        It is the ultimate purpose of school health management to protect, maintain, and prormote the students' health and to help for them to form a wholesome habit of health. To do this, school nurse is an actual professional personnel of school health management in our country. therefore, role performance of a school nurse is directly related to the quality of school health planning, we think. With this belief, this study intends to revise, supplement Rustia model for school health promotion and develop in into a suitable model for our actual situation. In so doing, based on Rustia model, we had surveyed the actual situation of a school nurse's role performance by making 77 school nurses in Boys' and Girls' Middle and High Schools in Seoul as the scope of this research for a period of two months beginning on September 1, 1984. The results were analyzed by employing percentage and mean value. The results of the investigation are as follows: 1. General Characteristics of the school nurses and schools. (1) The average age of the surveyed school nurses is 31.64± 6.6 years old. 81.6% of them was proved to have acquired academic background above Colleges and Graduate Schools; 74% were married; 61% were religious persons. And the average on-duty term was 6.42±5.03 years. 66.2% of them employed in the hospitals before this present job and 31.2% have worked at school as school nurses immediately after their graduation. In college, most of them (81.8%) went through a course of study for the teaching profession and only 9.1% of the taught other courses of study. (2) The majority of the surveyed schools (51.9%) were public schools and 62.3% of them were middle Schools. The average number of students in the school was 2628.52;that of school pesonnel was 66.17±9.7;that of classes was 39.68±12.0. 2. Role performance for primary prevention. (1) Among the serveyed schools, 83.1% of them performed epidemiological analysis of health problem for the promotion of health. As for health planning among health management plan, all of them put it in force, however, only 35% of them participated in preparation of the budget to achieve it. As to the evaluation program, 64.9% of them carry out it into operation and in case of being carried out once in a year marked the most frequency of enforcement as 32.4%. On guidance and support related to health planning, 42.9% of them held a staff meeting; of which 53.2% were school nurse-teacher meeting; 28.6%, school nurse-parent meeting. (2) To protect the students and school personnel from danger and injury, 67.5% of them participated in alleviating health problems-that is, the maintenance of health standard and safety within school, and inserting the means of settling those problems into curriculum and management of school events. Especially, only 14.3% administered school lighting and 35.1% observed safety standards of desks and chairs in class. Consequently, the result of this survey revealed that there is much to be desired in the administration for the maintenance of health standard and safety. As for pervention of infectious disease, the following were implemented; a) disinfection of toilets (92.2%); b) control of drinking water (75.5%); c) purification of sewage and garbage incineration (74%). What is pecular is that 71.4% of them have controlled vaccination but enforcement rate shows lack of enthusiasm when viewed from the kinds of vaccination: Typhoid fever (51.9%), cholera (41.6%) hepatitis (9.0%), encephalitis (23.4%) and that one or two sorts of vaccination were treated. As for the technical improvement of personnels who handled health problems, all of them put it in practice, and for prevention program and use of health resources, 87.5% participated in it. 3. Role performance for secondary prevention (1) 92.2% of them carried out screening tests of a physical assessment for an early detect on health problem and 24.7% did a screening tests which made a part of students its object after the first screening tests. A selection and check-up of measurement tool during a developmental evaluation was taken effect by 98.7% and a preliminary education on the persons who administered that examination and determine to select a place for it by 100%. Of discrimination and screening tests, various kinds of examination for finding disease was detected effectively by 98.7% and of this operation nothing could be come into force more than a check-up of parasites and urine with 97.4%. As to the methods explaning danger of health and safety, the case to be offered school personnels information in a staff meeting marked 96.1% and the one to offer students information in the case of generating such danger as infectious diseases was 100% (2) Control of health problem to be required regular treatment for taking a proper measure on that problem at an earlier time was carried into effect by 93.5%. 98.7% of them imposed a role of communication and intervention of offering and observation method for finding health problem and encourging this method to teacher in charge. All of them executed emergency care, School-nurses helped 33 persons a day on the average, and 89.6% of them did for students to get through a serious crisis. (3) In order to prevent complication and an disable, 83.1% of them carried out a continuous follow-up care of the students who were discharged from the hospitals. 4. Role performance for tertiary prevention (1) In order to care and treat the students who needed their rehabilitation and adaptation for activities of daily living, 89.6% of them performed reeducation after reevaluate their health status, 94.8% performed a role on motivation, 85.7% put in effect a continuous control of the students who needed rehabilitation and correction, and 15.6% did counselling for taking a measure to meet the situation of the disabled. 5. Role performance for health education. Unifying health education from the view points of the first, secondary and tertiary prevention against diseases, we surveyed the content, method, objectives of health education which is now in effect and should be emphasized in the future. Upon this study, we came to conclude as the following; as to the content of health education which is now is effect, prevention of infectious diseases has an absolute majority of all contents as 98.7%, and prevention against parasites, personal hygiene and oral hygiene second place. as 97.4% But education on prevention of venereal diseases shows inactiveness as 42.9%. On the contents to be emphasized, mental health (42.9%), sex education (40.3%) were enumerated but nobody stated about prevention of infectious diseases. As a method practising health education, all of them used that of counselling, and as an indirect education utilizing media, it comes printed matters (96.1%), poster (83.1%), interclass broadcasting casting (75.35) in that order. And the case of regular class on health education marked 23.4%, of the object of health education, 98.7% made students its object; 87.0% school personnels; 51.9% parents of students.

        경구용 속방성 성형제품의 허가 후 변경사항을 다루는 SUPAC-IR에 대한 검토

        The objective of this study was to provide a better understanding of SUPAC-IR and its application in handling postapproval changes to immediate release solid oral dosage forms. Originally, SUPAC-IR was aimed at reducing the regulatory burdern of the industry when they were making postapproval changes, but still at maintaining the formulation quality and performance of a drug product. The postapproval changes that were covered under SUPAC-IR included variations in the components and composition of formulation, the site of manufacturing, batch size, manufacturing equipment, and manufacturing process. The guidance defined levels of changes, based on the likelihood of risk ocurrence and potential impact of postapproval changes upon the safety and efficacy of a drug product. It suggested what a type of filling report should be submitted to the FDA for each level of change. Chemistry, manufacturing, and control tests to be executed were also recommended for each change level. The important tests specified in the guidance included batch release, stability, in vitro dissolution, and in vivo bioequivalence tests. However, there have been strong demands on revising the current SUPAC-IR in order to resolve some issues and to improve its usefulness in evaluating postapproval changes to immediate release solid oral dosage forms. In particular, the rigorous requirement of case C dissolution test and the definition of batch size were challenged by both academia and the industry. A revision work was in progress to reflect these inputs and to expand the utility of SUPAC-IR. As a result of these concerted efforts, an updated 2nd version of SPAC-IR would be likely to be issued very soon to the public.

        Background : Serum osteocalcin is synthesized by osteoblast and has been shown to be sensitive indicator of bone turnover inpatients with various metabolic bone disease. In renal osteodystrophy, serum osteocalcin is elevated due to decreased renal clearance and elevated level of PTH. This study was done to evaluate the usefulness of serum osteocalcin as a marker of bone metabolism and the correlation with other biochemical markers of bone metabolism. Methods : We measured serum osteocalcin, calcium, phosphorus, ALP(alkaline phosphatase) and PTH(parathyroid hormone) in 37 patients with end stage renal disease on hemodialysis. Osteocalcin was determined by radioimmunoassay and PTH was determined by radioimmunometric assay. Results : 1) The mean level of serum osteocalcin in ESRD patients was 233.8± 218.2ng/ml which was significantly higher than that of controls(p<0.0001). 2) The mean level of serum PTH in ESRD patients was 40.5± 43.8pg/ml was significantly higher than that of controls(p<0.005). 3) There was a significant positive correlation between the level of serum PTH, ALP and the level of serum osteocalcin in ESRD patients. 4) By using multiple regression, PTH is most reliable factor that affect to elevated level of serum osteocalcin ( beta coefficient = 0.687, Sig T<0.05). Conclusion : Serum osteocalcin as a marker of bone metabolism in ESRD patients is more useful than other biochemical marker such as serum calcium, phosphorus, ALP and PTH is a most reliable factor that affect to elevated level of serum osteocalin.

      • 당뇨병환자에서 게이트심장혈액풀신티그라피를 이용한 심기능 평가

        Major cardiovascular complications of diabetes are coronary atherosclerosis, diabetic dilated cardiomyopathy, autonomic neuropathy and those are major causes of morbidity and mortality in diabetic patients. Gated blood pool heart scan is noninvasive and useful method for evaluation of functional status of heart in diabetics. We evaluated 52 patients with diabetes and divided 3 groups. Group 1 were 11 patients without proteinuria or with proteinuria less than 550mng during 24 hours. Group 2 were 9 patients with proteinuria more than 550mg during 24 hours and group 3 were 32 patients with endstage renal diasese due to diabetes. We performed 99mTc-HSA cardiac gated blood pool scan and used left ventricular ejection fraction(LVEF), peak ejection rate(PEF) to indices of LV systolic function and peak filling rate(PER) to index of LV diastolic function. The results were follows : 1) LVEF, PER were significantly lower in diabetics with ESRD than diabetics without ESRD, but there were no significant difference between normal controls and diabetics without ESRD 2) PFR was significantly lower in diabetics than normal controls, but there were no significant differences in diabetics with or without nephropathy. 3) There were negative correlation between PER, PFR and duration of diabetes. On the basis of results, PFR is a LV functional index of GBPS which can disclose early change of LV dysfunction in patients with diabetes.

        경구용 서방성/지연성 성형제품의 허가 후 변경사항 관리를 위한 SUPAC-MR 응용

        The objective of this study was to scrutinize the rationale of SUPAC-MR and its application in processing postapproval changes to modified release solid oral dosage forms. The types of postapproval changes that were primarily covered with SUPAC-MR included variations in the components and composition, the site of manufacturing, batch size, manufacturing equipment, and manufacturing process. SUPAC-MR defined levels of postapproval changes that the industry might make. Classification of such categories was based on the likelihood of risk occurrence and potential impact of changes upon the safety and efficacy of approved drug products. In most cases, the changes could be classified into 3 levels. It described what chemistry, manufacturing, and control tests should be conducted for each change level. The important tests specified in SUPAC-MR were batch release, stability, in vitro dissolution, and in vivo bioequivalence tests. It then suggested what type of a filing report should be submitted to the FDA for each change level. In general, level 1 changes could be reported in an annual report, whereas level 2 and/or 3 changes could be submitted in changes-being-effected or prior approval supplements. It could be understood that the purpose of SUPAC-MR was to maintain the safety and quality of approved modified release solid oral dosage forms undergoing certain changes. At the same time, it contributed to providing a less burdensome regulatory process with the manufacturers when they wanted to make postapproval changes. European regulatory agencies also implemented SUPAC-like regulations in handling such changes to drug products. Therefore, in this study a recommendation was made for KFDA and the Korean industry to evaluate thoroughly the usefulness of these guidances and regulations in dealing with postapproval changes to modified release solid oral dosage forms.

        효소면역측정법을 이용한 Fumonisin의 검출법 개발

        Fumonisin에 대한 효소면역측정법(ELISA)을 개발하기 위하여 특이항체를 생산하고, 분석조건을 확립하여, 인위적으로 오염시킨 옥수수 시료의 분석을 행하였다. Fumonisin B_1 (FB_1)을 cholera toxin (CT)에 결합시킨 FB_1-CT conjugate를 면역원으로 하여 Freund's adjuvant와 함께, 또는 단독으로 두 군의 토끼에 면역하고 특이항체를 생산하였다. 항혈청을 ELISA로 분석한 결과 그 adjuvant와 함께 면역한 경우 항체역가(titer)가 높게 나타났다. 가장 높은 항체역가(1 : 16,000)를 나타낸 항혈청 및 그로부터 정제한 항체를 이용하여 간접법 및 직접법에 의한 경합적 ELISA(ciELISA 및 cdELISA)를 각각 확립하였다. 이 항체의 유사독소와의 교차반응을 ciELISA로 조사하였을 때, FB_3에 대한 교차반응율은 2%로 매우 낮았으나, FB_2에 대한 것은 179%로 다소 높았다. FB_1의 검출한계는 ciELISA에서 0.03ppb, cdELISA에서 0.3 ppb로 각각 나타났다. 실제 곡물시료의 분석에 ELISA 활용가능성을 조사하기 위하여 인위적으로 FB_1을 오염시킨 옥수수를 75% methanol로 추출하고 그 회수율을 측정한 결과, 분석이 불안정하여 회수율이 일정하게 나타나지 않았다. 그러나 방해물질의 제거를 위하여 시료추출액을 strong anion exchange(SAX) cartridge를 거쳐 세척한 다음 분석하였을 때, 3~10 ppm 범위의 FB_1 오염 옥수수에서 평균 34%(CV의 평균, 8.2%)의 안정된 회수율을 보였다. In order to develop enzyme-linked immunosorbent assay (ELISA) for fumonisins, production of specific antibodies, establishement of ELISA conditions, and quantitation of the toxin from spiked corns by ELISA were performed. Fumonisin B_1 (FB_1) conjugated to cholera toxin (CT) with or without Freund's adjuvant was subcutaneously injected into 2 groups of rabbits. When the titer of the antisera produced by each rabbit tested, higher titer was observed in case of the immunization with the adjuvant. By use of the antiserum showing the highest titer (1 : 16,000) and its purified antibodies, competitive indirect and direct ELISA's (ciELISA and cdELISA) were established, respectively. When the cross-reactivity of the antibody against fumonisin analogs was investigate by the ciELISA is was very low against B_3 (2%) but high against fumonisin B_2 (179%). The sensitivity of the ELISAs was also very high, because the detection limit for FB_1 was 0.03 ppb in ciELISA and 0.3 ppb in cdELISA. When the ELISA's were applied to the spiked corns after extraction with 75% methanol, the assay recovery of FB_1 was too unstable to assay. However, when cleanup by strong anion exchange (SAX) cartridge was introduced to remove interfering materials, the mean ELISA recovery of FB_1 from corns spiked to 3~10 ppm was found to be 34.0% and stable (mean of CV, 8.2%).

      • Parasitic Nematode-Induced CD4 <sup>+</sup> Foxp3 <sup>+</sup> T Cells Can Ameliorate Allergic Airway Inflammation

        <▼1><P><B>Background</B></P><P>The recruitment of CD4<SUP>+</SUP>CD25<SUP>+</SUP>Foxp3<SUP>+</SUP>T (T<SUB>reg</SUB>) cells is one of the most important mechanisms by which parasites down-regulate the immune system.</P><P><B>Methodology/Principal Findings</B></P><P>We compared the effects of T<SUB>reg</SUB> cells from <I>Trichinella spiralis</I>-infected mice and uninfected mice on experimental allergic airway inflammation in order to understand the functions of parasite-induced T<SUB>reg</SUB> cells. After four weeks of <I>T. spiralis</I> infection, we isolated Foxp3-GFP-expressing cells from transgenic mice using a cell sorter. We injected CD4<SUP>+</SUP>Foxp3<SUP>+</SUP> cells from <I>T. spiralis</I>-infected [Inf(+)Foxp3<SUP>+</SUP>] or uninfected [Inf(-)Foxp3<SUP>+</SUP>] mice into the tail veins of C57BL/6 mice before the induction of inflammation or during inflammation. Inflammation was induced by ovalbumin (OVA)-alum sensitization and OVA challenge. The concentrations of the Th2-related cytokines IL-4, IL-5, and IL-13 in the bronchial alveolar lavage fluid and the levels of OVA-specific IgE and IgG1 in the serum were lower in mice that received intravenous application of Inf(+)Foxp3<SUP>+</SUP> cells [IV(inf):+(+) group] than in control mice. Some features of allergic airway inflammation were ameliorated by the intravenous application of Inf(-)Foxp3<SUP>+</SUP> cells [IV(inf):+(-) group], but the effects were less distinct than those observed in the IV(inf):+(+) group. We found that Inf(+)Foxp3<SUP>+</SUP> cells migrated to inflammation sites in the lung and expressed higher levels of T<SUB>reg</SUB>-cell homing receptors (CCR5 and CCR9) and activation markers (Klrg1, Capg, GARP, Gzmb, OX40) than did Inf(-)Foxp3<SUP>+</SUP> cells.</P><P><B>Conclusion/Significance</B></P><P><I>T. spiralis</I> infection promotes the proliferation and functional activation of T<SUB>reg</SUB> cells. Parasite-induced T<SUB>reg</SUB> cells migrate to the inflammation site and suppress immune responses more effectively than non-parasite-induced T<SUB>reg</SUB> cells. The adoptive transfer of Inf(+)Foxp3<SUP>+</SUP> cells is an effective method for the treatment and prevention of allergic airway diseases in mice and is a promising therapeutic approach for the treatment of allergic airway diseases.</P></▼1><▼2><P><B>Author Summary</B></P><P>Many studies have investigated the down-regulation of the immune system by parasite infection. CD4<SUP>+</SUP>CD25<SUP>+</SUP>Foxp3<SUP>+</SUP>T (T<SUB>reg</SUB>) cells are key players in parasite-mediated immune downregulation. Our previous study suggested that T<SUB>reg</SUB> cells recruited by <I>Trichinella spiralis</I> infection were the key cells mediating the amelioration of allergic airway inflammation in mice. In the present study, we investigated the functions of parasite-induced T<SUB>reg</SUB> cells using mice expressing GFP-tagged Foxp3. <I>T. spiralis</I> infection increased the number of T<SUB>reg</SUB> cells. Adoptive transfer of the parasite-induced T<SUB>reg</SUB> cells to mice with allergic airway inflammation ameliorated allergic airway inflammation. The transferred cells were recruited to inflammation sites in the lung. Cells from parasite-infected mice expressed higher levels of T<SUB>reg</SUB>-cell homing receptors and activation markers than did cells from uninfected mice. This study might help explain why immune disorders (often of unknown cause) are more prevalent among people in developed countries (areas with low parasite infection) than among those in developing countries (areas with parasite epidemics). Our finding might improve current cell therapy techniques and facilitate the development of new techniques that use parasites or parasite-borne materials to treat diverse immune disorders.</P></▼2>

        A Research on the Relationship between Accrual-based Earnings Management and Real Earnings Management in the Retail Industry

        Purpose - In this paper, we examine the effect of accrual earnings management and real earnings management on the corporate value of retail corporations. Research design, data, and Methodology - The sample cover firms whose settlement is December among retail companies listed on the Korea Stock Exchange's securities market and KOSDAQ market from 2001 to 2016. Of these, the targets were companies with operating profit and equity capital of zero or higher and with sales data. The secondary data was collected through KIS-VALUE data base. The Jones model and the modified Jones model were used for the calculating the accrual-based earnings management and the real earnings management. Result - According to the empirical results, the relationship between accrual earnings management, real earnings management and firm value is positively significant in the retail industry as in manufacturing industry. These results are also significant when controlling the size, profitability, investment, debt ratio, dividend, and growth potential of a company. Conclusions - The characteristics of the distribution business can be identified and the influence of the various kinds of earnings management, which is being researched around the manufacturing industry, can be studied in the distribution industry to give practical implications to investors..
