        A new method of limb sparing by resection, extracorporeal irradiation and reimplantation has several theoretical advantages. This method preserves the mobility of a joint and avoids the problem of loosening or breakage of tumor prosthesis. This study involved using extracorporeal irradiated autogenous joint transplantation for reconstruction after en bloc resection, and observed the periods of functional union and histological changes in irradiated tissue of the knee joint. This study also aimed to clarify whether the degeneration of articular cartilage is induced in rabbits by a single 50Gy dose of irradiation at the knee joint. Twenty New Zealand rabbits about three kilograms were randomized into two groups of 10 rabbits each. In group 1, as control, we resected the knee joint followed by reimplantation without irradiation. Group 2 received extracorporeal irradiation on the resected knee joint followed by reimplantation. Following are the results of these observations. The osteotomy site showed external callus formation in the roentgenographic finding eight weeks after reimplantation. There was marked degenerative changes in the collagen fiber of the irradiated anterior cruciate ligament and meniscus during the fourth week, but new blood-vessel formation was observed in the vicinity. There was degenerative changes in the collagen fiber of articular cartilage treated extracorporeal irradiation at four and eight weeks in the scanning electron micrographic findings. These findings was in contrast to those of subchondral bone which showed decreased cellularity and empty lacuna at four and eight weeks. Autoradiography demonstrated active [$^3H$]uridine incorporation by irradiated chondrocyte at eight weeks after reimplantation. These results indicate that when destruction of the articular cartilage and soft tissue of the knee joint is not severe, extracorporeal irradiation and reimplantation can be used with several advantage in maintaining movement of the joint while avoiding problems of tumor prosthesis and rejection, and therefore extracorporeal irradiated autogenous joint transplantation can be used as a limb-sparing procedure for temporary biological spacer in the childhood bone tumor around the knee.

        일반 성인들의 보행속도에 따른 신체 관절 변화 차이 비교

        The purpose of this study is to analyze and compare differences in joint changes associated with the walking speed of typical adults. Additionally, it seeks to collect essential data for the development of customized exercise programs. To accomplish this, the research enlisted 27 adult participants residing in S City, Jeollanam-do, and conducted an analysis of how joint changes differed depending on their walking speed. A one-way analysis of variance(ANOVA) was performed to assess the differences in joint variables based on walking speed. When statistically significant differences emerged among the groups, further investigations were carried out to identify which groups exhibited distinctions, using Bonferroni post-hoc tests. A significance level of α=0.05 was applied to all statistical tests. Ultimately, the following conclusions were drawn. While there were no changes in cervical posture associated with walking speed, significant posture changes were observed in the trunk as walking speed changed. In terms of hip changes related to walking speed, the extension/flexion angle of the right anterior hip was significantly higher during medium speed, compared to normal and high speeds. Conversely, the rotation angle of the left internal hip exhibited significant increases when walking speed exceeded the normal pace. The analysis findings regarding pelvic-related variables indicated that, when compared to the normal speed, the right rotation angle was significantly reduced at the medium speed. Furthermore, the angles for left pelvic elevation and depression factors were significantly larger at the normal speed in contrast to the high speed. Regarding the factors related to the knee joint, it was observed that the right extension and flexion exhibited significantly larger angles at the higher speed compared to the normal speed. Conversely, the flexion and extension factors of the left knee joint showed significantly larger angles at the medium speed compared to the normal speed. In conclusion, it is essential to further investigate these biomechanical strategies across diverse demographic groups and contribute to the development of customized intervention methodologies. These methodologies, in turn, will provide optimized therapeutic and rehabilitation strategies for individual patients or specific cohorts with gait abnormalities, resulting in improvements in walking functionality and overall functional optimization.

        턱관절장애와 관련된 교합변화의 치료

        Temporomandibular joint disorder(TMJD) was mainly characterized with joint pain, motion limitation, joint sound, resulted from pathologic conditions in temporomandibular joint and around tissue. As temporomandibular joint is one of decisive factors determining the occlusion, disorders in temporomandibular joint may cause the occlusal changes. The causes of occlusal changes related with TMJD can be classified into 2 categories; (1) those related to progression of disorder, 2) those related to treatment of the disorder. The clinical manifestation of occlusal changes depend on their causes and affected site. Therefore, whenever possible, treatment should be directed to the relief of the underlying causes. However, it is not always possible to relieve the underlying conditions. Moreover, some occlusal changes may remain irreversible even after the considerable improvement in clinical symptoms. Regarding the treatment of the permanent occlusal changes, it has been reported that the extensive occlusal treatment including occlusal adjustment, prosthodontic treatment, orthodontic treatment should be applied. Here, we present with a case report of occlusal change caused by the progressive temporomandibular joint disorder, together with introducing the intermaxillary traction appliance as the possible treatment option.

      • 제3궤조 전차선의 신축장치(Expansion Joint)작용에 관한 고찰

        제3궤조 전차선 급전레일은 주위온도와 전류로 인하여 발생하는 열 때문에 길이 방향으로 신축변화하며, 그로 인한 각각의 급전레일들 사이에 기계적, 전기적 문제가 발생하지 않도록 하기 위하여 길이 변화를 수용할 수 있는 신축장치(Expansion Joint)를 사용한다.최근 신설되는 경량전철은 제3궤조 전차선 급전방식을 주로 적용하고 있으나, 급전레일온도에 따른 관리 및 유지보수기준이 명확하게 제시되지 않고 있다. 또한, 영업운전으로 인한 급전레일 급전중에는 신축장치의 상태변화를 확인할 수 있는 시스템이 구축되어 있지 않으므로, 검측을 통하여 측정된 데이터를 분석하여 최악의 환경에서도 장비성능을 유지하며 운용되는지 검증할 필요가 있다. 본 논문에서는 제3궤조 전차선의 운용경험을 토대로 급전레일의 온도변화에 따른 신축변화를 조사·분석하였다. 그 일련의 과정들을 통해 신축장치의 신축량 조사·분석 방법과 분석결과에 따른 신축장치의 유지보수기준 적용 방법을 제시하고자 한다. The third rail of conductor rail occur the rail expansion and contraction to length direction by surrounding temperature and electric current produced heat. In order to avoid both mechanical and electronic problem between each of the conductor rail, expansion joint normally is used to accommodate the change of the length. Lately, the newly created light rail transit is mainly applied the third rail of conductor rail with method of electric supply, the standards which include management and maintenance according to temperature change of conductor rail is not presented clearly yet. Besides, during the supply of power that conductor rail by business driving, the system which Inspect variable of expansion joint is not constructed. Therefore, it should be examining status of expansion joint even in the worst of circumstances maintaining and working capabilities instruments through analyzing measurements data. In this thesis examined and analyzed the rail expansion and contraction based on the management experience about the third rail of conductor rail. We kindly suggest the method of investigation and analysis that amount of expansion about expansion joint. And also, the standard application method of administration and maintenance of expansion joint in accordance with analytical researches through a series of those process as well.

      • Control of ankle joint motion change induced by vibration stimulation on the Gastrocnemius muscle during continuous joint motion

        It has been shown that changes in movement caused by reflex phenomena and kinesthetic illusion can be induced by giving vibratory stimulation on specific muscles during limb movements. By using this phenomenon, there is a possibility that the lower-limb perception-assist, in which inappropriate motion is automatically modified, is realized. In this study, change of ankle joint motion by giving the vibration stimulation on the gastrocnemius muscle in the lower-limb is studied by performing the experiment of adding vibration stimulation to the gastrocnemius muscle during the movement of ankle joint dorsiflexion and plantar flexion under several frequency ranges. The results indicate that the amount of motion change varies depending on the frequency of the vibration stimulation. Based on the characteristics of motion change with respect to vibration stimulation, a method to control the change of the user"s ankle joint dorsiflexion motion by adjusting the frequency of the vibration stimulation to achieve the target motion, which is different from the user"s intended motion, is evaluated. It is confirmed that there is a possibility that the vibration stimulation can be applicable to modify the human ankle joint motion for the perception-assist using frequency change.

        기후변화협약체제의 의무이행 문제: 주요쟁점과 향후 전망

        The 1997 Kyoto Protocol to the United Nations Framework Convention on Climate Change is the first document that establishes legally binding limits on emissions of carbon dioxide and other greenhouse gases for industrialized countries. The Kyoto Protocol incorporates three distinct mechanisms of cooperation, facilitating implementation of the parties’ obligations of trans-boundary emissions reduction: a target-based emissions trading system(Emission Trading), a project-based transfers of credits among Annex I countries(Joint Implementation) and, finally, a project-based transfers of credits between Annex I countries and non-Annex I countries(Clean Development Mechanism). The future of the Kyoto Protocol hinges largely on how these market mechanisms are operated to realize the full potentials for cost-effective emissions reduction. Another issue related to the long-term sustainability of the Protocol regime is obtaining commitments from developing countries as it is widely acknowledged that the objective of eliminating the threat of global warming cannot be achieved through reducing industrialized country emissions alone. Thus, the closer involvement of developing countries in efforts to combat climate change is inevitable. 1997년의 교토의정서는 선진국에 구속력 있는 온실가스 감축의무를 부과함으로써 기후변화에 대처하는 국제적 노력에 중대한 전기를 마련하였다. 그러나 교토의정서가 당초의 목표를 달성하기 위해서는 당사국들이 의정서 상의 의무를 충실히 이행할 것을 전제로 한다. 교토의정서는 부속서 I 국가의 의무이행에 신축성을 부여하는 메커니즘(교토 메커니즘)을 도입, 의무이행의 실효성을 확보하고자 하였다. 이런 점에서 배출권 거래, 공동이행, 청정개발체제의 효율적 운영 여부는 향후 교토의정서의 장래와 직결되는 문제라고 할 수 있다. 교토의정서 채택 이후 기후변화협약 당사국 회의는 교토 메커니즘의 투명성, 형평성 및 경제적 효율성을 확보하기위한 세부운용 방침에 합의함으로써 의정서의 조속한 발효를 위한 기반을 마련하였다. 기후변화협약 체제의 장래와 직결되는 다른 쟁점은 개도국 참여문제다. 기후변화협약은 공통의 차별적 책임의 원칙에 따라 온실가스 감축 의무를 일단 선진국에만 부과하였으나 동 체제가 장기적으로 지탱하기 위해서는 개도국의 참여는 불가피할 것이다. 문제는 기후변화에 효율적으로 대처하면서도 형평성과 공통의 차별적인 책임의 원칙을 충족시키는 현실적인 의무부담 방식에 대한 합의를 도출하는 일이다.

        Amid the ongoing pandemic resulted from COVID-19, it is getting more and more important to form a healthy city throughout the urban environment socially as well as physically. Starting with the joint policy of ‘creating an active living environment’ in 2018, the Korea Healthy City Partnership adopted ‘Coping with Climate Change’ as a new joint policy in 2021 in order to respond jointly to cross-regional problems that threaten the future of healthy cities and the lives of citizens. Joining in the Korea Healthy City Partnership in 2009, Siheung City has been promoting the policy to create a healthy city and promote health equity. To cope with the new joint policy on climate change, the city also has been enforcing the policy on the following areas: first, to create an environmentally friendly urban environment; second, to establish a virtuous cycle of play; and third, to raise awareness.

        1990년대의 한일관계와 한일공동선언 ─ 한일관계의 구조변동에 의한 탈냉전기 협력과 제도화 시도 ─

        The purpose of this study is to analyze the Korea-Japan relationship in 1990’s based with respect to structural changes (such as post-Cold War), political, and economic development of Korea to promote incentive for cooperation between the two nations. The earlier studies tended to focus on historical problem and regarded structural changes as the critical factor of historical friction. However, structural changes created a common perception that the two nations shared democratic values and are economically equal partner. Furthermore, the new situation after the Cold War brought Korea and Japan closer to each other and they even cooperated each other in North Korea problem and Asian Pacific regionalism. Through the initiative of Roh Tae-Woo and Miyazawa administrations, Korea and Japan set the vision that both the nations have the same domestic system and widen the extent of cooperation beyond Korean Peninsula. Moreover, Kim Young-Sam administration proposed a concrete cooperation agenda which he inherited from the predecessors. In other words, the above trend was observed in the prehistoy of Korea-Japan Declaration addressed by Kim Dae Jung and Keizo Obuchi. From this perspective, the significance of Korea-Japan Declaration was to institutionalize Korea-Japan Cooperation, that had been promoted by structural change. 본 논문의 목적은 탈냉전, 한국의 경제성장 및 민주화가 한일협력의 기회구조였다는 관점에서 1990년대의 한일관계를 재조명하는 것이다. 즉, 냉전 체제의 붕괴와 한국의 정치경제적 성장이라는 구조변동은 과거사 문제라는 갈등 요인을 가져왔지만, 한편으로는 새로운 시대에 걸맞은 협력 관계의 구축을 촉진하는 측면을 가지고 있었다는 것이다. 이에 본고는 1990년대의 한일관계는 과거사 문제라는 갈등 요인을 가지면서도 한국의 경제성장과 민주화를 바탕으로 양국이 민주주의라는 가치를 공유하고 서로 대등한 파트너라는 인식을 갖게 되었다는 점을 밝힐 것이다. 그리고 탈냉전의 국제정세 속에 한반도, 아시아태평양 지역 차원의 협력 가능성을 모색하면서 과거사 문제를 극복하고자 했다는 점을 강조할 것이다. 노태우 정권의 ‘한일 우호협력 3원칙’, 미야자와 총리의 ‘아시아 속의, 세계 속의 한일관계’를 통해 한일 양국은 서로 체제를 공유하고 있고, 한반도를 넘어 지역 차원의 협력을 추진한다는 방향성을 확인하였다. 이를 바탕으로 김영삼 정권에서는 구체적인 협력 의제를 논하는 수준으로 양국 관계가 나아간다. 이는 곧 1990년대 전반에 걸쳐 한일공동선언의 전사(前史)로 평할 수 있는 정부 간 협력 움직임이라 할 수 있다. 김대중, 오부치의 한일공동선언은 상술한 1990년대의 흐름 속에 역사화해, 북한문제, 지역협력 및 인간의 안전보장 문제에서 협력의 틀을 제도화하고자 하는 시도였다.
