접착제와 와동형성의 차이에 따른 5급 복합레진 수복의 전향적 임상연구

본 연구의 목적은 복합레진을 이용한 비우식성 치경부 5급 병소 수복에 있어서, 3가지 다른 접착제를, 유지구를 부여한 치아와 부여하지 않은 치아에 적용하였을 때의 임상적인 효능의 차이를 비교하기 위함이다. 총 150개의 치아를 각각 25개씩 6개의 군으로 나누었다. A군 Scotchbond Multi-Purpose (SBMP, 3M ESPE, St, Paul, MN, USA, 4세대 접착제)를 사용하고 유지구를 부여하지 않음. B군: SBMP를 사용하고 유지구를 부여. C군: BC Plus (Vericom Co., Anyang, Gyeonggido, Korea, 5세대 접착제)를 사용하고 유지구를 부여하지 않음. D군: BC Plus를 사용하고 유지구를 부여. E군: Adper Prompt (3M ESPE, Seefeld, Germany, 6세대 접착제)를 사용하고 유지구를 부여하지 않음. F군: Adper Prompt를 사용하고 유지구를 부여. 모든 치아는 복합레진인 Denfil(Vericom Co., Anyang. Gyonggido. Korea)을 이용하여 충전하였다. 수복 직후와, 수복 6개월 후에 modified USPHS (United States Public Health Service) criteria에 따라 수복물을 임상적으로 평가하였다. 추가적으로 임상사진을 촬영하였으며, 에폭시 레진을 이용하여 복제한 후 주사전자현미경을 이용하여 이를 관찰하였다. 6개월 후 관찰 결과. alpha rating의 수가 각 실험군마다 동일하지는 않았지만, 통계적으로는 3가지의 접착제간에 유의할만한 차이는 없었고, 기계적인 유지구를 부여한 군과 부여하지 않은 군 간에도 유의할만한 차이는 없었다 (p > 0.05). 그러나 6개월의 관찰 기간은 일반적으로 기대되는 레진의 수명보다 짧기 때문에, 각 실험군 간의 차이를 알아보기 위해서는 추후 더 오랜 기간의 관찰이 요구된다. The purpose of this study is to evaluate prospectively the effect of different bonding systems and retention grooves on the clinical performance of resin restorations in non-carious cervical lesions (NCCLs). Thirty nine healthy adults who had at least 2 NCCLs in their promolar areas were included in this study. One hundred and fifty teeth were equally assigned to six groups; (A) Scotchbond Multi-Purpose (SBMP, 3M ESPE, St, Paul, MN, USA, 4th generation bonding system) without retention grooves; (B) SBMP with retention grooves: (C) BC Plus (Vericom Co., Anyang, Gyeonggido, Korea, 5th generation bonding system) without retention grooves; (D) BC Plus with retention grooves; (E) Adper Prompt (3M ESPE, Seefeld, Germany, 6th generation bonding system) without retention grooves: (F) Adper Prompt with retention grooves. All cavities were filled with a hybrid composite resin, Denfil (Vericom Co., Anyang, Gyeonggido, korea) by one operator. Restorations were evaluated at baseline and at 6-month recall, according to the modified USPHS (United states Public Health Service) criteria, Additionally, clinical photographs were taken and epoxy resin replicas were made for SEM evaluation. At 6-month recall, there were some differences in the number of alpha ratings among the experimental groups. But, despite the differences in the number of alpah ratings, there was no significant difference among the 3 adhesive systems (p > 0.05). There was also no significant difference between the groups with or without mechanical retention (p > 0.05). Follow-ups for longer periods than 6 months are needed to verify the clinical performance of different bonding systems and retention grooves. 〔J Kor Acad Cons Dent 31(4):300-311, 2006〕

A formal framework for prefetching based on the type-level access pattern in object-relational DBMSs

Prefetching is an effective method for minimizing the number of fetches between the client and the server in a database management system. In this paper, we formally define the notion of prefetching. We also formally propose new notions of the type-level access locality and type-level access pattern. The type-level access locality is a phenomenon that repetitive patterns exist in the attributes referenced. The type-level access pattern is a pattern of attributes that are referenced in accessing the objects. We then develop an efficient capturing and prefetching policy based on this formal framework. Existing prefetching methods are based on object-level or page-level access patterns, which consist of object-ids or page-ids of the objects accessed. However, the drawback of these methods is that they work only when exactly the same objects or pages are accessed repeatedly. In contrast, even though the same objects are not accessed repeatedly, our technique effectively prefetches objects if the same attributes are referenced repeatedly, i.e., if there is type-level access locality. Many navigational applications in object-relational database management systems (ORDBMSs) have type-level access locality. Therefore, our technique can be employed in ORDBMSs to effectively reduce the number of fetches, thereby significantly enhancing the performance. We also address issues in implementing the proposed algorithm. We have conducted extensive experiments in a prototype ORDBMS to show effectiveness of our algorithm. Experimental results using the 007 benchmark, a real GIS application, and an XML application show that our technique reduces the number of fetches by orders of magnitude and improves the elapsed time by several factors over on-demand fetching and context-based prefetching, which is a state-of-the-art prefetching method. These results indicate that our approach provides a new paradigm in prefetching that improves performance of navigational applications significantly and is a practical method that can be implemented in commercial ORDBMSs.

<P>This paper investigates spectral shaping of multi-carrier-modulation waveforms based on combination of Nyquist windowing and subband filtering. The combined windowing/filtering allows simultaneous control on both subcarrier and subband spectra. When compared with the existing Nyquist windowing or subband filtering techniques under a fixed excess frame length constraint, the proposed scheme offers reduced sensitivity to carrier frequency and symbol timing offsets. Establishing an analytical tool based on the error spectrum stack consisting of the error signals evaluated at different signal delay positions, we explore the window-filter trade-off and provide the minimum interference power solution for given ranges of carrier frequency and symbol timing offsets. Our design targets low-latency applications having no provisions for high-precision synchronization and having potential need for spectrum aggregation.</P>

Low-cost enzyme sources are requested for the production of chitosan oligomers, which were recently spotlighted as functional oligosaccharides. Gizzard tissue of broiler and its chyme are selected for the production of chitosan oligomers, and chitosanolytic activities were investigated. Chitosanolytic activity of chyme was about two times higher than that of gizzard tissue. Repeated freezing and thawing did not affect the activity of crude enzyme preparations. Optimal pH for the gizzard and the chyme were 3.0 and 2.8, respectively. Optimal temperature of the crude enzymes ranged from 60℃ to 70℃ and their activities were maintained at 70% level at 80℃. At 50℃ for three days no decrease in their activity was observed. After 24 hour incubation, gizzard and chyme produced 40 yg oligomers/mg-protein and 80 yg oligomers/mg-protein, respectively. Under optimal conditions, 62% of total substrate were made water-soluble by the gizzard. Trimer of glucosamine was identified as its main reaction product.

Background/Aims: The most appropriate treatment for acute gastric variceal bleeding (GVB) is currently endoscopic gastric variceal obturation (GVO) using Histoacryl®. However, the secondary prophylactic efficacy of beta-blocker (BB) after GVO for the first acute episode of GVB has not yet been established. The secondary prophylactic efficacy of BB after GVO for the first acute episode of GVB was evaluated in this study. Methods: Ninety-three patients at Soonchunhyang University Hospital with acute GVB who received GVO using Histoacryl® were enrolled between June 2001 and March 2010. Among these, 42 patients underwent GVO alone (GVO group) and 51 patients underwent GVO with adjuvant BB therapy (GVO+BB group). This study was intended for patients in whom a desired heart rate was reached. The rates of rebleeding-free survival and overall survival were calculated for the two study groups using Kaplan-Meyer analysis and Cox``s proportional-hazards model. Results: The follow-up period after the initial eradication of gastric varices was 18.14±25.22 months (mean±SD). During the follow-up period, rebleeding occurred in 10 (23.8%) and 21 (41.2%) GVO and GVO+BB patients, respectively, and 39 patients died [23 (54.8%) in the GVO group and 16 (31.4%) in the GVO+BB group]. The mean rebleeding-free survival time did not differ significantly between the GVO and GVO+BB groups (65.40 and 37.40 months, respectively; P=0.774), whereas the mean overall survival time did differ (52.54 and 72.65 months, respectively; P=0.036). Conclusions: Adjuvant BB therapy after GVO using Histoacryl® for the first acute episode of GVB could decrease the mortality rate relative to GVO alone. However, adjuvant BB therapy afforded no benefit for the secondary prevention of rebleeding in GV. (Clin Mol HepatoI2013;19:280-287)

Objective : Glutathione S-transferase polymorphism (GST) were examined in 120 cases with ischemic cerebrovascular disease (ICVD) to test the hyperthesis that GST polymorphisms confer a risk to an individual to develop ICVD. Tobacco smoking is a major cause of both cancer and vascular disease. Methods : therefore We were stratified the subjects with ICVD for smoking status, and then examined whether polymorphisms in this detoxification enzyme gene, GST, influence risk of ICVD Results : Neither GSTM1 nor GSTT1 genotypes in the ICVD group was significantly different from the control group (n=207), even in smokers. We attempted the combined analyses for GSTM1 and GSTT1 genotypes in ICVD for smoking status. No significant association observed between the combined genotypes and ICVD Conclusion : Our observation do not confirm the effect of the GSTM1 and GSTT1 genotypes as a risk factor for ICVD, even in smokers.
