      • 항암화학요법을 받는 암 환자에 대한 Nucare^R의 영양지지효과

        Purpose : The purpose of this study was to evaluate the effect of nutritional support with enteral liquid supplement in cancer patients receving chemotherapy for possible benefit in nutritional, immunologic and golbal function of patients. Patients and Methods : From October 1995 to February 1997, 30 advanced cancer patients receving chemotherapy were divided two roups. The Nycare group, in addition to normal diet, Nucare^R enteral supplement was given for 1week right after chemotherapy for the duration of 2 chemotherapy cycles. Control group received only normal diet without parenteral fluid supplement for 2 chemotherapy cycles. Results : Median ages were 53 end 56 years for Nucare group and Contrl group respectively. Performance scores was less than 2 by ECOG scale. All patients were stage Ⅲ or Ⅳ. The physical parameters, such as weight, arm muscle circumference(AMC) and triceps skin fold(TSF) were decreased in both groups after 2 cycles of anticancer therapy. but it was less severe in Nucare group(p<0.05). Serum transferrin was maintained in mild deficit state in Nucare group, whereas it aggravated form mild to moderate deficit in Control group(p<0.05). Serum albumin level increased in Nucare group without statistical significance. but it decreased from normal to mild deficit in Control group. Serum total protein did not change significantly in Nucare group. but in Control group, serum total protein was decreased from 7.24±0.9 to 6.52±0.5(P<0.05). Total lymphocyte count did not change significantly in both groups. Conclusion : This study shows that the nutritional support with Nucare^R was effective in the prevention of nutritional deficit status in patients receving a nticancer chemotherapy.

      • 단기배양을 통한 말초혈액 CD34 양성세포의 체외증폭

        Background: It is suggested that clinical practice in the areas of bone marrow transplantation and gene therapy might rely on the ex vivo expansion of hematopoietic stem/progenitor cells. However, the condition for ex vivo expansion of hematopoietic progenitor cells is not well established. The authors pursued a series of experiments to define the proper conditions for the expansion of hematopoietic cells in the short-term liquid suspension culture of mobilized peripheral blood CD34+ cells. Methods: 1.0ml cultures were initiated with 9×10^3 PB CD34+ cells, which were isolated from PB mononuclear cells (MNCs) by high-gradient cell sorting, in 12 well plates with the various combinations of hematopoietic growth factors(HGF). The following recombinant human HGFs were used: stem cell factor(SCF) 100ng/ml, granulocyte colony-stimulating factor(G-CSF) 100ng/ml, GM-CSF(granulocyte, macrophage colony-stimulating factor) 100ng/ml, interleukin-1 beta(IL-1B) 1ng/ml, interleukin-3(IL-3) 20ng/ml, interleukin-6 (IL-6) 100ng/ml. At the end of culture, colony-forming cells were evaluated by semisolid clonogenic assay. Results: 1) Using the high-gradient magnetic sorting system, CD34^+ cells were isolated with a yield of 40 3% 2) In 7 day culture of PB CD34^+ cells(9×10^3 cells), nucleated cells expanded mean 10×10^3(range, 9 to 20×10^3) with the addition of SCF alone, 35×10^3(range, 10 to 60×10^3) with SCF plus G-CSF plus GM-CSF, and 130×10^3(range, 40 to 300×10^3) with the combination of SCF, G-CSF, IL-1, IL-3, IL-6, GM-CSF. In 14 day culture, nucleated cells expanded 10×10^3 to 1,860×10^3 with combination of human hematopoietic growth factors. 3) In 10 day culture without medium change of PB CD34^+ cells, CFU-GM numbers expanded 16. 5 fold(range, 7 to 59 fold) with the addition of SCF plus G-CSF plus Il-1 plus IL-3, 31.3 fold(range, 20.5 to 101.1 fold) with the combination of SCF, G-CSF, IL-1, IL-3, GM-CSF. In 14 day culture with or without medium change of PB CD34^+ cells was inferior to 10 day culture for CFU-GM expansion. 4) There was no significant difference for CFU-GM expansion between five growth factors(SCF,G-CSF,IL-1,IL-3,GM-CSF) and six growth factors(five growth factors plus IL-6). Conclusion: The authors could confirm that short-term suspension culture of peripheral blood CD34+ cells could expand hematopoietic progenitor cells. Ten-day culture with medium change of CD34+ cells with the addition of five growth factors, i.e. SCF, G-CSF, IL-1B, IL-3, and GM-CSF, might be the most efficient in this system.

        Support Effects of Containing Catalysts on Methanol Dehydrogenation

        CuO/ZnO, CuO/SiO,sub>2, and CuO/ZrO2 catalysts were prepared for investigating the support effects on methanol dehydrogenation. It was found that the conversion of methanol was proportional to the copper surface area on Cu/ZnO cat alysts and was independent on that on Cu/ZrO2 and Cu/SiO2. The highest copper surface area was obtained with the Cu/ZrO2 (9/1). The unusual deactivation of the Cu/ZnO, which showed the highest selectivity among the catalysts tested, was observed. Pulse reaction with methanol indicated that the lattice oxygen in ZnO could be removed by forming CO2 in the catalytic reaction, supporting that the ZnO reduction was responsible for the severe deactivation of the Cu/ZnO.

        Study on CO Adsorption on in-situ Brass Formed Cu/ZnO

        The isotherms of CO adsorption on the Cu/ZnO sample treated with with hydrogen and methanol are investigated. The heats of adsorption of CO on the Cu/ZnO treated with hydrogen at 723K for 3 h are in the range from 25.7 kJ/mol at $\theta=0.8$ to 59.8 at $\theta=0.1$, while those on the Cu/ZnO sample treated with methanol at 523 K for 3 h are in the range 30.3 kJ/mol at $\theta=0.8$ to 99.8kJ/mol at $\theta=0.1.$ The Cu/ZnO samples treated with hydrogen do. The heats of adsorption at q=0.1 increase with the methanol treatment time within 30 min and leveled off afterwards.

      • Effect of Physical Training in a Hot Environment on Thermoregulatory Responses

        In order to investigate the differences on thermo- regulatory responses under different environmental temperatures and effect of heat acclimatization training on theses responses, six high school elite distance runners were tested before and after a period of 4-week training session at 10-11 ˚C(Environment 1), 17-18 ˚C(Environment 2), and 24-25˚C (Environment 3). Relative humidity was kept constant at 70% for all three conditions. All subjects were trained on a bicycle ergometer at the intensity of 40% V'O_2 max for 1 hour, 4 times a week at 35˚C(+2) with 80% of relative humidity. The local and total sweat volume, ions in sweat and blood, energy efficiency were measured before and after training. All the variables were also measured before, during, and after exercise. Local sweat volume decreased at both Environment 1 and Environment 2 while it increased at Environment 3 after training. There was a tendency that the phenomenon of hidromeiosis was higher after acclimatization training than that of pre-training. Total evaporation volume significantly (p<0.01) increased as environmental temperature increased pre- and post-training. Density of Na^+, CI^-, and K^+ in sweat remained unchanged. Na^+ and CI^- density in sweat significantly decreased after training. Pre-exercise Hematocrit was higher than that of post-exercise and increased after training(p<0.05). Energy efficiency significantly increased before training but it did not differ after training when the environmental temperature increased.

        Korean Asthma Guideline 2014: Summary of Major Updates to the Korean Asthma Guideline 2014

        Asthma is a prevalent and serious health problem in Korea. Recently, the Korean Asthma Guideline has been updated by The Korean Academy of Tuberculosis and Respiratory Diseases (KATRD) in an effort to improve the clinical management of asthma. This guideline focuses on adult patients with asthma and aims to deliver up to date scientific evidence and recommendations to general physicians for the management of asthma. For this purpose, this guideline was updated following systematic review and meta-analysis of recent studies and adapting some points of international guidelines (Global Initiative for Asthma [GINA] report 2014, National Asthma Education and Prevention Program [NAEPP] 2007, British Thoracic Society [BTS/SIGN] asthma guideline 2012, and Canadian asthma guideline 2012). Updated issues include recommendations derived using the population, intervention, comparison, and outcomes (PICO) model, which produced 20 clinical questions on the management of asthma. It also covers a new definition of asthma, the importance of confirming various airflow limitations with spirometry, the epidemiology and the diagnostic flow of asthma in Korea, the importance and evidence for inhaled corticosteroids (ICS) and ICS/formoterol as a single maintenance and acute therapy in the stepwise management of asthma, assessment of severity of asthma and management of exacerbation, and an action plan to cope with exacerbation. This guideline includes clinical assessments, and treatment of asthma.chronic obstructive pulmonary disease overlap syndrome, management of asthma in specific conditions including severe asthma, elderly asthma, cough variant asthma, exercise-induced bronchial contraction, etc. The revised Korean Asthma Guideline is expected to be a useful resource in the management of asthma.

      • Systemic Corticosteroid Use and Cardiovascular Events in Adults Hospitalized for Pneumonia

        Purpose There are limited data on cardiovascular risk according to adjunctive corticosteroid use in patients with pneumonia. We aimed to assess the associations between systemic corticosteroid use and the occurrence of major adverse cardiovascular events (MACE) in patients hospitalized for pneumonia. Method Among subjects enrolled via the surveillance for severe acute respiratory infection from July 2016 to January 2017, the clinical course of patients with pneumonia till December 2019 was retrospectively investigated. We evaluated the occurrence of in-hospital and after-discharge MACE according to steroid use during hospitalization. Result Of 424 patients hospitalized for pneumonia, 118 patients (28.8%) received administration of systemic corticosteroids during hospitalization. The most common cause of steroid use was acute exacerbation of chronic lung disease (75.4%). Systemic steroid use was significantly associated with increased risk of in-hospital MACE (aOR, 46.2; 95% CI 2.4-881.5; P=0.011), but not with after-discharge and overall MACE. Risk of in-hospital MACE was significantly higher in patients with longer duration of steroid use, higher medication possession ratio of steroid, higher total cumulative dose of steroid and delayed steroid administration from admission. Conclusion In patients hospitalized for pneumonia, systemic corticosteroid use is associated with increased risk of in-hospital MACE.
