Rib Bone Graft 의 이용

        Purpose: The ideal material in orbital blowout fracture should be flexible, easy to sculpture, strong enough to support the herniating orbital soft tissue, and adequately voluminous to avoid enophthalmos and damage of orbital contents. To satisfy these mechanical and physical requirements with an autogenous material and to minimize donor site morbidity and complication, the author developed in situ split rib bone graft (ISRBG). Materials and Methods: From 1996 to 2003, fifteen patients with pure orbital blowout fracture (30±12years) underwent repair of the fracture with ISRBG. These patients were available for follow-up at least 2 months. Results: Preoperative ophthalmologic evaluation showed diplopia in 12 patients and enophthalmos in 8 patients. Three patients with combined medial and inferior wall fracture revealed persistent diplopia on extreme gaze at the last follow-up, but out of their cardinal fields of vision. Enophthalmos did not continue in any patient postoperatively. Otherwise, there were no immediate or delayed recipient complications of hemorrhage, infection, extrusion, and displacement of bone graft. There were no functional, cosmetic and infectious donor site complications. Conclusion: For 8 years, ISRBG proved itself a safe, reliable, and versatile material in repair of orbital blowout fractures. With its excellent physical and mechanical properties and minimal donor site complication, ISRBG is the best option between autogenous material with possibility of serious donor and recipient complications and alloplastic implants with little biocompatibility and greater economic burden especially in complex three dimensional defect in extensive blowout fracture cases. 연구목적: 안와파열골절의 수복에서 이상적인 물질은 유연하고 모양을 만들기 용이하며 탈출하는 안와연부조직을 지지할 수 있는 강도를 가져야 한다. 도한, 안구함몰과 안와조직손상을 모두 방지할 수 잇을 정도의 적당한 부피를 가져야 한다. 이러한 기계적이고 물리적인 조건을 만족하는 자가물질로서 공여부의 부작용을 최소화하기 위하여 저자는 in split rib bone graft (ISRBG)를 개발하였다. 대상 및 방법: 1996년에서 2003년까지 15명(30±12세)의 단순 안와파열골절 환자를 대상으로 ISRBG를 이용하여 골절을 수복하였다. 이들 환자들을 최소 2개월이상 경과를 관찰하였다. 결과: 수술전 안과적 검사상 12명에서 복시, 8명에서 안구함몰을 관찰할 수 있어다. 안와안쪽벽과 아래쪽 벽의 복합적 골절을 가진 3명의 환자에서 술후 시야의 변방에서 복시가 관찰되었으나 기본 안위에서는 이상이 없었다. 안구함몰은 술후 모두 교정되었다. 그 외 즉각적인 또는 지연성의 출혈, 감염, 골이식편의 돌출, 변위 등 수혜부 부작용은 없었다. 공여부의 기능적, 미용적 부작용 역시 관찰할 수 없었다. 결론: 8년동안에 걸쳐 ISRBG는 안전하고, 믿을 만한, 다목적의 안와파열골절 수복용 물질로 사용할 수 있었다. ISRBG의 우수한 물리적, 기계적 성질과 최소한의 공여부 부작용을 고려할 때 광범위한 안와파열골절의 복잡한 3차원적 골결손 부위에 사용할 수 있는 가장 우수한 물질 가운데 하나로 볼 수 있다.

      • 수술방법에 따른 액취증의 합병증 비교

        연구목적 : 액취증은 겨드랑이에서 암내라고 불리는 특징적인 냄새를 내는 증상으로 비교적 흔하면서 환자에게 사회적, 정신적으로 많은 괴로움을 주는 질환이다. 이러한 액취증을 치료하기 위해 과거로부 터 수술적 절제술이 많이 시술되어 왔다. 하지만 불행하게도 수술적 절제술은 여러 가지 합병증이 보고 되고 있다. 현재, 액취증을 치료하기 위해 흡입술이 도입되면서 기존 치료방법과 효과와 만족도에 대한 비교의 필요성이 대두되어 왔다. 대상 및 방법 : 2005년 7월부터 2009년 3월까지 충북대학교병원 성형외과에서 액취증 일차 수술을 한 환자는 총 24명(남자 15, 여자 9)으로 이중 수술적 절제술이 8명, 흡입술이 16명이었다. 추적은 의무기록 검토 및 전화통화를 통한 설문으로 이루어 졌으며, 시술을 받은 24명의 환자중 수술적 절제 술 8명 중 7명(87.5%), 흡입술 16명 중 10명(62.5%)과 인터뷰가 이루어 졌다. 나머지 환자들은 수 신거부, 전화번호변경 등으로 추적하지 못했다. 인터뷰가 이루어진 17명의 환자들은 최소 10개월(평균 27.3개월) 간의 추적기간을 거쳤다. 액취제거에 대한 각 시술의 효과는 매우좋음, 좋음, 보통, 나쁨, 시술에 대한 만족도는 매우만족, 만족, 보통, 불만족 이렇게 각각 4단계로 분류하였다. 결과 : 효과에 대해서 수술적 절제술을 시행한 환자군에서는 매우좋음 4명(57.1%), 좋음 3명(42.9%) 으로 모두 좋음 이상의 답변을 하였고 흡입술을 시행한 환자군에서는 매우좋음 1명(10%), 좋음 5명 (50%), 나쁨 4명(40%)으로 답변을 하였다. 만족도에 대해서 수술적 절제술을 시행한 환자군 중 5명 (71.4%)이 만족 이상을 답변하였고, 흡입술을 시행한 환자군 중 4명(40%)이 만족 이상을 답변하였다. 합병증으로 수술적 절제술의 경우 과색소침착을 동반한 경계부위 조직괴사 및 과색소침착, 상처개열, 혈 종 각 1명으로 나타났고(총 3/7, 42.9%), 흡입술에서는 어떠한 합병증도 발견되지 않았다(0%). 결론 : 수술적 절제술 환자의 효과 및 만족도가 더 높지만 합병증이 흔한 것으로 나타났으며 흡입술의 경우 합병증은 없지만 효과 및 만족도가 떨어지는 것으로 나타났다. 이에 따라 두가지 방법 어느 하나 가 다른 하나에 비해 반드시 우월하다고 볼 수는 없는 것으로 보이며 환자와 상황에 따라서 알맞은 방 법을 쓰는 것이 좋을 것으로 생각된다.

        Application of Mini-abdominoplasty after Conservative Excision of Extensive Cesarean Scar Endometriosis

        Endometriosis is defined as the presence of functioning endometrial tissue outside the uterine cavity, and the standard treatment is extensive surgical excision. Cesarean scar endometriosis is a type of cutaneous endometriosis arising on or near a Cesarean section scar. A 44-year-old woman presented with a $9{\times}6$ cm sized dark-brown, stony-hard, irregular, lower abdominal mass of four years duration. The patient had a history of two Cesarean deliveries, 14 and 16 years ago. Suspecting endometriosis, we excised the tumor conservatively rather than extensively to prevent incisional hernia considering the benign nature of the tumor and the low possibility of recurrence because the patient's age was near menopause, along with simultaneous bilateral salpingo-oophorectomy that was performed in this case. On reconstruction, mini-abdominoplasty was adopted to avoid possible wound complications and cosmetic deformities. The patient was satisfied with the cosmetic results, and neither recurrence nor functional problems occurred during the 1-year follow-up period. Plastic surgeons should keep in mind the possibility of cutaneous endometriosis in an abdominal mass of a female of reproductive age with a previous history of pelvic or intra-abdominal surgery. An optimal result from oncological, functional, and cosmetic standpoints can be achieved with conservative excision followed by mini-abdominoplasty of extensive Cesarean scar endometriosis.

        This research is an analysis of medical essay article 62 by Jang Jong-jung that is quotated in 『Donguibogam』. The opinion through out the essay is that the way of healing on the whole is eliminating the origin of illness above all. Three methods of sweating, vomiting, and purging that Jang applied are profoundly connected to this opinion. Non-medicinal treatment of emotion disorders that is quoted in 『Donguibogam』 is also closely related. The consideration of Unqi(circulating qi) as the best way of searching etiological factor, pathology, and treatment, in 『Donguibogam』 is close to the view of medical essay by Jang Jong-jung.

        Extraction of Critical Low-Level Image Features for Effective Emotion Analysis

        Sentiment analysis has received considerable critical attention in machine learning, artificial intelligence, human-computer interface, etc. In these fields, many studies have analyzed emotions using images, audio, and bio-signals as features. Among them, those that utilize image features are the most typical for emotion recognition. There are two types of image features: high-level and low-level. Low-level features are more robust than high-level in sentiment analysis. Therefore, in this paper, we investigated the critical features for effective sentiment analysis in low-level image features. For an objective performance evaluation, we utilized the International Affective Picture System dataset for training and testing. We applied the iterative Han and Cha’s feature selection/extraction algorithms and used a multilayer perceptron classification. We also carried out cross-validation by replacing features. Our evaluation items consisted of two elements: accuracy and computational time. According to our results, we were able to find the critical features in sentiment analysis and our method proved more competitive compared with existing algorithms in terms of accuracy and operation time.

        Thermo-mechanical Degradation of NiCr Thin Film Subjected to Thermal Cycling

        The thermo-mechanical degradation of NiCr films during thermal cycling was investigated in terms of microstructural and chemical changes to the surface. Thermal cycling was performed between room temperature and 200℃ in thermally evaporated NiCr films on an Mn-Ni-Co-O substrate. Isothermal exposures were also performed as a control experiment. The sheet resistance of the NiCr films increased linearly (from 35 to 52 Ω/sq.) with a temperature difference of the thermal cycling, whereas the films subjected to isothermal aging increased only slightly, from 35 to 38 Ω/sq. After 300 thermal cycles at ΔT=175℃, cracks formed and proliferated on the coating surface, and local delamination occurred at the interfaces. XPS results showed that the intensity of a metallic peak at 574.0 eV decreased and that of the Cr oxide peak at ~577 eV increased with an increase of the thermal cycles. Consequently, the degradation of the NiCr films by accelerated thermal cycling stress was attributed to a proliferation of lateral surface cracks from thermally induced tensile stress compounded by surface oxidation, causing an increase in sheet resistance.

        Electrical Degradation of Al-doped ZnO Thin Films by a Damp Heat Test

        The degradation behavior of AZO thin films against thermal and humidity stress was examined by a damp heat test. The resistivity of all films tested under various conditions increased due to the decreased carrier concentration and mobility with elapsed damp heat test time. The resistivity of the films also increased with increasing humidity, and these results indicated that the resistivity of AZO films had a stronger dependence on humidity than on temperature. Among the tested samples, the AZO films at 85℃/85% relative humidity conditions highly degraded, indicating that additional -OH bonds may deteriorate the properties of ZnO thin films. The degradation of the AZO films under the damp heat test is attributed to chemisorption and diffusion of water vapors, and the AZO reacts with the water molecules and is then transformed into insulating Zn(OH)2.

      • A Study of the Relation between Arousal and Valence in the Emotional Dimension

        In this paper, we verify the relation between elements of the dimension parameters of Russell’s emotional dimension (“A Circumplex Model”) and propose a new method of emotional expression. A Circumplex Model expresses words (happy, joy, sad, etc.) as a point on the dimension. However, previous studies have shown that there have been problems such as the possibility of mixed feelings, difference of emotion and sensibility, and difference in the representation/recognition of the method according to arousal and valence. Studies claimed that it is necessary to modify the representation method. Therefore, in this paper, we propose a new method to modify Russell``s emotional dimension model. Our idea is to verify the relationship between the elements of two axes such as arousal and valence. We believe that it is possible, by using the method proposed in this paper, to solve the inherent flaws in Russell``s model and represent the emotional data more accurately.

        Power Control for Interference Management and QoS Guarantee in Heterogeneous Networks

        <P>We consider the sum-rate optimization problem with power control for uplink transmission in a heterogeneous network (HetNet) consisting of a macrocell and multiple femtocells. The considered problem includes the HetNet's crucial constraints of both cross-tier interference protection and user QoS in terms of outage probability and average delay. We transform the original nonconvex problem into a convex problem and develop a distributed algorithm that can attain the global optimal transmit power values. This algorithm, however, has heavy network overheads, which may lead to increased energy consumption for femtocell user equipment. We propose a new practical near-optimal distributed algorithm that eliminates these network overheads. Numerical results show that the schemes have nearly identical performance.</P>
