        한 다리 서기 시 내측 쐐기(wedge)의 적용이 하지 근활성도에 미치는 영향

        Foot posture is important in the development of the musculoskeletal structure in the lower limbs because it can change the mechanical alignment. Although foot orthotics are widely used for the correction of malalignments in the lower extremities, the biomechanical effects of wedges have not yet been cleared. The aim of this study was to investigate whether medial wedges affect the electromyographic (EMG) activity of the knee and hip joints in healthy adults that are performing one leg standing. Seventeen healthy volunteers performed the one leg standing under two foot conditions: A level surface, and a 15° medial wedge. The subjects’ EMG data for the gluteus maximus (Gmax), gluteus medius (Gmed) , tensor fasciae latae (TFL), biceps femoris (BF), vastus lateralis (VL), and vastus medialis oblique (VMO) were recorded, along with the surface EMG, and all were analyzed. The EMG activity of the Gmed and TFL had significantly decreased under the medial wedge condition during one leg standing. Further study is needed in order to investigate whether medial wedges influence the EMG activity and kinematic data of the knee and hip joints as well as the ankle joints in adults with flexible flatfoot, while they are performing one leg standing.

        뇌졸중 환자의 수동적 어깨 관절 가동범위 측정에 관한 스마트폰과 측각기의 측정자내, 측정자간 신뢰도 연구

        The purpose of this study were to determine the intra-rater and inter-rater reliability of shoulder passive range of motion measurement using the "Clinometer + bubble level", a smartphone application and to compare with the intra-rater and inter-rater reliability of measurement using a goniometer. Twenty six patients with stroke were recruited for this study. Two raters measured the passive range of motion of four types of shoulder movements (forward flexion; FF, abduction; ABD, external rotation at 90° abduction; ER90 and internal rotation at 90° abduction; IR90) using a goniometer and a smartphone to determine within-day inter-rater reliability. A retest session was performed thirty minutes later to determine within-day intra-rater reliability. The reliability was assessed using intraclass correlation coefficients (ICC) and the standard error of measurement (SEM). The ICC (2,1) for the inter-rater reliabilities of the goniometer and smartphone were good in FF and ABD [ICC (2,1)=.75∼.87] and excellent in ER90 [ICC (2,1)=.90∼.95]. The intra-rater reliabilities for the goniometer and smartphone were good or more than good, with an ICC (3,1) value >.75, the exception was IR90 measured by rater 2 on the smartphone. These results suggest that smartphone could be used as an alternative method tool for measurement of passive shoulder range of motion in patients with stroke.

        컴퓨터 단층촬영을 통한 깊은목굽힘운동 수행 시 일자목 환자와 정상인에게 나타나는 목근육의 단면적 비교

        PURPOSE: This study compared the cross-sectional areas (CSA) of the cervical muscles of straight neck patients and normal participants during a craniocervical flexion exercise (CCFE) using computerized tomography (CT) to investigate the effects of CCFE on the cervical curve. METHODS: Eighteen subjects were recruited for this study. Nine subjects were allocated to the straight neck group (subjects with pain and a cervical lordosis angle of less than 20°); the remainder formed the control group (subjects with a cervical lordosis angle greater than 20°). The CSA of the subjects’ neck flexors (longus colli, longus capitis, and sternocleidomastoid) were measured by CT during rest and CCFE in the supine position, and the range of motion (ROM) of neck flexion was measured using a C-ROM instrument in a sitting. RESULTS: The straight neck group had a significantly smaller CSA of the longus colli, longus capitis, and sternocleidomastoid than the control group (p < .05). Both the straight neck and control groups showed statistically significant increases in the CSA of the neck flexors during CCFE compared to that at rest (p < .05). In addition, the straight neck group showed a significantly smaller ROM of neck flexion than the control group (p < .05). CONCLUSION: The results of this study provide more concrete evidence for therapists by demonstrating that CCFE improves the neck function by strengthening the neck flexors and increasing the neck stability for straight neck patients. Therefore, it is necessary to perform CCFE and neck extension exercises to rehabilitate straight neck patients.

        Landslides pose significant threats to communities and infrastructure, necessitating comprehensive risk assessment for effective mitigation strategies. This abstract presents a novel approach to landslide risk assessment for South Korea, revealing three previously unconsidered factors in the traditional local landslide risk assessment: aspect, elevation, and soil type. Conventional risk assessments have overlooked and have not considered these crucial causative factors, leading to incomplete evaluations. This study uncovers correlations between landslide occurrences and the overlooked variables by integrating data mining and co-occurrence network analysis. The goal is to provide landslide risk evaluators with a more accurate and holistic understanding of landslide risk, facilitating better-informed decision-making. The research highlights the significance of integrating previously neglected factors to enhance the overall effectiveness of landslide risk assessment. The key findings underscore the importance of considering aspect, elevation, and soil type when evaluating landslide risk, enabling stakeholders to anticipate better and address landslide susceptibility in vulnerable regions. This research advances local landslide risk assessment methodologies by shedding light on three essential factors overlooked in traditional approaches. By embracing data mining and network analysis techniques, our findings empower communities and decision-makers with the knowledge needed to develop proactive strategies for mitigating landslide risks and ensuring the safety of vulnerable areas.

        휠체어에서 엉덩이 들기 동작 동안 발위치가 척수손상환자의 어깨 근활성도, 최대 족저압, 무릎굽힘 각도, 운동자각도에 미치는 효과 비교

        Background: Individuals with spinal cord injury (SCI) rely on their upper limbs for body-lifting activity (BLA). While studies have examined the electromyography (EMG) and kinematics of the shoulder joints during BLA, no studies have considered foot position during BLA. Objects: This study compared the effects of different foot positions during BLA on the shoulder muscle activities, peak plantar pressure, knee flexion angle, and rating perceived exertion in individuals with SCI. Methods: The study enrolled 13 mens with motor-complete paraplegic SCI, ASIA (American Spinal Injury Association) A or B. All subjects performed BLA with the feet positioned on the wheelchair footrest and on the floor independently. Surface EMG was used to collect data from the latissimus dorsi, pectoralis major, serratus anterior, and triceps brachii. The peak plantar pressure was measured using pedar-X and the knee flexion angle with Image J. Borg’s rating perceived exertion scale was used to measure the physical activity intensity level. The paired t-test was used to compare the shoulder muscle activities, peak plantar pressure, knee flexion angle, and rating perceived exertion between the two feet positions during BLA. Results: The activity of the latissimus dorsi, pectoralis major, serratus anterior, and triceps brachii and rating perceived exertion decreased significantly and the peak plantar pressure and knee flexion angle increased significantly when performing BLA with the feet positioned on the wheelchair footrest compared with on the floor (p<.05). Conclusion: These findings suggest that individuals with SCI may perform BLA with the feet positioned on the wheelchair footrest for weight-relief lifting to decrease the shoulder muscle activities and the rating perceived exertion and to increase the peak plantar pressure and the knee flexion angle.

        저수지 제방 보수보강을 위한 하이브리드형 그라우트재 연구

        노후저수지 제체 보강을 목적으로 시멘트 그라우팅공법이 일반적으로 적용되고 있으나, 주입재의 재료적 한계로 인해 여러 문제점이 제기되어 왔다. 이를 극복하기 위하여 국내에서도 다양한 그라우트 재료들을 개발하고 현장에 적용하고 있으나 실제 현장에서 침투주입시 원지반 교란으로 인한 문제가 야기되는 경우가 많다. 본 연구에서는 이와 같은 문제점을 개선하기 위하여 원지반에의 이상적인 침투주입이 가능하도록 고분말, 고점도의 특성을 가지며 실트질 모래층까지 침투주입을 확장시키고 복합조건의 지반에서도 적용할 수 있는 하이브리드형 그라우트재의 물리적 특성을 분석하였으며, 최적의 팽창제를 선정하여 그라우트재와 혼합한 후 부피팽창에 따른 압력과 다짐효과를 검토하여 실효성을 검증하고자 하였다. 팽창율, 일축압축강도, 팽창압력 및 다짐효과확인시험 결과, HI-E(2%) 시료는 팽창제에 의한 개선효과가 탁월한 것으로 나타나 이를 혼합한 하이브리드형 그라우트재는 노후저수지, 방조제 등의 차수 및 보강공법에 효과적으로 적용이 가능할 것으로 판단된다. Cement grouting method is generally applied for the purpose of reinforcement of deteriorated reservior. Problems have been raised due to the limit of the injection material. In order to solve these problems, various grout materials have been developed. However, there are many cases in which the grounds are disturbed in actual field. In this study, the physical properties of hybrid grout with high fineness and high viscosity characteristics were analyzed to enable penetration into the ground. Optimum inflation agent was selected and mixed with the grout. The pressure and compaction effect on expansion was examined and its effectiveness was verified. From the result of confirming expansion ratio, uniaxial compressive strength, expansion pressure and compaction effect, the HI-E (2%) sample was analyzed to be excellent in improvement effect by the inflation agent. Hence, hybrid grout can be effectively applied for the impermeable and reinforcement method of deterioration reservoir and tide embankment.

        침투다짐형 팩커를 이용한 침투다짐 그라우트의 주입 효과

        저수지 제방의 그라우팅시 가장 이상적인 주입형태는 침투주입이지만 일반적으로 침투와 할렬주입이 복합적으로 시공되어 원지반의 교란이 발생될 가능성이 존재한다. 이와 같은 문제점을 개선하기 위하여 이상적인 침투주입을 가능하게 하는 그라우트 재료를 이용하여 저압주입과 팽창제를 이용한 주입재의 팽창력으로 그라우팅 주입공 주변에 대한 다짐을 실시하고 순차적으로 그라우트재를 주입하는 방법이 유용하다. 본 연구에서는 그라우트 주입공 주변에 대한 침투다짐효과를 얻기 위하여 하이브리드형 주입재와 팽창제를 혼합한 침투다짐 그라우트를 PC(Permeable Compaction)팩커를 이용하여 주입한 그 효과에 대해 고찰하였다, 본 연구결과, PC팩커와 침투다짐 그라우트를 저수지 보수보강 그라우팅 공사에 적용할 경우 저수지 차수 그라우팅의 성능기준에 부합하는 침투다짐효과가 발휘될 수 있음을 확인하였다. Permeating injection is commonly known as an ideal type of injection in grouting reservoir embankment, yet often-combined permeating and fracturing injection grouting operation can disturb the original soil. A grouting method has been regarded as effective and developed to ameliorate the possible disturbance problem. It involves compaction grouting with low expansive pressure near the injection hole and repetitive injection and compaction with grout material that allows ideal permeating injection. This thesis develops Hybrid Grout (ie. HG grout) that allows various application in any ground condition combined together, has high fineness and low viscosity, and expands permeation injection to silty sand. It researches on the injection effect of permeable compaction grout which is done with PC packer and is a combination of HG grout and expansion agent to obtain permeation compaction effect on the area near grout injection spot by developing Permeable Compaction Type Packer(ie. PC packer). As the developed PC packer, HG grout, and and expansion agent (HI-E) are applied to reservoir embankment reinforcement grouting, possibile permeation compaction effect that satisfies reservoir embankment grouting standard is confirmed according to the research.

        진동 자극에 노출된 근로자의 작업 중 개인보호구 착용이 건강문제와 부상에 미치는 영향 : 5차 근로환경조사를 바탕으로

        Objective: The purpose of this study was to identify the effects of wearing personal protective equipment (PPE) during work on the health problems and injuries of workers exposed to vibration stimuli. Method: We used data from the fifth Korean Working Condition Survey (2017). After applying the inclusion and exclusion criteria, 3,062 workers were included. The occurrence of health problems and injuries was investigated using a self-reported questionnaire. Chi-squared tests were used to compare the differences in baseline characteristics between the groups wearing and not wearing PPE. The odds ratios and 95 % confidence intervals were estimated using a multivariate logistic regression model adjusted for several covariates. Results: As a result of multivariate logistic regression analysis, the probability of back pain was 1.379 times higher in the group not wearing PPE than in the group wearing PPE (OR =1.379, 95 % CI: 1.031 - 1.845). Additionally, the likelihood of headaches and eye fatigue was 1.651 times higher (OR = 1.651, 95 % CI: 1.228 - 2.220). Conclusion: We found that wearing PPE could reduce the risk of back pain, headaches, and eye fatigue due to exposure to vibration stimuli between tasks. These results can contribute to improvements in the working environment and the health promotion of workers as a basis for practical educational data to improve the PPE-wearing rate.

        바이오피드백을 이용한 심부목굽힘근운동이 목 질환에 미치는 영향 : 메타분석

        Previous studies have reported that deep neck flexor (DNF) exercise can improve neck problems, including neck pain, forward head posture, and headache, by targeting the deep and superficial muscles of the neck. Despite the prevailing opinion across studies, the benefits of DNF can vary according to the type of neck problems and the outcome measures adopted, ranging from positive outcomes to non-significant benefits. A meta-analysis was conducted in this study to assess conclusive evidence of the impact of DNF exercise on individuals with neck problems. We used PUBMED, MEDLINE, NDSL, EMBASE, and Web of Science to search for primary studies and the key terms used in these searches were “forward head posture (FHP),” “biofeedback,” “pressure biofeedback unit,” “stabilizer,” “headache,” and “neck pain.” Twenty-four eligible studies were included in this meta-analysis and were coded according to the type of neck problems and outcome measures described, such as pain, endurance, involvement of neck muscle, craniovertebral angle (CVA), neck disability index (NDI), cervical range of motion (CROM), radiographs of the neck, posture, strength, endurance, and headache disability index. The overall effect size of the DNF exercise was 0.489. The effect sizes of the neck problems were 0.556 (neck pain), –1.278 (FHP), 0.176 (headache), and 1.850 (mix). The effect sizes of outcome measures were 1.045 (pain), 0.966 (endurance), 0.894 (deep neck flexor), 0.608 (superficial neck flexor), 0.487 (CVA), 0.409 (NDI), and 0.252 (CROM). According to the results of this study, DNF exercise can effectively reduce neck pain. Thus, DNF exercise is highly recommend as an effective exercise method for individuals suffering from neck pain.
