<P>This study evaluated the efficacy of periodontal tissue regeneration using guided tissue regeneration (GTR) with a mixture of tooth ash and plaster as a bone substitute material in the treatment of class II furcation defects in dogs. GTR was performed at the site of a surgically formed mandibular premolar bifurcation area in dogs. Four adult dogs were used in the animal study. The animals were in a good periodontal state without any systemic disease. A resorbable membrane (Bio-Gide®, Swiss) and bone graft material using a mixture of tooth ash and plaster were used to examine the biological activity. The defect in the control group was filled a Bio-Gide® only. For the experimental group, the defects were filled with a mixture of tooth ash and plaster and covered by a Bio-Gide®. Oral rinsing with 0.12% chlorhexidine was performed twice a day for 2 weeks. In control group, new bone formation was observed adjacent to the preexisting bone. Soft tissue ingrowth was also noted. In experimental group, new lamellar type trabecular bone formation as well as centum and PDL regeneration was observed in the base of the infrabony pocket. More bone regeneration would be expected when GTR is performed using a mixture of human tooth-ash and plaster of Paris as a bone graft material.</P>
외측 반월상 연골의 방사형 완전 파열 시 섬유성 응괴를 이용한 봉합술

흔하게 발생하는 슬관절 손상 중의 하나인 반월상 연골 파열은 기능적 중요성이 강조됨에 따라 점차 기능을 보존하는 방향으로 다양한 치료방법이 연구되고 있다. 방사형 완전 파열은 반월상 연골의 원주 테 장력을 소실시키기 때문에 봉합하여 기능을 보존하는 것이 중요하지만 손상부위의 구조적 특성상 치유가 쉽지 않은 것으로 알려져 있다. 저자들은 외측 반월상 연골의 방사형 완전 파열 손상을 입은 5예의 환자에 대해 섬유성 응괴를 이용한 봉합술을 시행하여 전례에서 양호한 반월상 연골의 치유와 함께 좋은 임상 결과를 얻었기에 보고하는 바이다. Meniscus tear is one of the most common knee injuries. The increasing recognition of the importance of the meniscal function has led to a range of new treatments for meniscal tears. A complete radial tear disrupts the circumferential hoop tension of the meniscus. Therefore, restoring its natural architecture is of utmost importance. However, it is difficult to achieve meniscal healing of a complete radial tear due to its structural characteristics. We report five cases of a complete radial tear of the lateral meniscus treated successfully using a fibrin clot that showed good meniscal healing and excellent clinical outcomes.
해상 시추선의 Hoisting System 동적 거동 해석을 위한 다물체계 동역학 kernel 개발

본 논문에서는 선박 및 해양구조물의 설계 단계 필요로 하는 동역학 시뮬레이션을 위하여, 다물체계 동역학 kernel 을 개발하였다. 다물체계 동역학 kernel 은 전처리기(pre-processor)와 동역학 시뮬레이터(dynamic simulator)로 구성하였다. 그 중 전처리기는 물체의 형상정보, 속성정보, 전역 좌표계(global coordinate system)를 기준으로 한 국부 좌표계(local coordinate system), 그리고 단일 물체간의 연결 관계를 통해 시뮬레이션 모델의 운동학적(kinematic)정보를 입력 받는 기능을 한다. 그리고 동역학 시뮬레이터는 전처리기에서 입력 받은 시뮬레이션 모델의 운동학적 정보를 이용하여 recursive Newton-Euler formulation 방법을 기반으로 운동방정식을 자동으로 구성하고, 수치적으로 해를 계산하는 기능을 한다. 이를 토대로 개발한 다물체계 동역학 kernel 의 기능을 상용 다물체계 동역학 엔진과 비교하여 검증하였으며, 이를 적용하여 해상 시추선의 hoisting system 동적 거동 해석을 수행하고, 관절에서의 구속력을 계산하였다.

Background: In cases with lymphovascular invasion or positive vertical margins after endoscopic submucosal dissection(ESD) for early gastric cancer(EGC), additional radical gastrectomy is performed in principle. However, an additional surgery is diffi cult to consider if the surgical approach itself is challenging or the patient refuses surgery. In such cases, only close surveillance is performed, without additional surgical procedures. This study aimed to examine EGC cases with lymphovascular invasion or positive vertical margins after ESD. Methods: We retrospectively studied 83 patients with lymphovascular invasion or positive vertical margins after ESD from July 2005 to November 2013. Results: Of the 83 patients, 45 (54.2%) underwent radical additional gastrectomy (surgical group) and 38 (45.8%) were under close surveillance without surgical or endoscopic treatments (close surveillance group.) Cancer-free survival period was 78.3 ± 3.4 months in the surgical group and 64.5 ± 4.6 months in the close surveillance group. The recurrence rates did not signifi cantly differ between the 2 groups, at 7.9% in the surgical group and 6.7% in the non-surgical group. Conclusions: Close surveillance may be suggested as an option for EGC patients for whom surgical approach is diffi cult, who exhibit a positive vertical margin after ESD, and who have no lymphovascular or deep submucosal invasion after ESD.
