The Zanthoxylum schinifolium seed oil modulates immune function under the biological safety level

Zanthoxylum schinifolium (Z. schinifolium) and the seed oil of Z. schinifolium has been widely used as culinary applications and traditional medicine for epigastric pain in Korea, China, and several Asia nations and the components or extracts of Z. schinifolium have been reported to have pharmaceutical effects such as anti-platelet aggregation, inhibitory activities against monoamine oxidase, antioxidant and anticancer activities, and anti-inflammatory activity. In this study we got the results that 200 mg/kg/day seed oil of Z. schinifolium oral administration for 13 weeks enhances most immune-related cells and is biologically safe. However as B cell population was increased by Z. schinifolium seed oil administration but B cell function was not changed by that, ultimately we deduced that Z. schinifolium seed oil doesn't have the modulation effect about B cell. To evaluate the biological safety of repeated Z. schinifolium seed oil administration the index of biological safety such as CBC, hematological enzymes, and especially histopathological morphologies was measured but no changes related with Z. schinifolium seed oil administration had been shown. Conclusively Z. schinifolium seed oil is one of the safe candidates to modulate immunity.
A case of incontinentia pigmenti in a male neonate

Incontinentia pigmenti (IP) is a rare X-linked dominant genodermatosis that is typified by distinctive cutaneous findings and often by abnormalities of teeth, hair, nails, eyes, musculoskeletal system, and central nervous system. IP is generally lethal in male fetuses, whereas heterozygous females survive owing to functional mosaicism. Two potential mechanisms have been proposed to explain the survival of males with IP carrying the inhibitor of nuclear factor kappa B kinase subunit gamma (IKBKG) mutation. One is abnormal karyotype (i.e. 47, XXY Klinefelter syndrome) and the other is somatic mosaicism, a concept that is increasingly being recognized as an explanation for skin conditions. A 4-day-old male neonate had a 2-day history of vesicles on pubis, right leg and axilla. Physical examination revealed linearly arranged vesicles on an erythematous base on his pubic area, right thigh, calf, and right axilla. There was no family history of IP. Ophthalmologic, neurologic and musculoskeletal examinations showed normal findings. Histological examination revealed eosinophils within intraepidermal multiloculated vesicles. Chromosomal analysis from peripheral blood leukocytes showed a normal male karyotype . Based on these findings, he was diagnosed with incontinentia pigmenti. Herein, we report a case of incontinentia pigmenti in male patient, which has rarely been reported in the dermatologic literatures.
A case of multiple eruptive milia mimicking syringoma

Milia are common benign keratinous cysts that occur most commonly on the face, particularly the eyelids and the cheeks. However, multiple eruptive milia (MEM) is a rare skin condition characterized by sudden onset of multiple milia occurring mainly on the head, neck, and trunk. The lesions generally are asymptomatic and may arise spontaneously or secondary to a number of processes. Primary milia typically present as single or multiple papules that are randomly distributed on the face, predominantly around the eyes, forehead, and cheeks; head; and/or trunk of children and young adults. Secondary milia usually arise following trauma; prolonged use of topical steroids; or clearance of inflammatory skin diseases. A 45-year-old woman presented with multiple yellowish and skin colored papules on forehead, nose, and bilateral inner periorbital areas of 3-year duration. She reported that the papules had appeared within 3 weeks. Cutaneous examination revealed numerous 1- to 3-mm and flesh-colored papules on the nose, forehead, and bilateral periorbital areas. Biopsy specimen from lesion on left periorbital area showed multiple milial cysts located in the superficial dermis. No therapeutic interventions had been attempted prior to presentation. Finally, she was diagnosed with primary MEM. Herein, we report a case of multiple eruptive milia, which has rarely been reported in the dermatologic literatures.

Background: This study aimed to evaluate the effect of iodine restriction on thyroid function in subclinical hypothyroidism (SCH) patients in an iodine-replete area. Subjects & Methods: The study consisted of 146 patients who were diagnosed with SCH. Urinary iodine concentration (UIC) was measured in 82 patients; of these, 20 patients with UIC <300 μg/L were excluded, and 62 patients with UIC =300 μg/L were educated about the restriction of iodine-rich foods. Following the first follow-up visit, these patients were divided into 2 groups based on the UIC level: group A (well controlled iodine intake, UIC <300 μg/L, n=40) and group B (poorly controlled iodine intake, UIC =300 μg/L, n=22). The remaining 64 patients did not restrict iodine rich foods (group C). The 82 patients with measured UICs were reevaluated every 3-6 months. Thyroid function and UIC were measured at each visit. The correlation between serum TSH level and UIC was determined for the 82 patients in whom UIC was measured. Results: Following 3-6 months of iodine restriction, the serum TSH levels significantly decreased in group A (9.0 mU/L to 4.7 mU/L, p <0.01). In addition, the serum free T4 levels in group A significantly increased (1.11 ± 0.23 ng/dL to 1.18 ± 0.17 ng/dL, p <0.05). However, there were no significant changes in serum TSH or free T4 levels in group B and C. Serum TSH levels were significantly correlated with UIC (r=0.33, p <0.01). Conclusion: Iodine restriction may decrease serum TSH levels in SCH patients, and serum TSH levels are strongly correlated with UIC. Therefore, restriction of iodine intake could be a primary treatment option in SCH patients in an iodine-replete area.

In this paper, natural convection of a magnetic fluids(W-40) in a half circular pipe enclosure are investigated by numerical and experimental method. One side wall is kept at a constant temperature(25℃), and the opposite side wall is also kept at a constant temperature(20℃). Under above conditions, various magnitudes of the magnetic fields were applied up. Theoretical study through the governing equation derived by Siliomis is carried out with numerical analysis by the GSMAC Method. And the thermo-sensitive liquid crystal film(R20C5A) is utilized in order to visualize wall-temperature distributions as an experimental method. This study has resulted in the following fact that the natural convection of a magnetic fluids are controlled by the direction and intensity of the magnetic fields.

Background: Verruca vulgaris (VV) is a common cutaneous viral infectious disease caused by human papilloma virus (HPV) infection. Main treatment modalities are cryotherapy, immunotherapy and laser therapy, but there is no definite criteria for evaluating clinical improvement. Objectives: This study was designed to evaluate the new clinical treatment response criteria of VV called ‘Hardening sign’, which divided treatment courses into 4 stages through clinical observation and palpation. Methods: We conducted a retrospective analysis of patients clinically diagnosed with VV in Hanyang university Guri hospital from January 2016 to January 2017. We divided patient into four groups according to clinical evaluation of degree of rigidity by inspection and palpation. In case of multiple lesions, it was based on the largest lesion. Results: 55 patients with VV (28 male and 27 female) were identified. Mean age at diagnosis was 17.6 and duration of treatment was 31.3 weeks. Mean number of hospital visit was 18.1. In ANOVA, there was significantly shorter duration of treatment in patient with higher stage of hardening sign. In post-hoc test, significant difference was identified between hardening sign stage 4 and hardening sign stage 1. Conclusion: The hardening sign is a new and simple criteria to determine the treatment response and prognosis.
Systemic sclerosis developing after cast application

Systemic sclerosis (SSc) is an autoimmunological multisystemic disease, characterized by sclerotic and fibrotic changes of the skin and internal organs. Several triggering factors may contribute to the development of SSc such as viral infection, silica exposure or radiation therapy. In addition, there are rarely reported that scleroderma had developed after physical trauma, surgery, constant skin friction and limb immobilization. Herein, we report a case of SSc developing after fracture and cast application. A 70-year-old female presented with severe induration on her right hand and wrist for 8 months. She complaint that it had begun just after cast removal due to right distal radius fracture. Anti-centromere antibody was detected from the laboratory examination and pulmonary hypertension was revealed from echocardiography. Punch biopsy specimen showed homogenized and thickened collagen bundles in the dermis with compressed skin appendages. From clinicopathological and laboratory findings, she was diagnosed with limited cutaneous SSc. In our case, mechanical injury from the fracture and skin friction with joint immobilization from the cast application might be triggering events to development of SSc.
