Registration of Phalange Region from CR Images Based on Genetic Algorithm

In Japan, the number of patients with osteoporosis and rheumatoid arthritis is increasing. Image diagnosis using CR images is effective for osteoporosis and rheumatoid arthritis. Development of a CAD system is important for reducing burdens on doctors. In this paper, we propose an automatic registration algorithm in the CAD system. In the proposed method, the genetic algorithm is used to register bone regions between identical parts of the same subject with different time series. In the experiment, the proposed method is applied to 176 bone area, and 98.14 % of TPR, 1.85 % of FPR are obtained respectively. Even when the area difference is used as the fitness of the genetic algorithm, it has cross-correlation and positioning accuracy equivalent to mutual information.
Automatic Segmentation Method of Phalange Regions Based on Residual U-Net and MSGVF Snakes

Bone diseases include rheumatoid arthritis and osteoporosis. Although visual screening using computed radiography (CR) images is an effective method for diagnosing osteoporosis, there are some similar diseases that exhibit low bone mass status. To this end, we aim to develop a computer-aided diagnostic (CAD) system to support the automatic diagnosis of osteoporosis from CR images. In this paper, we use convolutional neural network (CNN) and multiscale gradient vector flow snakes (MSGVF Snakes) algorithms to segment each finger bone regions from the CR image. The proposed method is applied to 15 cases, 92.95 [%] of the true positive rates, 2.21 [%] of the false positive rates, 7.05 [%] of the false negative rates are obtained respectively.

Background: This study aimed to identify reduced home care use among older people and its impact on user numbers and service provider revenues during the coronavirus disease 2019 (COVID-19) pandemic. Methods: We conducted this secondary analysis of cross-sectional data from long-term care providers in Japan and estimated the probability of client cancellations and service contraction for institutional and home care services adjusted for the service type, area, size, infection occurrence, and staff employment impacts. We then estimated the rate of change in user numbers and revenue related to reduced usage. Results: Seventy-two percent of home care providers experienced client cancellations and 42.1% experienced service contraction, both of which were more prevalent in home care than in institutional care (adjusted odds ratio [AOR]=11.09 and 1.60). Home-visit (AOR=0.70) and short-term stay (AOR=0.38) services were less likely to experience client cancellations compared with adult day services. Service contraction was less likely in home-visit services (AOR=0.60) but were more likely in short-stay services (AOR=1.49) compared to adult day services. The estimated reductions in user numbers and revenue related to service contraction for adult day services were 9.1% and 7.1%, respectively. Home-visiting services decreased by an estimated 3.4% owing to service contraction. Conclusion: The secondary effects of the COVID-19 pandemic include reduced use of home care services, especially adult day services that include social contact. These secondary effects of the COVID-19 pandemic may cause functional deterioration in non-infected clients and financially impact service providers.
Epidermal Growth Factor Receptor Mutations in Japanese Men with Lung Adenocarcinomas

Background: Epidermal growth factor receptor (EGFR) mutations play a vital role in the prognosis of patients with lung adenocarcinoma. Such somatic mutations are more common in women who are non-smokers with adenocarcinoma and are of Asian origin. However, to our knowledge, there are few studies that have focused on men. Materials and Methods: One hundred and eighty-four consecutive patients (90 men and 94 women) of resected lung adenocarcinoma were studied retrospectively. Results: EGFR mutations were positive in 48.9% and negative (wild type) in 51.1%. Overall mutation was significant in women (66.0% vs. 32.2%) compared with men (p<0.001). For overall patients, EGFR mutation status was associated with gender, pStage, pT status, lepidic dominant histologic subtype, pure or mixed ground-glass nodule type on computed tomography and smoking status. However, in men, EGFR mutation status was only associated with lepidic dominant histologic subtype and not the other variables. Interestingly, the Brinkman index of men with mutant EGFR also did not differ from that for the wild type ($680.0{\pm}619.3$ vs. $813.1{\pm}552.1$ p=0.1077). Conclusions: The clinical characteristics of men with lung adenocarcinoma related to EGFR mutation are not always similar to that of overall patients. Especially we failed to find the relationship between EGFR mutations and smoking status in men.

Background: The national health survey in 2011 estimated nearly 20 million Japanese adults are being or possibly having diabetes mellitus. Treatment for type 2 diabetes mellitus requires multiple oral antidiabetic agents (OADs) and/or insulin in the long run. In 2009, the . rst DPP-4 inhibitor sitagliptin launched, and currently seven DPP-4 inhibitors are available in Japan. Aim: We conducted this survey in order to reveal changes in pharmacological treatment pattern, especially before and after the introduction of DPP-4 inhibitors, and to delineate its ef. cacy on glycemic control. Methods: We reviewed the electric medical record from March 1st to May 31st in 2008, that is one year before the introduction of DPP-4 inhibitor, and the same period in 2013. As for pharmacological agents, we focused on six classes of OAD, Insulin and GLP-1 receptor agonists (GLP-1 RAs). Primary outcomes were the number and proportion of patient using each drug. Secondary outcome was mean HbA<sub>1c</sub>. Statistical analyses were performed by JMP5.0.1J using SAS. Results: Total number of patient increased by 40 % in 5 years. While SU and BG users increased moderately and insulin users tended to decrease, DPP-4 inhibitors use reached to 65% of the patients in 2013 (Table). TZD use remained the same despite bladder or other cancer issue, mainly because our mean dose was less than 15mg/d (median 7.5mg/d). Glinides and AGI use declined rapidly probably due to the restriction on DPP-4 inhibitors when prescribed. HbA<sub>1c</sub> improved signi. cantly (P<0.05) compared to year 2008. Conclusions: The introduction of DPP-4 inhibitor made treatment pattern changed substantially, and glycemic control improved. Further study is needed to evaluate cost-effectiveness of multiple drug combinations.
Supporting Appropriate Communication Media Selection Through Context-Awareness

Communicating with someone via the right communication media seems to be difficult nowadays as people use various mode of communication media and senders do not have any knowledge about receivers’ current communication media, especially in ubiquitous environment. Thus, we propose the method how the most appropriate media to reach the receiver can be selected based on the effective measures. In the paper, a preliminary evaluation of the proposed method is also described to show its effectiveness. We are developing a research prototype system to support the sender with the information of the best reachable media to the receiver.
