        Endoscopic Ultrasound-Guided Fine-Needle Aspiration of the Adrenal Glands: Analysis of 21 Patients

        Background/Aims: Endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) cytology of adrenal masses helps in etiological diagnosis. The aim of this study was to evaluate the diagnostic yield of EUS-FNA of adrenal masses in cases where other imaging methods failed and/or were not feasible. Methods: Twenty-one consecutive patients with adrenal masses, in whom adrenal FNA was performed because conventional imaging modalities failed and/or were not feasible, were prospectively evaluated over a period of 3 years. Results: Of the 21 patients (mean age, 56±12.2 years; male:female ratio, 2:1), 12 had pyrexia of unknown origin and the other nine underwent evaluation for metastasis. The median lesion size was 2.4×1.6 cm. Ten patients were diagnosed with tuberculosis (shown by the presence of caseating granulomas [n=10] and acid-fast bacilli [n=4]). Two patients had EUS-FNA results suggestive of histoplasmosis. The other patients had metastatic lung carcinoma (n=6), hepatocellular carcinoma (n=1), and adrenal lipoma (n=1) and adrenal myelolipoma (n=1). EUS results were not suggestive of any particular etiology. No procedure-related adverse events occurred. Conclusions: EUS-FNA is a safe and effective method for evaluating adrenal masses, and it yields diagnosis in cases where tissue diagnosis is impossible or has failed using conventional imaging modalities.

      • Planning for Rural Tourism Promotion and Infrastructure Development in Uttrakhad, India- An Experiment

        This paper reflects about Rural Tourism Planning and infrastructure development in remote areas of Uttrakhnad. The intended objectives of rural tourism is to control rural migration, create new livelihood option for the villagers, and Conservation & Restoration of the Temple precincts when integrated with other facilities like the approach road and sanitation and services as part of the concerned subproject proposal, is to develop this area as an attractive destination for cultural and rural tourism. Under this Project, thrust will be to promote village tourism as the primary tourism product to spread tourism and its socio-economic benefits to rural and new geographic regions. Key geographic regions have been identified for development and promotion of Rural Tourism. The implementation would be done through a Convergence Committee headed by the District Magistrate. Activities like improving the natural environment, hygiene, socio-economic status, infrastructure etc. would be eligible for assistance.

      • Load Balancing with Tree Parity Machine

        With rapidly growing influence and demand for cloud computing services, issues involving control, migration, security, availability of data, trust matters directly related to sensitive information stored can no longer be treated lightly. Through our work as explained in this paper, we discuss these topics, and also review several algorithms which are used in cloud computing. These algorithms are implemented in a cloud computing architecture to balance load to improve efficiency. Weighted active monitoring load balancing algorithm, dynamic load balancing algorithm, static algorithms, and ant colony algorithm are all used for load balancing in distributed computing systems. In this paper, we propose a load balancing technique based on Tree Parity Machine. Our approach is to design a Tree Parity Machine based model that would distribute workload evenly among all nodes. The technique proposed by us is not complicated. It has been conceived such that it can work efficiently with training sets that have been proven to produce intended results. Our Tree Parity Machine based model would predict the demand and, accordingly, allocate necessary resources. Thus, it would be able to maintain active servers to meet existing demands. As a result, consumption of energy can be significantly lowered. The conservative approach of over-provisioning is, thus, avoided.

        Morphological, microstructural, and mechanical study of FGM coatings prepared using the HVOF technique

        In this work, functionally graded material (FGM) coating that uses three powders with WC-10 wt% Co-4 wt% Cr (designated as P1), WC-12 wt% Co (designated as P2), and Ni-25 wt% Cr (designated as P3) are proposed for use over the liners of CI engines. Scanning electron microscopy and energy dispersive x-ray spectroscopy are conducted to analyze the microstructural properties, composition, and quality of the powder and the prepared coating. Results shows that the porosity of powder P3 is the highest of all three powders used in this work, and the porosity of powder P2 is smallest, whereas the porosity of powder P1 lies between P1 and P3. In addition, the hardness of the FGM coating, measured using Vicker’s test, containing a combination of P1 and P2 (designated as sample 1), is higher than that of the FGM prepared with a combination of P2, P3 and P1, P3. The findings of the present work have potential implications for improving the performance and durability of CI engine liners by using an appropriate FGM coating.

        Design of an ON/OFF Mist Duty Cycle in Mist Bioreactors for the Growth of Hairy Roots

        In this study, we designed a strategy for selecting the operating conditions of a mist reactor for the growth of hairy roots. This was accomplished by developing a mathematical model to optimize the ON/OFF mist duty cycle for the specified growth of hairy roots. The availability and rate of transport of nutrients to the roots are important parameters in the design and operation of the reactor. The thin liquid film over the root surface, which continuously builds up during the ON cycle, is a major factor that limits mass transfer. The same film also acts as a finite reservoir of nutrients in the absence of any replenishment during the OFF cycle. This reservoir gets depleted as growing roots continue to consume the nutrients. As a result of this depletion the duration of the OFF cycle must be limited such that the nutrient concentration does not go below the critical value required for the specified growth rate. The depleted reservoir is then replenished during the next ON cycle to an extent that depends on feed concentration and duration of the next ON cycle. It was shown that the use of increasing feed concentrations in the fed-batch mode of operation could maintain a specified growth rate in the reactor. Interestingly, this also resulted in the efficient operation of the reactor whereby the reactor operated at slightly above the required concentration and close to the point of minimum mass transfer resistance.

      • Role of Dental Institutions in Tobacco Cessation in India: Current Status and Future Prospects

        Tobacco abuse is a major preventable cause of premature death and disease, including various cancers. The Global Adult Tobacco Survey India (GATS) 2009-10 revealed that more than one-third of adults use tobacco in one form or the other. Nearly two in five smokers and smokeless tobacco users made attempts to quit the habit in the past 12 months. Tobacco dependence is a chronic condition characterized by susceptibility of relapse over years. It can be well handled by sustained professional support from health care providers mainly through behavioral counseling and pharmacotherapy. Dental professionals can play a pivotal role in diagnosing and effectively managing tobacco dependence. Dental Institutions have rapidly grown in last two decades across the country and so has the curriculum been adapted to improve student competencies to accommodate changing disease patterns and technological advances, but not in regard to tobacco cessation. Untapped dental manpower like undergraduates, dental hygienists and other paramedical staff need effective training to be more penetrative. The present review paper explores the potential role of dental training institutions and recommends various approaches to counter public health jeopardy of tobacco related diseases.
