Combination Therapy of Sophoraflavanone B against MRSA: <i>In Vitro</i> Synergy Testing

<P>Sophoraflavanone B (SPF-B), a known prenylated flavonoid, was isolated from the roots of <I>Desmodium caudatum</I>. The aim of this study was to determine the antimicrobial synergism of SPF-B combined with antibiotics against methicillin-resistant <I>Staphylococcus aureus</I> (MRSA). MRSA, a multidrug-resistant pathogen, causes both hospital- and community-acquired infections worldwide. The antimicrobial activity of SPF-B was assessed by the broth microdilution method, checkerboard dilution test, and time-kill curve assay. The MIC of SPF-B for 7 strains of <I>S. aureus</I> ranges from 15.6 to 31.25 <I><I>μ</I></I>g/mL determined. In the checkerboard method, the combinations of SPF-B with antibiotics had a synergistic effect; SPF-B markedly reduced the MICs of the <I><I>β</I></I>-lactam antibiotics: ampicillin (AMP) and oxacillin (OXI); aminoglycosides gentamicin (GET); quinolones ciprofloxacin (CIP) and norfloxacin (NOR) against MRSA. The time-kill curves assay showed that a combined SPF-B and selected antibiotics treatment reduced the bacterial counts below the lowest detectable limit after 24 h. These data suggest that the antibacterial activity of SPF-B against MRSA can be effectively increased through its combination with three groups of antibiotics (<I><I>β</I></I>-lactams, aminoglycosides, and quinolones). Our research can be a valuable and significant source for the development of a new antibacterial drug with low MRSA resistance.</P>
Case Report : Primary Squamous Cell Carcinoma of the Liver Initially Presenting with Pseudoachalasia

Pseudoachalasia secondary to primary squamous cell carcinoma (SCC) of the liver is extremely rare and has not been reported until now. Here, we report a unique case of primary SCC of the liver initially presenting with progressive dysphagia along with short periods of signifi cant weight loss. A 58-yearold man initially presented with progressive dysphagia along with significant weight loss over brief periods of time. The radiographic and manometric fi ndings were consistent with achalasia. Subsequent esophagogastroduodenoscopy revealed a moderately dilated esophagus without evidence of neoplasm or organic obstruction. However, firm resistance was encountered while traversing the esophagogastric junction (EGJ), although no mucosal lesion was identifi ed. Due to the clinical suspicion of the presence of a malignant tumor, endoscopic ultrasonography (EUS) and computed tomography scans of the chest and abdomen were obtained. A huge hepatic mass with irregular margins extending to the EGJ was found. EUS-guided fine-needle aspiration was performed, and the mass was diagnosed as a primary SCC of the liver by immunohistochemical staining. (Gut Liver 2012;6:275-279)

<P><B>Abstract</B></P> <P> <I>Pueraria lobata</I> root (PLR), well known as Kudzu root, has recently become commercially available in Western dietary supplements for menopausal symptoms. The scientific basis for its action has been attributed to the action of phytoestrogens. This study aimed to investigate the estrogen-like activity of isoflavonoids isolated from <I>P. lobata</I> root and their safety with respect to their effect on breast cancer cell proliferation. In an E-screen assay, crude MeOH extract of PLR significantly increased the proliferation of MCF-7 cells in a concentration-dependent manner. Among the four fractions obtained by solvent fractionation of MeOH extract, the <I>n</I>-BuOH fraction had significant estrogen-like activities at all concentrations tested. Phytochemical analysis of the <I>n</I>-BuOH fraction led to the isolation of 10 isoflavones (<B>1</B>–<B>10</B>), among which genistein (<B>10</B>) had significant estrogen-like activities at all concentrations tested. These activities were significantly enhanced by treatment with genistein and 17β-estradiol compared with 17β-estradiol alone, and this effect was mediated by decreased expression of estrogen receptor (ER)α and phospho-ERα in MCF-7 cells. In a cell cytotoxicity assay, genistein (<B>10</B>) exhibited significant cytotoxicity in both ER-positive MCF-7 and ER-negative MDA-MB-231 breast cancer cells. This cytotoxicity was characterized by the induction of apoptotic cells stained with annexin V conjugated with Alexa Fluor 488 and involved activation of mitochondria-independent and -dependent apoptosis pathways in MCF-7 cells. <P><B>Abstract</B></P> <P>Nine new xanthones, cudracuspixanthones I - Q (<B>12</B>–<B>14</B>, <B>25</B>, <B>32</B>–<B>36</B>), and 30 known xanthones (<B>1</B>–<B>11</B>, <B>15</B>–<B>24</B>, <B>26</B>–<B>31</B>, <B>37</B>–<B>39</B>) were isolated from the stems of <I>Cudrania tricuspidata</I> (Moraceae). The structures of isolated compounds were established by using 1D and 2D NMR in combination with HR-TOF-MS. Xanthones from the stems of <I>C. tricuspidata</I> exerted pancreatic lipase inhibitory activity. In addition, cudracuspixanthone P (<B>35</B>), a new xanthone, reduced the fat accumulation in liver cells stimulated with fatty acids. Nitric acid(HNO3) exists as fume in the atmosphere and flows into water in the form of acid rain by wet deposition. The effect of pH on the aqueous environment could result in a fish kill, which occurs when the pH in a water body is lowered rapidly. When released to soil, HNO3 finds its way down toward the ground water table. Upon reaching the ground water table, HNO3 continues to move, in direction of ground water flow. Therefore environmental impact assessment for ground water is needed. In this study, on-site rapid detection methods and official laboratory analytical methods were investigated. Applicable analytical methods were suggested considering environmental media by official analytical method investigations.
