A Case Report of Efficacy of Growth Height and Peak-Luteinizing Hormone Level Suppression on Idiopathic Gonadotropin-Dependent Precocious Puberty Patient Using Herbal Remedy, Aesopjiyoun-tang

The purpose of this report is to evaluate effect of Korean medical treatment on idiopathic gonadotropin-dependent precocious puberty (G-DPP) patient received herbal medicine. We administered Aesopjiyoun-tang remedy to idiopathic G-DPP and analyzed the delay effect by hormonal value and radiographs; the height growth effect by measurement of height. After Korean medical treatment, suppression effect to peak- Luteinizing Hormone level (LHL) is 14.39IU/L to 10.9IU/L for 13month, growth effect to height value is 11cm/13month; and change of mean growth velocity (MGV) is 6.08cm/year to 10.06cm/year. The gain in height by treatment is 3.98cm/year. The result suggests Aesopjiyoun-tang can be an effective treatment for G-DPP. Herbal medicine can be used as an alternative treatment in place of the GnRH treatment.

Isolation and Morphological Characteristics of Nematophagous fungi

1997년 8월과 9월에 걸쳐 경남 철마군 일대의 상추재배지에서 임의로 재취한 토양에서 선충 포식성곰팡이를 분리한 결과 Arthrobotrys cladodes한 종을 분리 동정하였으며, 그 형태적 특징으로서 배지상에 길이가 200~400㎛이며 몇 개의 분지를 가진 수직으로 형성되는 분생자가 형성되고, 각 분생포자는 중앙에 1개의 격막을 가지고 있으며, 그 크기는 11~18×6.2~8.8㎛이다. 선충을 잡기 위한 포식기관으로서 끈끈이 그물을 형성한다. A nematophagous fungus, Arthrobotrys cladodes, was isolated and identified from soil samples which were randomly selected throughout lettuce fields at Kyungnam between August and September 1997. A. cladodes produces erect conidiospores producing 1-2 side branches frequently. The size of conidiospores is 200∼400㎛ long, and are non-proliferation. Conidia is obovoidal to ellipsoidal with one median septum (11∼18×6.2∼8.8㎛ in size). The nematophagous fungus captures nematodes by means of adhesive networks.

Hall mobility and characteristics of gas-phase polymerized poly(3-iodothiophene) thin films

<P><B>Abstract</B></P><P>The semi-conductive poly(3-iodothiophene)(P3IT) films were fabricated by gas-phase polymerization through a chemical vapor deposition process. The P3IT nanoscale films have a high crystalline morphologies, and possessed a high Hall mobility up to 10 cm<SUP>2</SUP>/Vs. The degree of crystalline and the mobility values measured through Scanning Electron Microscopy, atomic force microscopy, and X-ray photoelectron spectroscopy with structural analysis. These conductive thin films, possessing polycrystalline structures, have a very high mobility and are capable of being applied to organic electronic layers for electrical devices such as the thin film transistors and organic photovoltaic cells.</P> <P><B>Highlights</B></P><P>► Poly(3-iodothiophene) film were fabricated by gas-phase polymerization. ► These P3IT films have a possessed a high Hall mobility up to 10 cm<SUP>2</SUP>/Vs. ► These films could apply to organic electronic layers for electrical devices.</P>

Peroneus longus activities according to various angle of ramp during cross ramp walking and one-leg standing

The purpose of study was to clear up the environmental risk factor that can be easily occurred ankle sprain during gait (stance phase) and one leg standing at various ramp environment. Twenties healthy males of 48 subjects is participated in this study. Participants was measured muscle activation on peroneus longus during both conditions (walk across the ramp and one leg stand in the transverse direction) of seven different angle (0°, 2°, 5°, 10°, 15°, 20°, 25°). The measured data were analyzed using one-way ANOVA to investigate the effect of muscle activation on the each condition. The statistical analyses were performed using SPSS ver. 17.0 and p-value less than 0.05 were considered significant for all cases. A post-hoc test was performed by Tukey’s test. This study was approved by Kyungsung University's Human Research Ethics Committee. The study showed that more muscle activities on peroneus longus is increased by increasing the ramp angle. Post-hoc analysis demonstrated that the peak and mean of muscle activity increased significantly with a ramp angle of 10° and 25° during gait and one-leg stand. According to the study results, ankle sprain can be easily occurred when cross walk and one-leg stand on a ramp from higher than 10°, and highest risk was angle of 25° or more. As a people with peroneus longus weakness walks a ramp, the ramp angle has a cross relationship with the sprain on the ankle. If people with peroneus longus weakness walk on the more than 10° of ramp angle, they will need a lot of attention for prevent sprain on ankle. This study will provide the basic data about conditions of prevention ankle sprine ; when patients with peroneus longus weakness are walking and one-leg standing on the cross ramp.

Perceived Stressful Life Events, Coping Style, Social Support and Depressive Symptoms of the Functional Dyspepsia Patients

연구목적: 가능성 소화장애증 환자의 대처방식, 사회적 지지, 지각된 스트레스, 정신병리 특히 불안과 우울을 조사하고, 이를 기능성 소화장애증이 없는 비기질적 증상을 가진 환자와 비교하고자 하였다. 방법: 기질적 병변이 없이 기능성 소화장애증으로 진단된 30명(환자군)을 기능성 소화장애증의 증상이 없이 피로 같은 비기질적 증상을 호소하여 내원한 정상 성인 30명(대조군)이 비교되었다. 대조군의 모든 검사 결과는 정상이었다. 소화기 내과 의사가 진단적 평가를 내리기 전에, 두 군 모두 불안과 우울, 대처방식, 사회적 지지 등이 평가되었다. 평가도구로는 간이정신진단검사, Beck 우울척도, Spielberger 상태-특성 불안척도, 대처방식 척도, 대인관계 지지 척도, 그리고 지각된 스트레스적 생활 사건의 양을 평가하는 자기 보고형 설문지가 사용되었다. 결과: 기능성 소화장애증 환자는 대조군에 비하여 우울과 스트레스적 생활사건이 유의하게 많았고, 문제집 중적 대처와 대인관계적 지지가 유의하게 낮았다. 기능성 소화장애증 환자의 우울은 대인관계적 지지와 유의한 부적 상관이 있었고, 특성불안과 유의한 정적 상관이 있었으며, 문제집중적 대처와 대인관계적 지지는 유의한 정적 상관이 있었다. 두 군은 불안과 정서집중적 대처에서 유의한 차이가 없었다. 결론: 기능성 소화장애증 환자는 신경정신과적 평가 및 약물치료를 포함한 정신치료적 중재가 필요함을 시사하고 있으며, 기능성 소화장애증은 생물정신사회적 과정이 그 원인과 반응에서 중요한 역할을 하고 있는 증후군으로, 실제적인 정신신체장애 증후군임을 나타내고 있다. 중심단어:기능성 소화장애증·대처·사회적 지지·지각된 스트레스·우울 Objectives: To investigate various factors that can mediate stress and response, such as coping style, social support, level of perceived stress, and psychopathology(especially anxiety and depression) of the functional dyspepsia(FD). Methods: Thiry adults with functional dyspepsia were compared to thirty healthy adults who had visited the hepatobiliary clinic due to non-organic complaints(tiredness and fatigue) withour FD, but the results of medical examination were normal. All subjects were evaluated for anxiety and depressive symptoms, perceived stress, coping style and social support. Symptom Checklist-90-Revision, Beck Depression Inventory, Spielberger State-trait Anxiety Inventory, Ways of Coping checklist, Interpersonal Support Evaluation List, and a self-reporting questionnaire were included to measure the quantity of perceived stressful life events. Results: The FD patients had significantly more symptoms of depression, more perceived stressful life events, demonstrated less problem-focused coping, and less social support than the control subjects. Depressive symptoms of the FD patients were negatively correlated with interpersonal support and were positively correlated with trait anxiety, whereas, problem-focused coping were positively correlated with interpersonal support. The two groups did not differ signigicantly in terms of anxiety and emotion-focused coping. Conclusions: Our results suggest that FD patients need to be evaluated for psychiatric problems, and might benefit from psychiatric treament with psychotherapeutic and psychopharmacological modalities.

Perioperative Complications of Orthopedic Surgery for Lower Extremity in Patients with Cerebral Palsy

<P>Because complications are more common in patients with cerebral palsy (CP), surgeons and anesthesiologists must be aware of perioperative morbidity and be prepared to recognize and treat perioperative complications. This study aimed to determine the incidence of and risk factors for perioperative complications of orthopedic surgery on the lower extremities in patients with CP. We reviewed the medical records of consecutive CP patients undergoing orthopedic surgery. Medical history, anesthesia emergence time, intraoperative body temperature, heart rate, blood pressure, immediate postoperative complications, Gross Motor Function Classification System (GMFCS) level, Cormack-Lehane classification, and American Society of Anesthesiologists physical status classification were analyzed. A total of 868 patients was included. Mean age at first surgery was 11.8 (7.6) yr. The incidences of intraoperative hypothermia, absolute hypotension, and absolute bradycardia were 26.2%, 4.4%, and 20.0%, respectively. Twenty (2.3%) patients had major complications, and 35 (4.0%) patients had minor complications postoperatively. The incidences of intraoperative hypothermia, absolute hypotension, and major postoperative complications were significantly higher in patients at GMFCS levels IV and V compared with patients at GMFCS levels I to III (<I>P</I><0.001). History of pneumonia was associated with intraoperative absolute hypotension and major postoperative complications (<I>P</I><0.001). These results revealed that GMFCS level, patient age, hip reconstructive surgery, and history of pneumonia are associated with adverse effects on intraoperative body temperature, the cardiovascular system, and immediate postoperative complications.</P>

Combined Antitumor Chemotherapy in a Refractory Schizophrenic Receiving Clozapine

Clozapine을 투여 받고 있는 정신분열병 환자에게 항암제 병합투여는 clozapine과 항암제 모두의 무과립구증 유도 가능성 때문에 권고되지 않고 있다. 그러나 무과립구증 치료에 granulocyte-colony stimulating factor(G-CSF)가 소개된 이후로, G-CSF는 항암제 또는 Clozapine과 연관된 무과립구증의 치료에 사용되어 왔다. 저자들은 clozapine을 투여 받고 있던 치료 불응성 정신분열병 환자에서 성선간질 종양이 발견되어 항암제와 병합 치료하던 중 발생한 무과립구증을 G-CSF를 이용하여 성공적으로 치료한 1예를 보고하는 바이다. Clozapine과 항암제 병합 투여 전의 혈액학적 소견은 정상이었으며, 항암제 치료를 받는 도중 발생한 무과립증은 G-CSF 치료에 의해서 회복되었다. 본 증례는 clozapine 유지 치료 중이던 치료 불응성 정신분열병 환자에서 종양이 발생한 경우, clozapine에 대한 치료 효과가 좋았고 clozapine과 항암제 치료전의 혈액학적 소견이 정상이었다면, 종양 전문의사와 정신과 의사에 의해서 혈액학적 추적조사를 지속해 나가면서, 무과립구증 발생을 예견하여, G-CSF 사용을 고려하는 경우에 clozapine 치료를 유지해 갈 수 있다고 조심스럽게 제안한 증례이다. It has been known that clozapine treatment combining with cytotoxic antitumor therapy for schizophrenia is not recommended because both drugs have agranulocytosis as their side effect. Since the introduction of granulocyte colony-stimulating factor(G-CSF), it has been used to treat agranulocytosis or granulocytopenia associated with antitumor chemotherapy or clozapine. We report a case with schizophrenia on clozapine treatment who developed agranulocytosis following combined cytotoxic chemotherapy for a sex-cord stromal tumor which was successfully treated with G-CSF. The hematological status before combining with antitumor chemotherapy had been within normal range, and agranulocytosis following the antitumor chemotherapy returned to normal after treatment with G-CSF. This suggests that clozapine could be administered in combination with cytotoxic antitumor agents if the following indications are met : normal hematological status before starting antitumor chemotherapy, carefully monitoring hematological status by oncologist and psychiatrist, and prepared G-CSF administration when agranulocytosis is anticipated.
