        실내사무환경의 환경성담배연기(ETS)의 지표물질에 관한 연구

        The purpose of this research is to evaluate indoor office environment using the concentrations of nicotine, 3-enthenyl-pyridine(3-EP), and respirable suspended particulate(RSP), which are indicators for environmental tobacco smoke(ETS) and the correlations between indicators and environmental conditions(smoking density, smoking index). The mean air charge per hour (ACH) in smoking rooms was 10.4 and most of the smoking rooms showed non-compliance withe ASHRAE standard value except only one smoking rooms. The concentrations of RSP, 3-EP, nicotine showed log=normal distributions, and became different statistically depending on smoking condition(p〈0.01). The geometric mean concentration of RSP in smoking room was 441.7 ug/㎡ that is far exceeded environmental standard(150 ug/㎡). This implies that fine particulate in smoking room should be carefully controlled considering smoking density and ventilation fate. The mean concentrations of nicotine and 3-EP were 93.4 ug/㎥, respectively. The correlation coefficients between RSP and SI, 3-EP and SI, and Nicotine and SI were 0.67, 0.84 and 0.74, respectively. The correlation coefficient between nicotine and 3-EP, nicotine and RSP, and RSP and 3-EP were 0.76,0.78 and 0.57 respectively.

        The authors have studied the pathogenecity of staphylococci isolated from 96 patients in Chungnam National University Hospital and Staphylococci isolated from nose and axilla of 50 medical and 50 nursing students in Chungnam National University. The authors conducted the survey from January 1986 to June 1986. The results obtained were as follows : 1. Out of staphylococci isolated from 96 patients, Staphylococcus aureus and Staphylococcus epidermidis were 59.4%, and 32.3%, respectively. In staphylococci isolated from nose of medical and nursing students, Staphylococcus aureus was 13.6^(male), 17.2%(female) and Staphylococcus epidermidis was 61.0%(male), 75.9%(female), respectively. 2. In the biotype of Staphylococcus epidermidis isolated from patients, the rate of biotype 1 was 93.5%. In the staphylococcus epidermidis isolated from nose of medical and nursing students, the rate of biotype 1 was 55.6%(male), 31.8%(female). In the axilla, the rate of biotype 1 was 56.5%(male), 52.6%(female), respectively. 3. In the biotype 1 of Staphylococcus epidermidis isolated from patients and control groups, enzymatic activities were more active than other biotypes. 4. Crystal violet ring test of Staphylococcus epidermidis isolated from patients, biotype 1 was WR type 22.6%, SR type 67.7% and DR type 3.2%. 5. To the antibiotic activities of staphylococci isolated from patients, Staphylococcus aureus was resistant to gentamycin(96.5%), penicillin(78.9%), lincomycin(47.4%) and erythromycine(35.1%). In the nose of medical students, Staphylococcus aureus was resistant to penicillin(11.9%), gentamycin(11.9%), lincomycin(6.8%) and tetracycline(5.1%). 6. In the multiple antibiotic resistance of staphylococci isolated from patients, Staphylococcus aureus was resistant to 2 antibiotics(35.1%), 3 antibiotics(17.5%), 5 antibiotics(15.8%) and, Staphylococcus epidermidis was resistant to 3 antibiotics(32.3%), 4 antibiotics(19.4%) and 5 antibiotics(16.1%).

      • Palmitate induced secretion of IL-6 and MCP-1 in orbital fibroblasts derived from patients with thyroid-associated ophthalmopathy

        <P><B>Purpose</B></P><P>Orbital fibroblasts are now recognized as the key effectors in the development of thyroid associated ophthalmopathy (TAO). TAO is clinically apparent in approximately 50% of patients with Graves’ hyperthyroidism. High levels of plasma free fatty acids (FFAs) are frequently seen in patients with hyperthyroidism. Palmitate is one of the most abundant FFAs in plasma and aggravates inflammation by promoting secretion of pro-inflammatory cytokines in various cells. In the present study, we characterized orbital fibroblasts from patients with TAO and then examined the effect of palmitate on the production of pro-inflammatory cytokines and hyaluronic acid (HA) in orbital fibroblasts.</P><P><B>Methods</B></P><P>Orbital fat explants were obtained from patients with TAO undergoing orbital decompression surgery (n=5). The fibroblasts were characterized by antibodies specific for fibroblast markers and Thy-1 (cluster differentiation 90, CD90) by immunostaining and flow cytometry. We then investigated the capability of orbital fibroblasts to secrete cytokines and HA in response to interleukin (IL)-1β using an enzyme-linked immunosorbent assay (ELISA). The effect of palmitate on cytokine and HA production in orbital fibroblasts was examined at the protein level by ELISA and at the mRNA level by quantitative real time RT–PCR. The level of phosphorylation of mitogen-activated protein kinase (MAPK)s, including p38 MAPK (p38), extracellular signal-regulated kinase (ERK), and c-Jun N-terminal kinase (JNK), was measured by immunoblot analysis. We then examined the role of MAPKs on palmitate-induced cytokine production using specific inhibitors to p38, ERK, and JNK, respectively.</P><P><B>Results</B></P><P>The orbital fibroblasts from patients with TAO were Thy-1- positive fibroblasts (>90%) with the ability to secrete IL-6, IL-8, monocyte chemotactic protein-1 (MCP-1), and HA in response to IL-1β. Treatment with palmitate induced significant production of IL-6 and MCP-1, but not IL-8 and HA, in orbital fibroblasts. IL-6 and MCP-1 expression by palmitate were differentially regulated by MAPKs. IL-6 expression was mediated by the p38, ERK, JNK pathways, whereas MCP-1 expression was mediated by ERK and JNK, but not by p38, in palmitate-treated orbital fibroblasts.</P><P><B>Conclusions</B></P><P>We show the possible involvement of palmitate in the promotion of inflammation within orbital tissues. This finding may be helpful for understanding the development of TAO in patients with hyperthyroidism.</P>

        Case Reports : Frontoethmoidal Mucocele Presenting as Progressive Enophthalmos

        In this case report we describe a 27-year-old man who presented with progressive enophthalmos for 5 months without any other associated ocular symptoms such as pain, diplopia, or visual disturbance. Computed tomography showed that his progressive enophthalmos originated from a frontoethmoidal mucocele and this caused destruction of the lamina papyracea and shrinkage of the ethmoidal air cell. Finally the enlarged orbital space caused an inward deviation of the eyeball. Endoscopic marsupialization was successfully performed by an otolaryngologist and did not result in any ophthalmologic sequelae. Although frontoethmoidal sinus mucoceles mostly frequently originates from orbital mucoceles, enophthalmic manifestations are very rare. Enophthalmic conditions are not as responsive to surgical interventions as exophthalmic conditions.

      • A Patient With Combined Corneal and Ingrowing Conjunctival Tissue Tattooing by Micropigmentation Method

        PURPOSE: This report describes the clinical efficacy of combined corneal and conjunctival micropigment tattooing for the treatment of a corneoconjunctival opacity. CASE SUMMARY: Combined corneal and conjunctival micropigment tattooing was performed on a 39-year-old man who had a cosmetic problem due to traumatic corneoconjunctival opacity. After removal of the corneal epithelium, a 30-gauge needle filled with dye was placed into the corneal stroma. The dye was injected into the stroma and conjunctiva. The patient was followed up for 24 months postoperatively. Although some of the dye dissipated, the majority of it remained in place 24 months after surgery. The patient had no specific postoperative complications and was satisfied with the surgical results. We obtained cosmetically good results. CONCLUSIONS: Combined corneal and conjunctival micropigment tattooing can be an effective treatment for patients with a corneoconjunctival opacity.

        The change of corneal refraction and corneal coverture after upper lid blepharoplasty and blepharoptosis repair surgery is still controversial. Dermatochalasis is redundant and lax eyelid skin and muscle, which commonly occurs in older people. Acquired ptosis involves eyelid drooping caused by a thinning of the levator aponeurosis. Overall, upper eyelid surgery is becoming increasingly popular. Patients who have undergone upper eyelid surgery occasionally complain of blurred vision in the operated eye. Several studies have investigated the effects of eyelid surgery on corneal shape. In this study, we reviewed several studies about the corneal curvature and refraction change after upper eyelid blepharoplasty or ptosis surgery and we want to share with many physicians about the outcomes of studies.

      • Complete Nucleotide Sequence Analysis of the Norovirus GII.4 Sydney Variant in South Korea

        <P>Norovirus is the primary cause of acute gastroenteritis in individuals of all ages. In Australia, a new strain of norovirus (GII.4) was identified in March 2012, and this strain has spread rapidly around the world. In August 2012, this new GII.4 strain was identified in patients in South Korea. Therefore, to examine the characteristics of the epidemic norovirus GII.4 2012 variant in South Korea, we conducted KM272334 full-length genomic analysis. The genome of the gg-12-08-04 strain consisted of 7,558 bp and contained three open reading frame (ORF) composites throughout the whole genome: ORF1 (5,100 bp), ORF2 (1,623 bp), and ORF3 (807 bp). Phylogenetic analyses showed that gg-12-08-04 belonged to the GII.4 Sydney 2012 variant, sharing 98.92% nucleotide similarity with this variant strain. According to SimPlot analysis, the gg-12-08-04 strain was a recombinant strain with breakpoint at the ORF1/2 junction between Osaka 2007 and Apeldoorn 2008 strains. This study is the first report of the complete sequence of the GII.4 Sydney 2012 strain in South Korea. Therefore, this may represent the standard sequence of the norovirus GII.4 2012 variant in South Korea and could therefore be useful for the development of norovirus vaccines.</P>

      • Poling effects on the performance of a lead-free piezoelectric nanofiber in a structural health monitoring sensor

        <P><B>Abstract</B></P> <P>This study investigates the poling effects on the performance of a lead-free piezoelectric nanofiber module. The lead-free piezoelectric nanofiber of 0.78Bi<SUB>0.5</SUB>Na<SUB>0.5</SUB>TiO<SUB>3</SUB>-0.22SrTiO<SUB>3</SUB> (BNT-ST) and polyvinylidenefluoride (PVDF) is prepared by an electrospinning, and the lead-free piezoelectric nanofiber modules are fabricated with the bottom and top interdigitated electrodes. The poling effects on the performance of the lead-free piezoelectric nanofiber module were investigated by XRD and temperature-dependence dielectric properties. The possibility of a flexible structural health monitoring sensor application is confirmed by measuring the output voltage of the poled and unpoled lead-free piezoelectric nanofiber modules. The flexible lead-free piezoelectric nanofiber modules are directly attached on the structure with various shapes. We expect that this system will improve the efficiency of sensing. Tests on impact detection and the structural health monitoring based on the poled and unpoled lead-free piezoelectric nanofiber modules were carried out.
