경막외 마취를 통한 무통분만이 분만에 미치는 영향에 관한 연구

N/A Objective : This study was designed to evaluate the effect of epidural analgesia on the course of labor and delivery mode. Method : Three hundred eighteen term pregnant women with singleton fetus in vertex presentation were admitted for vaginal delivery at the Department of Obstetrics and Gynecology, medical center Chung-ju hospital from January 1, 1998 to December 31, 1998. They were divided into two groups: epidural analgesia group and non-epidural analgesia group. Epidural analgesia group was 106 women (79 primiparas and 27 multiparas). Non-epidural analgesia group was 212 women (138 primiparas and 74 multiparas). Course of labor and delivery mode were compared between the two groups. Results : 1. The duration of the first stage of labor was not significantly different between two groups (primiparas: 672±110 min vs 625±134 min, multiparas: 458±152 min vs 422±184 min), and that of the second stage of labor in the primiparas was significantly longer in epidural analgesia group than non-epidural analgesia group (62±25 min vs 42±20 min, p=0.03), but did not differ significantly in the multiparas groups (36±12 min vs 31±20 min). 2.Cesarean delivery rates were not significantly different between two groups (19.8% vs 15.1%). 3.Cesarean delivery rates due to failure to progress were not significantly different between two groups (85.7% vs 78.1%). 4.Oxytocin augmentation rates were significantly higher in epidural analgesia group than in non-epidural analgesia group (primiparas: 42.3% vs 20.1%, p=0.008, multiparas: 38.5% vs 19.7%, p=0.01). 5.The newborn birthweight, Apgar score and the incidence of meconium-stained amnionic fluid were not significantly different between two groups. 6.The complication of the epidural analgesia were back pain (10.4%), shivering (7.5%), nausea and vomiting (1.3%), hypotension (0.9%), and voiding difficulty (0.9%). Conclusion : Though epidural analgesia prolonged second stage of labor in the primiparas and increased oxytocin augmentation rates but did not increased the cesarean delivery rates. So intrapartum epidural analgesia provided safe and effective pain control without undesirable effects on labor outcomes.

본 논문에서는 태양광발전용 소프트 스위칭 부스트 컨버터를 제안하였다. 900W급 어레이 태양전지를 사용하였고, 제안된 컨버터가 최대 전력점에서 발전을 하도록 P&O방식의 MPPT 알고리즘을 적용하였다.[1] 제안된 컨버터의 스위칭 소자 시비율은 MPPT 알고리즘의 수행결과에 따라 제어되며, 그 결과 컨버터에서 태양전지 출력전압(132∼260V)을 입력받아 380Vdc로 승압한다. 소프트웨어를 활용하여 태양전지 구성과 MPPT 알고리즘 구현하였고 제안된 컨버터를 시뮬레이션 하였다.

Heterotopic pregnancy is defined as coexistence of intrauterine and ectopic pregnancy. The reported incidence of heterotopic pregnancy, which is normally rare, is particularly high (1%) after IVF treatment. It is important to have a high index of suspicion for the occurrence of heterotopic pregnancies because only 40% to 84% of cases can be diagnosed with transvaginal ultrasound at the initial presentation. Edwards syndrome occurs in 8,000 newborns and the incidence is much higher in elderly gravidas. We report a case of heterotopic pregnancy following IVF-ET resulting in 16 weeks of intrauterine pregnancy with fetal Edwards syndrome, and tubal pregnancy with a brief review of literature.
만삭 정상 임부와 전자간증 임부의 혈장 leptin 농도와 신생아 제대 혈 leptin 농도와 신생아 체중과의 연관성에 관한 연구

목적 : 임신 중 leptin은 체내 지방세포 뿐만 아니라 비 지방세포인 태반과 양막에서도 합성되어 양수와 제대 혈에서도 leptin이 검출되는 바, 제대 혈의 leptin 치는 태아의 체중과 유의한 상관관계를 보이는 것으로 알려져 있다 이에 저자들은 만삭 정상 혈압 임부 및 전자간증 임부의 혈장 및 제대 혈 leptin 치의 상호 연관성과 이들이 임부의 임신 중 체중변화, 체질량지수, 태아의 성별 및 출생체중 등에 미치는 영향을 판정하고자 하여 본 연구를 시행하였다. 연구 방법 : 본원에 분만하기 위해 내원한 임부 중 임신 36주 이후에 분만한 정상 혈압 임부 20명과 전자 간증 임부 20명을 대상으로 하였다. 임부의 혈액은 입원 당시에 채취하였으며, 제대 혈은 태아 분만 직후 태반 만출 전에 채취하였다. 채취된 혈액은 2,500 rpm으로 원심 분리하여 혈장만을 검사 전까지 -70℃에 냉동 보관하였으며 human recombinant leptin을 이용한 radioimmnunoassay 방법과 gamma counter를 사용하여 leptin 치를 측정하였으며, 임부와 아기의 체중 및 체질량지수, 임신 중 체중 증가, 태반 무게 둥을 측정하였다. 결과 : 정상 혈압 임부와 전자간증 임부의 평균 혈장 leptin 치는 각각 16.16 ng/㎖ 및 17.09ng/㎖이었으며, 각 군의 평균 제대 혈 leptin 치는 7.11 ng/㎖ 및 8.55 ng/㎖로서 전자간증 임부에서 모두 높은 경향을 보였지만 통계적인 의미는 없었다. 또한 키 군의 평균 혈장 및 제대 혈 leptin 치도 성별에 따른 차이는 없었다. 정상 혈압 임부에서 혈장 leptin 치는 분만 전 체중 (r^2=0.201. p=0.047)과 임부의 체질량지수 (r^2=0.307, p=0.011)차 상호연관성이 있었으며, 전자간증 임부에서는 임신 중 체중증가 (r^2=252, p=0.024)와 상호연관성이 있었다. 제대 혈 leptin 치는 정상 혈압 임부에서 아기의 출생체중 (r^2=0.360, p=0.005), 아기의 체질량지수 (r^=0.241, p=0.028) 및 태반 무게 (r^=0.311, p=0.011)와 상호 연관성이 있었으면, 전자간증 일부에서도 아기의 출생체중 (r^2=0.582, p=0.0001) 및 아기의 체질량지수 (r^2=0.533, p=0.0003)와 상호 연관성이 있었다. 결론 : 본 연구에서 정상 혈압 임부와 전자간증 임부 모두에서 혈장 leptin 농도와 제대 혈 leptin농도사이 에는 상호 연관성이 없었다. 그러나 혈장 leptin 치는 정상 혈압 임부 및 전자간증 임부 모두에서 분만 전 체중이나 체질량지수, 임신 중 체중증가 등과 같은 일부의 지방 양과 연관성이 있었다. 제대 혈 leptin 치는 양군에서 모두 아기의 출생체중 및 아기의 체질량지수와 연관이 있어 임부의 혈장 leptin과는 무관하게 독립적으로 태아성장을 반영하는 자료로 여겨진다. Objective : The aim of this study is to assess the relationships between maternal plasma and umbilical cord leptin concentrations and their effects on newborn birth weights, maternal body mass indices and fetal sex in term normotensive (NT) and preeclampsia (PE) women. Methods : Blood samples were obtained at delivery from 20 NT group and another 20 from PE group of at least 36 weeks of gestation. And the umbilical cord samples were also taken from their newborns at birth. Plasma leptin levels were determined in both groups using a human recombinant leptin 125-I radioimmunoassay. Results : Mean maternal plasma and umbilical cord leptin concentrations were 16.16+2.05 ng/ml and 7.11±1.01 ng/㎖ in NT group, 17.09±1.67 ng/㎖ and 8.55±6.63 ng/㎖ in PE group, and there was no statistical significances among them. The differences of leptin concentrations in maternal plasma and umbilical cord according to baby sex were not significant in both NT and PE groups. Plasma leptin concentrations were related with maternal weight gain and BMI in NT group and with body weight in PE group. The BMI and birth weights of the neonates have significant effects on the umbilical cord leptin concentrations in both NT and PE groups. Conclusion : In this study, no correlation was found between maternal plasma and umbilical cord leptin concentrations in both NT and PE groups. But maternal plasma leptin concentrations had positive correlation with maternal body weight, BMI, and body weight changes during pregnancy in both NT and PE groups. There were also positive correlation among umbilical cord leptin concentrations, BMI and birth weights of the neonates of NT and PE groups term. Therefore umbilical cord leptin is considered to be the index of fetal birth weight.
인터리브드 소프트 스위칭 부스트 컨버터의 입출력 리플 분석

In this paper, the input current and output voltage ripple of the soft switching interleaved boost converter was analyzed. Ripples of input current and output voltage with an interleaved method is analysed and as a result, the facts that it has lower ripple current than conventional interleaved method is verified. it means that a capacity of a main inductor can be reduced. Besides, a low capacitance of capacitor which means high lifetime and confidence can be used because of reducing ripples of output voltage. In order to verify the validity of the proposed converter used 10uF film capacitor, experiment was performed, and the efficiency of the proposed converter was measured with variable load and duty conditions.

In this paper, we proposed a design photovoltaic generation systems with SEPIC-Flyback converter. The SEPIC-Flyback converter is operated to SEPIC converter the state tum-on of the switch and the state tum-off the switch is operated Flyback converter. Therefore application rate of the core increases and voltage stress of switch and transformer decreases with active clamp operation. Also we performed MPPT(Maximum Power Point Tracking) control for efficient working of photovoltaic power generation system.

The conventional battery energy storage system (BESS) has many limits due to weight, size and low energy utilization resulted form battery characteristics. To solve this problem, we proposed a grid connected peak load compensation system with high discharge current characteristics based on lithium polymer battery for development of the next generation power-station. The lithium polymer battery has faster discharge current characteristics than conventional battery, so that can compensate high active power demanded by load in a short time using the low capacity battery bank. Therefore, it is possible to control power leveling of grid by measuring storage energy of battery and active power which is needed from load. The validity of proposed system was verified through the simulation and experiment.
