      • Humanitarian Assistance Ontology for Emergency Disaster Response

        <P>Massive amounts of open data related to a crisis aren't fully used to identify humanitarian needs because most of the data exists in an unstructured format, thus hindering machines in its interpretation. Automatic processing of crisis data in a short time period would provide useful information to decision makers. To address these problems, this article presents a method for merging ontologies and logic rules to represent humanitarian needs and recommend appropriate humanitarian responses. The main advantage of the method is that it identifies humanitarian needs and prioritizes humanitarian responses automatically, including health survival issues and possible responses by creating a working system during the evolution of the crisis. The method is implemented on 125 real data reports from the Hurricane Wilma crisis and compared to the actual support provided.</P>

        Efficacy and Acceptance of a Commercial Hoodia parviflora Product for Support of Appetite and Weight Control in a Consumer Trial

        Species of Hoodia Sweet ex Decne., family Apocynaceae, a southern African succulent plant, have been recognized for their appetite suppressing properties. Products that support appetite and weight control have been developed in Israel from locally cultivated Hoodia spp. To study consumer acceptance, efficacy of, and tolerance for a frozen product based on whole aerial parts of Hoodia parviflora N.E. Br., we initiated and conducted this single-blind, randomized, placebocontrolled consumer trial. Volunteer participants ingested flavored 3 g frozen Hoodia or placebo cubes for 40 days. Subjects were weighed and measured and baseline body–mass index was determined. Adverse events were monitored and eight mild, transient, possible treatment-emergent events were reported. No moderate, severe, or chronic events were reported. On days 1, 10, and 40, subjects self-reported their perceptions of food consumption, hunger development, incidence and control of food cravings, and efficacy of the product. On day 40, the treatment group demonstrated a statistically significant decrease in measured quantitative parameters against the placebo and reported a positive perception of the product.

        Predisposing factors for high output stoma in patients with a diverting loop ileostomy after colorectal surgeries

        Purpose: One of the most common ileostomy-related complications is high output stoma (HOS) which causes significant fluids and electrolytes disturbances. We aimed to analyze the incidence, severity, and risk factors for readmission for HOS. Methods: We reviewed all patients who underwent loop ileostomy closure in a single institution between 2010 and 2020. Patients that were readmitted for dehydration due to HOS during the time interval between the creation and the closure of the stoma were identified and divided into a study (HOS) group. The remaining patients constructed the control group. Results: A total of 307 patients were included in this study, out of which, 41 patients were readmitted 73 times (23.7% readmission rate) for the HOS group, and the remaining 266 patients constructed the control group. Multivariate analysis identified; advanced American Society of Anesthesiologists (ASA) physical status (PS) classification, elevated baseline creatinine, and open surgery as risk factors for HOS. Renal function worsened among the entire cohort between the construction of the stoma to its closure (mean creatinine of 0.82 vs. 0.96, P<0.0001). Conclusion: Loop ileostomy formation is associated with a substantial readmission rate for dehydration as a result of HOS, and increasing the risk for renal impairment during the duration of the diversion. We identified advanced ASA PS classification, open surgery, and elevated baseline creatinine as predictors for HOS.

      • Factors Affecting Graft Survival After Adult/Child Split-Liver Transplantation: Analysis of the UNOS/OPTN Data Base

        <P>When considering advocacy of split-liver transplantation, it is important to understand whether comparable outcomes can be achieved. The goal of this study was to identify donor and transplant characteristics predictive of comparable outcomes by risk factor analysis. Using the United Network for Organ Sharing/ Organ Procurement and Transplantation Network data base between January 1996 and May 2006, first time adult/child split cases (568 adults, 508 children) were examined. In multivariate analysis, recipient medical condition (hospitalization), status 1 assignment, ABO incompatibility, donor age (>40 years), donor body weight (< or = 40 kg), calculated whole graft volume to recipient body weight ratio (cGRWR < or = 1.5%) and no sharing between centers were significant risk factors in adult recipients. Recipient diagnosis of tumor, dialysis prior to transplant, recipient body weight (< or = 6 kg), donor age (>30 years), donor history of cardiac arrest after declaration of death and cold ischemia time (CIT > 6 h) increased the risk of graft failure in pediatric recipients. The livers from young donors showed comparable outcomes to whole deceased liver transplantation (LT) when other transplant-related risk factors were minimized in adult recipients. Reducing CIT is important to obtain comparable outcomes to living donor LT in pediatric recipients.</P>

Fluorescence intensity of the B–SAL–OXIME species increases in the presence of DECP, whereas it decreases in the presence of DCP and DEMP (limit of detection = 997 nM).
