Radial head resection is the accepted treatment of comminuted radial head fractures in adults, but the results are not always satisfactory. A number of well-known problems can ensue. These include chronic elbow and wrist pain, limited of motion, cubitus valgus, proximal radial migration, and new bone formation at the site of excision. Prosthetic replacement of radial head after excision offers theoretical advantages in the prevention of these problems. We are reporting three cases of patients, who had treated Swanson silastic prosthesis after comminuted radial head fracture associated posterior dislocation of the elbow.

There are many reports on the treatment of rheumatoid and degenerative arthritis of the knee joint including conservative and surgical treatments. Recently proteolytic enzymes, especially Urokinase has been used, and this procedure is called enzymatic synoviolysis. Urokinase has a nti-inflammatory and anti-edema effects, but its effects are weaker than other proteolytic enzymes. Urokinase also has powerful fibrinolytic effects are weaker than other proteolytic enzymes. Urokinase also has powerful fibrinolytic effect by activation of plasminogen to plasmin, and its effect is more powerful than other proteolytic enzymes. Rheumatoid arthritis of the knee joint has synovial hypertrophy and fibrin accumulation, so we applied Urokinase 12,000 I.U./10ml intra-articularly and aspirated after 5-hours and Depomedrol 40mg was injected intra-articularly for the purpose of anti-inflammatory and anti-edema effects. This procedure was repeated about 3 times to 7 times with regular intervals. 14 patients with rheumatoid arthritis and 2 patients with degenerative arthritis of the knee joint who were treated with many methods previously but not improved, were treated with Urokinase from March, 1984 to March, 1985 in Department of Orthopaedic Surgery, College of Medicine, Soon Chun Hyang University. The results were followed, 1. In 15 cases(94%), the pain was improved. 2. In 11 cases(69%), the circumference of the knee was decreased and 10 cases(63%), the range of motion of the knee joint was improved. 3. In 15 cases(83%), the amount of synovial fluid was decreased and its components' findings were improved to nearly normal. 4. In 5 cases(83%) of 6 cases of rheumatoid arthritis, arthroscopic findings were improved. 5. Biopsy of the synovial membrane of rheumatoid arthritis of the knee joint reveals improved inflammatory reaction. 6. Only 1 case had post-injection flare, but remain cases had no problems 7. The results of 8 cases(50%) were excellent, 6 cases(38%) were good and 2 cases(12%) were poor.

The anteversion of femoral neck is implicated in treatment and prognosis of congenital dislocation of hip, as well as cerebral palsy, Legg-Calve-Perthes disease, and intoeing gait. So, the anteversion of femoral neck is considered as one of the orthopaedic interests and measuring it has been investigated since 1878, when Milkulicz reported the degree of the femoral neck anteversion for the first time. Several techniques were developed in measuring the anteversion, using the fluoroscopy, biplane radiography, axial tomography, computed tomography, and ultrasound, and among them the ultrasound technique, which we adopted, was regarded as useful method clinically because of more accuracy, more reproducibility, no radiation hazard, and low economy, in addition to non-invasiveness and easy applicability. We measured the anteversion of femoral necks of 166 Korean children below 20 year-old ages who were chosen randomly among inpatient and outpatient without femoral neck problems, during the period from Dec. 1984 to Sep. 1985 at the Soonchunhyang University Hospital. The results were as follows; 1. The ratio of male to female was 1.4:1. 2. The degree of femoral neck anteversion was 33.53°in group of below 2 year-old and 17.9°in group of above 18 year-old, and between two groups the inclination of degree was marked around 2 years and mild clination was seen since then. 3. The difference between right and left femoral neck anteversion was below 5°in 82.5%, but the comparison was considered to be insignificant statistically. 4. The comparison between male and female anteversion was also considered to be insignificant statistically in all groups.

Traumatic dislocation of the hip is a severe injury which often produces a multiplicity of immediate and late complications and presents a major therapeutic problem. The present analysis deals with the treatment of this injury, the complications, and the end results in patients who have been observed at the Soon Chun Hyang University Hospital since March, 1978 to September, 1983. Out of 31 patients, an adequate follow-up has been possible in 20 cases. The average follow-up was 2 years. Out of 31 patients, 2 were anterior hip dislocation and the remainders were posterior hip dislocation. The results were as follows, 1. Of 31 patients, 27 patients were male and 4 were female. The prevalent age distribution was from 21 to 30 years of age-11 patients(35.4%) showing this distribution. 2. 25(80.3%) of 31 dislocations of the hip were the result of automobile accidents. 3. The order of incidence according to the Thompson-Epstein classification is: posterior type I(38.7%), II(29.3%), and type III(12.9%). 4. 20(64.5%) dislocations of the hip were complicated by multiple injuries at other sites. Of associated injuries, ipsilateral knee joint injury was the most common, which occurred in 9 patients. 5. 31 patients were reduced by closed reduction. Within 10 days after closed reduction, fragments of acetabulum were reduced and fixed with screw in 4 patients, and prosthetic replacement of femoral head and acetabulum was performed in 1 patient. 6. The results were rated according to the criteria of the Thompson and Epstein. Satisfactory results were rated according to the criteria of the Thompson and Epstein. Satisfactory results were obtained in 15(75%) patients and those of 12 patients were reduced within 12 hours after injury. 7. As the complications, avascular necrosis, myositis ossificans and osteoarthritis were developed in one patient, one patient, and five patients, respectively.

It has been controversial about several techniques and studies of the operating method for unstable compression fracture of the spine, spondylolysis and spondylolisthesis. We have treated these patients by conservative treatment and operation from past. Recently, we have experienced several posterior-spinal fusion with wire-fixation method and there are some advantages. first, relatively easy technical procedure. second, shorten shouter operation time than other methods. third, easy postoperative care. We have carried out the posterior-fusion with wire-fixation method on 7 cases from August, 1982 to October, 1982. The result were: 1. The age distribution was between 30 & 50. (males 6 cases, female, case) 2. 3 patients were sponylolisthesis, 2 were compression fracture of L-spine combined with fracture of vertebral arch, 1 was compression fracture of Ll-spine combined with spinal stenosis, 1 was unstable at L2 level. 3. After operation, patient's complaints were almost resolved except 1 case.
