Evolution of Asthma Concept and Effect of Current Asthma Management Guidelines

Concept of asthma has changed from symptom-complex or airway hypersensitivity to airway inflammation and airway remodeling. Based on this concept asthma management guidelines (JGL) has been developed in Japan. Death from asthma has decreased drastically since the publication of the guidelines, although it is still high in elderly population. Further works are expected for “zero-death” from asthma and for tighter control of airway inflammation and resultant airway remodeling.
소아 기관지 천식 환자에 있어서 객담내 Creola body에 관한 연구

The Creola Body(CrB), a hallmark of asthma, is a clump of desquamated bronchial epithelial cells. It occurs in sputum of bronchial asthma patient especially during asthmatic attack. To determine the clinical significance of Creola body in childhood bronchial asthma, We analyz ed 45 cases of hospitalized bronchial asthma patient sputa and 50 cases of bronchial asthma patient who visited out-patient clinic due to mild asthma attack, and measured pulmonary function respectively. The result were as follows .' 1. There was significant relationship between eosinophil score in sputum and CrB detection rate in sputum 2. The Creola body was detected in 29 cases of in-patient(64.6%) and 10 cases of out-patient group(20%) (P<0.001) 3. There was significantly low level of %FEV, and %V50 at the first admission day of CrB(+) g IOUp. 4. In in-patient, there was significant difference of Downes severity score between CrB(+) group and CrB( ) group at the first day of admission. 5. In in-patient, ther was also significant difference of remission duration of bronchial asthma attack between the two groups. 6. The re was significant relationship between CrB score and serum total IgE in mild to moderate severity cases. We conclued that creola body in sputum was essential for pediatric bronchial asthma.
