        Frequency, Expected Effects, Obstacles, and Facilitators of Disclosure of Patient Safety Incidents: A Systematic Review

        Objectives: We performed a systematic review to assess and aggregate the available evidence on the frequency, expected effects, obstacles, and facilitators of disclosure of patient safety incidents (DPSI). Methods: We used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines for this systematic review and searched PubMed, Scopus, and the Cochrane Library for English articles published between 1990 and 2014. Two authors independently conducted the title screening and abstract review. Ninety-nine articles were selected for full-text reviews. One author extracted the data and another verified them. Results: There was considerable variation in the reported frequency of DPSI among medical professionals. The main expected effects of DPSI were decreased intention of the general public to file medical lawsuits and punish medical professionals, increased credibility of medical professionals, increased intention of patients to revisit and recommend physicians or hospitals, higher ratings of quality of care, and alleviation of feelings of guilt among medical professionals. The obstacles to DPSI were fear of medical lawsuits and punishment, fear of a damaged professional reputation among colleagues and patients, diminished patient trust, the complexity of the situation, and the absence of a patient safety culture. However, the factors facilitating DPSI included the creation of a safe environment for reporting patient safety incidents, as well as guidelines and education for DPSI. Conclusions: The reported frequency of the experience of the general public with DPSI was somewhat lower than the reported frequency of DPSI among medical professionals. Although we identified various expected effects of DPSI, more empirical evidence from real cases is required.

        Assessing Reliability of Medical Record Reviews for the Detection of Hospital Adverse Events

        Objectives: The purpose of this study was to assess the inter-rater reliability and intra-rater reliability of medical record review for the detection of hospital adverse events. Methods: We conducted two stages retrospective medical records review of a random sample of 96 patients from one acute-care general hospital. The first stage was an explicit patient record review by two nurses to detect the presence of 41 screening criteria (SC). The second stage was an implicit structured review by two physicians to identify the occurrence of adverse events from the positive cases on the SC. The inter-rater reliability of two nurses and that of two physicians were assessed. The intra-rater reliability was also evaluated by using test-retest method at approximately two weeks later. Results: In 84.2% of the patient medical records, the nurses agreed as to the necessity for the second stage review (kappa, 0.68; 95% confidence interval [CI], 0.54 to 0.83). In 93.0% of the patient medical records screened by nurses, the physicians agreed about the absence or presence of adverse events (kappa, 0.71; 95% CI, 0.44 to 0.97). When assessing intra-rater reliability, the kappa indices of two nurses were 0.54 (95% CI, 0.31 to 0.77) and 0.67 (95% CI, 0.47 to 0.87), whereas those of two physicians were 0.87 (95% CI, 0.62 to 1.00) and 0.37 (95% CI, -0.16 to 0.89). Conclusions: In this study, the medical record review for detecting adverse events showed intermediate to good level of inter-rater and intra-rater reliability. Well organized training program for reviewers and clearly defining SC are required to get more reliable results in the hospital adverse event study.

        Perceptions regarding utilization of meteorological information in healthcare in Korea: a qualitative study

        Background: Health forecasting has been used in an attempt to provide timely and tailored meteorological information to patients and healthcare providers so that they might take appropriate actions to mitigate health risks and manage healthcare-related needs. This study examined the in-depth perceptions of healthcare providers and the general public regarding the utilization of meteorological information in the healthcare system in Korea. Methods: The COREQ (Consolidated Criteria for Reporting Qualitative Research) checklist was applied to this study. We conducted three focus group discussions in accordance with semi-structured guidelines developed to deal with various aspects of the utilization of meteorological information in healthcare settings. The verbatim transcriptions and field notes were analyzed according to content analysis. Results: Six physicians, four nurses, three emergency medical technicians, and seven members of the general public participated in the focus group discussions. There were some individual discrepancies among most participants regarding the health effects of climate change. Although several physician participants felt that meteorological information utilization is not a prime concern during patient care, most of the general public participants believed that it should be used in the patient care process. The provision of meteorological information to patients undergoing care is expected to not only improve the effective management of climate-sensitive diseases, but also boost rapport between healthcare providers and patients. Conclusions: More attempts should be made to provide meteorological information to groups vulnerable to climate change, and the effects of this information should be evaluated in terms of effectiveness and inequality. The findings of this study will be helpful in countries and institutions trying to introduce health forecasting services.

        Identifying Adverse Events Using International Classification of Diseases, Tenth Revision Y Codes in Korea: A Cross-sectional Study

        Objectives: The use of administrative data is an affordable alternative to conducting a difficult large-scale medical-record review to estimate the scale of adverse events. We identified adverse events from 2002 to 2013 on the national level in Korea, using International Classification of Diseases, tenth revision (ICD-10) Y codes. Methods: We used data from the National Health Insurance Service-National Sample Cohort (NHIS-NSC). We relied on medical treatment databases to extract information on ICD-10 Y codes from each participant in the NHIS-NSC. We classified adverse events in the ICD-10 Y codes into 6 types: those related to drugs, transfusions, and fluids; those related to vaccines and immunoglobulin; those related to surgery and procedures; those related to infections; those related to devices; and others. Results: Over 12 years, a total of 20 817 adverse events were identified using ICD-10 Y codes, and the estimated total adverse event rate was 0.20%. Between 2002 and 2013, the total number of such events increased by 131.3%, from 1366 in 2002 to 3159 in 2013. The total rate increased by 103.9%, from 0.17% in 2002 to 0.35% in 2013. Events related to drugs, transfusions, and fluids were the most common (19 446, 93.4%), followed by those related to surgery and procedures (1209, 5.8%) and those related to vaccines and immunoglobulin (72, 0.3%). Conclusions: Based on a comparison with the results of other studies, the total adverse event rate in this study was significantly underestimated. Improving coding practices for ICD-10 Y codes is necessary to precisely monitor the scale of adverse events in Korea.

        Perceptions of the General Public About Health-related Quality of Life and the EQ-5D Questionnaire: A Qualitative Study in Korea

        Objectives: The aim of this qualitative study was to investigate how members of the general public in Korea interpret the concept of health, and which dimensions of health are most important to them. We also explored their perceptions of the EuroQoL 5-Dimension (EQ-5D), including the EuroQoL visual analogue scale (EQ-VAS). Methods: We conducted face-to-face, in-depth interviews with 20 individuals from the general population, using a semi-structured interview guide. Content analysis was performed with verbatim transcripts and field notes to identify codes and categorize them according to their similarities and associations. Results: In total, 734 different codes were derived and classified into 4 categories. Participants cited the importance of both the mental and physical aspects of health, although they emphasized that the physical aspects appeared to play a larger role in their conceptualization of health. Participants noted that the EQ-5D has the advantage of being composed of 5 dimensions that are simple and contain both physical and mental areas necessary to describe health. However, some of them mentioned the need to add more dimensions of mental health and social health. Participants showed great satisfaction with the visually well-presented EQ-VAS. However, participants opined that the EQ-VAS scores might not be comparable across respondents because of different ways of responding to the scale. Conclusions: While physical health is a fundamental aspect of health, mental and social aspects are also important to Koreans. The content of the EQ-5D broadly matched the attributes of health considered important by Koreans.

        Disability Weights Measurement for 228 Causes of Disease in the Korean Burden of Disease Study 2012

        <P>Disability weight for each disease plays a key role in combining years lived with disability and years of life lost in disability adjusted life year. For the Korean Burden of Disease 2012 study, we have conducted a re-estimation of disability weights for causes of disease by adapting the methodology of a recent Global Burden of Disease study. Our study was conducted through a self-administered web-based survey using a paired comparison (PC) as the main valuation method. A total of 496 physicians and medical college students who were attending in third or fourth grade of a regular course conducted the survey. We applied a probit regression on the PC data and computed the predicted probabilities of each cause of disease from the coefficient estimates of the probit regression. We used 'being dead (1)' and 'full health (0)' as anchor points to rescale the predicted probability of each cause of disease on a scale of 0 to 1. By this method, disability weights for a total of 228 causes of disease were estimated. There was a fairly high correlation between the disability weights of overlapping causes of disease from this study and a previous South Korean study despite the differences in valuation methods and time periods. In conclusion, we have shown that disability weights can be estimated based on a PC by including 'full health' and 'being dead' as anchor points without resorting to a person trade-off. Through developments in the methodology of disability weights estimation from this study, disability weights can be easily estimated and continuously revised.</P>

        송변전 지리정보 시스템을 이용한 고압 송전선과 거주지 간 이격거리에 대한 인식 평가

        이번 연구에서는 송변전 지리정보 시스템(Transmission and substation Geographic Information System: TGIS)을 이용하여 일반인들이 판단한 고압 송전탑 및 송전선, 변전소와 거주지 간 거리 인식이 실측 거리와 일치하는지 확인해 보았다. 총 725명의 응답자 중 136명이 고압 송전탑 및 송전선, 변전소가 눈에 보이는 거리에 있다고 응답하였고, 이 중 114명이 TGIS로 분석할 수 있는 유효한 주소를 제공하였다. 114명 중 42명(36.8 %)은 실측 거리보다 인식 거리가 짧았으며, 15명 (13.2 %)은 인식 거리보다 실측 거리가 짧았다. 일반인들의 고압 송전탑 및 송전선, 변전소와 거주지 간 거리 예측의 타당도가 높지 않은 점을 바탕으로 했을 때, 이를 극저주파 전자기장 노출량의 대용물로 사용하는 것은 부적절할 것으로 보인다. The purpose of this study was to evaluate the perceived distance from home to high-voltage power lines using a transmission and substation geographic information system(TGIS). Of the 725, 136 respondents reported that high voltage power transmission tower, power lines or substation was within visual field range from home. Among them, 114 respondents reported valid address, which could be used in the analysis of TGIS. Forty two respondents(36.8 %) estimated that the power lines were closer and 15 respondents(13.2 %) estimated that they were farther than they actually were. Our current findings suggest that the reported distance from home to high-voltage power transmission towers, power lines, and substations cannot be used as a proxy for exposure to electromagnetic fields due to the low validity.

        장애가중치 산출 연구에서의 쟁점 검토

        Summary measures of population health (SMPHs) have been used to estimate the burden of diseases. Among various type of SMPHs, disability adjusted life year (DALY) and healthy life expectancy (HALE) have been calculated in the global and national burden of disease studies. In order to calculate DALY and HALE, disability weight is an essential element. Disability weights quantify the level of disability for health states or diseases and have values between 0 (full health) to 1 (being dead). In this study, we reviewed the main disability weights studies and determined their meaning and limitations. Furthermore, we provided the whole process of typical disability weight study and reviewed key issues as follows: health state or disease description development, panel composition, valuation method, validation of disability weight, cross-cultural variability in health state or disease, and so on. The results from this study will be helpful to conduct future disability weight studies for adapting disability weights and developing new methodologies.
