S-307 : A case of pulmonary artery sarcoma presented as cavitary pulmonary lesions

Pulmonary artery sarcoma (PAS) is a rare, poorly differentiated malignancy arising from the intimal layer of the pulmonary artery. We report a case of PAS that was diagnosed during the evaluation of cavitary pulmonary lesions. An 18-year-old man with no past medical history visited a primary clinic for cough, blood tinged sputum and right chest wall pain 3 months ago. The initial chest radiography showed patchy consolidations in the right upper lobe and non-enhanced computed tomography (CT) of the cheat revealed cavitary consolidation and multifocal nodular consolidations in the right lung. He was treated with empirical antibiotics and non-enhanced chest CT scan was followed. Despite of the medical treatment, his symptoms were not improved and the cavitray pulmonary lesions did not disappear. So, he referred to our hospital. Initially, we suspected the possibilities of various conditions including bacterial or fungal infection, tuberculosis, autoimmune disease, vasculitis, and malignancy. Percutaneous needle aspiration of the cavitary consolidation was performed and the pathology revealed necrotic tissue and fibrosis. Contrast-enhanced CT of the cheat was performed and it showed filling defect within the lumen of the right interlobar pulmonary artery, raising the possibilities of pulmonary thromboembolism or PAS. Positron emission tomography (PET)-CT with 18F-fluorodeoxyglucose (FDG) showed increased FDG uptake along the right interlobar pulmonary artery, suggesting a malignant lesion. Open thoracotomy and excisional biopsy were performed. Frozen biopsy specimens obtained from the right interlobar pulmonary artery revealed a PAS. In succession, the patient underwent a curative right pneumonectomy. Although PAS is very rare and its parenchymal lesion is mostly nonspecific, the suspicion of PAS in a patient with cavitary pulmonary lesion is needed. So, we report the case and reviewed the literatures.

Contrast-enhanced chest computed tomography (CT) is a good diagnostic modality that shows a low-attenuation filling defect of the pulmonary artery in PAS patients. An 18-year-old man was referred to our hospital for the evaluation and management of cavitary pulmonary lesions that did not respond to treatment. A contrast-enhanced CT of the chest was performed, which showed a filling defect within the right interlobar pulmonary artery. The patient underwent a curative right pneumonectomy after confirmation of PAS. Although lung parenchymal lesions of PAS are generally nonspecific, it can be presented as cavities indicate pulmonary infarcts. Clinicians must consider the possibility of PAS as well as pulmonary thromboembolism in patients with pulmonary infarcts. So, we report the case with PAS that was diagnosed during the evaluation of cavitary pulmonary lesions and reviewed the literatures.

Pheochromocytoma (PCC) is a rare catecholamine-producing tumor with the incidence in hypertension of 0.1-0.6%. PCC crisis is an endocrine emergency that can lead to hemodynamic disturbance and organ failure such as catecholamine-induced cardiomyopathy. The circulatory collapse caused by it often requires mechanical support. The author reports an unusual case in which a patient who previously underwent surgery for malignant PCC developed catecholamine-induced cardiomyopathy, and successfully recovered using extracorporeal membrane oxygenation.

CMOS 아날로그 전류모드 곱셈기의 선형성과 동적범위 향상을 위한 회로설계 기법에 관한 연구

이 논문에서는 아날로그 전류모드 인공지능 프로세서에서 핵심 디바이스 중에 하나인 아날로그 전류 모드곱셈기 회로의 선형성과 동적범위 향상을 위한 설계 기법을 소개한다. 제안하는 회로는 4 quadrant Translinear loop를 NMOS 트랜지스터만으로 구성하여, 트랜지스터의 물리적 Mismatch를 최소화하는 설계로 0.35𝜇𝑚 CMOS 공정에서 117𝜇𝑚 x 109𝜇𝑚로 구현가능하였으며, 최대 전고조파왜율 0.3% 의 선형성을 확보할 수 있었다. 제안한 아날로그 전류모드 곱셈기는 전류모드 인공지능 프로세서의 핵심 회로로 유용할 것으로 기대된다. In this paper, we present a design method for improving the linearity and dynamic range of the analog current mode multiplier circuit, which is one of the key devices in an analog current mode AI processor. The proposed circuit consists of 4 quadrant translinear loops made up of NMOS transistors only, which minimizes physical mismatches of the transistors. The proposed circuit can be implemented at 117𝜇𝑚 x 109𝜇𝑚 in 0.35𝜇𝑚 CMOS process and has a total harmonic distortion of 0.3%. The proposed analog current mode multiplier is expected to be useful as the core circuit of a current mode AI processor.

Chronic lymphedema is caused by an impairment of the lymphatic system due to primary or secondary causes. Vascularized lymph node transplantation (VLNT) is currently the most promising and frequently used technique besides lymphaticovenous anastomosis. However, the vessel anatomy in the lateral thoracic region is sometimes quite variable. Based on our experiences with vascular anatomical inconstancy in the lateral thoracic region, we planned a lateral intercostal artery perforator flap for VLNT in a female patient with chronic stage II lymphedema of both legs after cervical cancer treatment. After surgery, the patient reported significant improvement in limb volume and the accompanying symptoms. The limb circumference was reduced by an average of 19.2% at 6 months postoperatively. Despite having a short pedicle and small vessel caliber, the lateral intercostal artery perforator flap can safely be used for VLNT in lymphedema patients with anatomical variants.

DNA Microarray Analysis of Immediate Response to EGF Treatment in Rat Schwannoma Cells

Epidermal growth factor (EGF) activates many intracellular effector molecules, which subsequently influence the expression levels of many genes involved in cell growth, apoptosis and signal transduction, etc. In this study, the early response of gene expressions due to EGF treatment was monitored using oligonucleotide DNA microarrays in rat schwannoma cell lines. An immunoblotting experiment showed the successful activation of EGF receptors and an effector protein, STAT5, due to EGF treatment. The microarray study showed that 35 genes were significantly induced and 2 were repressed within 60 min after the treatment. The list of induced genes included early growth response 1, suppressor of cytokine signaling 3, c-fos, interferon regulatory factor 1 and early growth response 2, etc. According to the microarray data, six of these were induced by more than 10-fold, and showed at least two different induction patterns, indicating complicated regulatory mechanisms in the EGF signal transduction.

Protein kinase CKII (CKII) is a heterotetramer composed of two catalytic (α or α') and two regulatory (β) subunits. Using the yeast two-hybrid system, we have identified the highly basic, ribosomal protein L41 as a cellular protein capable of interacting with the β subunit of CKII. We show, furthermore, using purified proteins, that L41 protein and CKIIβ associate directly in vitro. L41 protein is not a substrate for CKII phosphorylation, and it does not stimulate CKII activity with either β-casein or synthetic peptide substrate (RRREEETEEE). However, L41 protein stimulates the phosphorylation of DNA topoisomerase IIα by CKII by 2.5 times. Additionally, L41 protein enhances the autophosphorylation of CKIIα. The data indicate that L41 protein associates with CKII and can modulate its activity toward a specific substrate or substrates. The direct interaction of CKIIβ with ribosomal proteins also suggests that CKIIβ itself or CKII holoenzyme may be involved in ribosome assembly or translational control.
