        Novel Natural Fill Telemetric Pressure Flow Study of Discomfort and Bladder Outlet Obstruction

        <P><B>Purpose</B></P><P>We evaluated the accuracy of natural fill telemetric pressure flow study performed in a private room, and assessed patient discomfort and experience after the procedure compared to those of standard pressure flow study.</P><P><B>Materials and Methods</B></P><P>In 58 patients with lower urinary tract symptoms/benign prostatic hyperplasia free uroflowmetry, and natural fill telemetric and standard pressure flow studies were prospectively performed. Immediately after each step patients were asked to rate the experience in terms of pain, embarrassment, bother, boredom and repeat testing. Subjective items and objective urodynamic parameters were compared among the 3 tests.</P><P><B>Results</B></P><P>Maximum urine flow on natural fill telemetric pressure flow study was not different from that on free uroflowmetry. In contrast, maximum flow, detrusor pressure at maximum flow and bladder contractility index on the standard pressure flow study were significantly lower than on the natural fill pressure flow study. However, the bladder outlet obstruction index was not significantly different between the 2 studies, and the bladder outlet obstruction and bladder contractility indexes correlated well. There were also no differences in bother and embarrassment scores. However, natural fill telemetric pressure flow study was superior in terms of pain and boredom scores. Patients were more willing to undergo repeat natural fill telemetric pressure flow study than standard pressure flow study. Time to complete the test was significantly shorter for the natural fill study than for the standard study.</P><P><B>Conclusions</B></P><P>Flow rate differences between pressure flow studies and free uroflowmetry are not due to mechanical obstruction by the catheter but to other factors, such as the bladder filling method. Objective parameters on the natural fill telemetric pressure flow study correlated with standard pressure flow study findings. The natural fill telemetric pressure flow study shortened the time needed for the test and may decrease the discomfort of the standard pressure flow study.</P>

        헬스케어 스마트홈 운동프로그램이 비만 여성 노인의 대사증후군 위험요인에 미치는 효과

        본 연구는 유헬스 기반의 스마트홈 기술에 운동서비스의 개념을 더한 헬스케어 스마트홈 운동프로그램이 비만 여성 노인의 대사증후군 위험요인에 미치는 효과를 알아보는 데 목적이 있다. 연구대상자는 서울시 K구의 영구임대주택단지에 거주하는 체지방률 30%이상의 비만 여성 노인 중 헬스케어 스마트홈 서비스에 참여를 희망하는 자로 운동군 21명(77.19±6.94세), 통제군 13명(74.08±6.73세)으로 분류하였다. 헬스케어 스마트홈 운동프로그램은 유산소운동과 저항운동의 복합운동 형태로 12주 동안 하루 30~50분, 주당 3회로 실시하였고, 운동전후 대사증후군 위험요인을 측정하였다. 검사결과에 대한 운동효과 분석을 위해 각 변인별로 이원 반복측정 분산분석(repeated measures two-way ANOVA)을 실시하였으며 통계학적 유의수준은 α=.05로 하였다. 12주간의 헬스케어 스마트홈 운동프로그램 실시 후, 대사증후군 위험요인 중 허리둘레(p=.034)에서 사전사후 시기 및 집단 간 상호작용 효과가 유의하게 나타났다. 결론적으로 12주간의 헬스케어 스마트홈 운동프로그램은 비만 여성 노인의 대사증후군 지표인 복부비만의 개선에 효과적이었음을 확인 할 수 있었다. This study set out to investigate effects of healthcare smart-home exercise program, which adds the concept of exercise service to the healthcare-based smart home technology, on the metabolic syndrome risk factors of obese elderly women. The subjects include obese elderly women that were living in a permanent rental housing complex in K-gu, Seoul, had 30% body fat or more, and wished to participate in the healthcare smart home service. The subjects were divided into the exercise group of 21 persons(age: 77.19±6.94) and the control group of 13 persons(age: 74.08±6.73). The healthcare smart home exercise program was the combined exercise of aerobic exercise and resistance exercise and administered for 30~50 minutes per day three times a week for 12 weeks, and the metabolic syndrome risk factors of the subjects before and after exercise were measured. The author performed repeated measures two-way ANOVA for each variable at the statistical significance level of α=.05 to analyze the effects of exercise according to the test results. After 12 weeks of the healthcare smart home exercise program, there were significant interactions in waist circumference(p=.034), one of the metabolic syndrome risk factors, before and after the program and between the groups. In conclusion, the 12-week healthcare smart home exercise program was effective for improving abdominal obesity, one of the metabolic syndrome indexes, among obese elderly women.

        Preliminary study on application of augmented reality visualization in robotic thyroid surgery

        Purpose: Increased robotic surgery is attended by increased reports of complications, largely due to limited operative view and lack of tactile sense. These kinds of obstacles, which seldom occur in open surgery, are challenging for beginner surgeons. To enhance robotic surgery safety, we created an augmented reality (AR) model of the organs around the thyroid glands, and tested the AR model applicability in robotic thyroidectomy. Methods: We created AR images of the thyroid gland, common carotid arteries, trachea, and esophagus using preoperative CT images of a thyroid carcinoma patient. For a preliminary test, we overlaid the AR images on a 3-dimensional printed model at five different angles and evaluated its accuracy using Dice similarity coefficient. We then overlaid the AR images on the real-time operative images during robotic thyroidectomy. Results: The Dice similarity coefficients ranged from 0.984 to 0.9908, and the mean of the five different angles was 0.987. During the entire process of robotic thyroidectomy, the AR images were successfully overlaid on the real-time operative images using manual registration. Conclusion: We successfully demonstrated the use of AR on the operative field during robotic thyroidectomy. Although there are currently limitations, the use of AR in robotic surgery will become more practical as the technology advances and may contribute to the enhancement of surgical safety.

      • Development of an aperture-type radiation regulator for shielding against secondary radiation from x-ray tubes and collimators in computed tomography

        <P>During computed tomography (CT) scans, radiation scatters in all directions, increasing radiation exposure. In this study, an aperture-type radiation regulator was developed to provide shielding against secondary radiation from the x-ray tube and collimator in CT. To evaluate the usefulness of the developed aperture-type radiation regulator, (1) spatial dose distribution within the CT room was measured, (2) dose intensity at 1 m from the isocenter was compared, (3) absorbed dose in the nearby organs was evaluated using a human equivalent phantom, and (4) noise, CNR, and SNR were compared for assessment of image quality. The results showed that the developed aperture-type radiation regulator reduced the intensity of secondary radiation by approximately 25% in front of the gantry and 15% to the rear of the gantry. The maximum dose distribution on 10 <I>μ</I>Gy was reduced by approximately 18% in front of the gantry and 12% in the rear. In addition, when the neck and head were scanned, the absorbed dose in the chest decreased by 25% and 40%, respectively, and noise was reduced by 3.3%–4.5% for different phantoms. Evaluation of abdominal CT images showed 18% noise reduction, with 27% and 28% increases in the signal-to-noise and contrast-to-noise ratios, respectively. These results confirmed that the proposed aperture-type radiation regulator can reduce radiation exposure without affecting primary radiation that creates medical images. The results also confirmed that the radiation regulator effectively improves the quality of medical images.</P>

        코어운동장비 기반의 요부강화 운동프로그램이 만성요통 노인의 요부근력에 미치는 효과

        This study was conducted to investigate the effect of back strengthening exercise program using core exercise equipment on the back muscles strength in elderly patients with chronic lower back pain. The subjects of this study were 40 elderly people with chronic back pain who are 70 years or older using Seniors Welfare Centers located in Seoul city and suffer from back and lower limb pains that interrupt daily activities. The change in peak torque of adduction and abduction of back extensor, hip and pelvic was measured after a 12-weeks back strengthening exercise program using core exercise equipment. The back strength showed statistically significant difference by measurement point (105°, 120°), between groups (105°, 120°, 135°, 150°, 165°)and by interaction effect (105°, 120°) at each angle (p<.05). The muscle strength of hip also showed significant difference based on interaction effect (p<.05) and the change of strength during abduction and adduction showed significant interaction effect between the measurement points and groups (p<.05). This study applied the exercise program to the range of motion of pelvic and hip, which is greater than that of those in the back. It extended back exercise treatment to the pelvic and hip muscles, which showed positive effect on chronic lower back pain. As such, the effect of back strengthening exercise program using core exercise equipment would be used to wider area of rehabilitation program of chronic lower back pain with expected positive outcome.

        소아 환자의 치수 혈류 평가를 위한 광용적맥파 파형 분석

        The purpose of this study was to analyze photoplethysmographic waveforms from pulse oximeter using raw data of red and infrared light and investigate the reference values of parameters (Height, Width50, Maximum slope, Minimum slope, Area) for evaluating pulpal blood flow in maxillary central incisors with normal pulp vitality in children. The study was performed in 30 pediatric patients, aged 7-16 years old, using pulse oximeter (MEKICS Co., Ltd, Korea) combined with a custom-made sensor. The raw data was obtained and recorded by custom-made software and analyzed by LabChart (v.7.3, ADInstruments, Germany) offline. In this study, we analyzed photoplethysmographic waveforms from pulse oximeter applied to maxillary central incisor for assessment of pulpal blood flow and suggested several reference values of young permanent maxillary central incisor with normal pulp. On average, the waveform of red light was higher, stiffer and wider than that of infrared light. Future studies about reference values for other normal teeth and the teeth with impaired pulp vitality are needed. 본 연구는 맥박산소측정기를 이용하여 광용적맥파 파형의 원데이터를 기록하고 분석함으로써 치수 생활력 측정에 이 원데이터 분석의 적용가능성을 살펴보고 정상 치수 생활력을 갖는 소아환자의 상악 중절치에 대한 광용적맥파 파형의 파라미터에대한 기초 자료를 마련할 목적으로 시행되었다. 상악 중절치에 대해 정상 치수 생활력을 갖고 전신질환이 없는 30명의 환아(남 16명, 여 14명, 평균연령 11.4세)를 연구대상으로 선정하였다. 맞춤형 센서와 맥박산소측정기를 이용하여 측정하였으며 측정결과는 적색 파장과 적외선 파장의 원데이터 형태로 기록되었고 LabChart(v.7.3, ADInstruments, Germany)를 이용하여 분석하였다. 본 연구를 통해 소아에서 정상 치수생활력을 갖는 상악 중절치에 대한 광용적맥파 파형의 파라미터의 기준값을 제시할 수있었으며 소아의 치수혈류측정에 이러한 광용적맥파 파형 분석이 활용될 수 있음을 확인하였다.

        Forearm Muscle Activity Patterns during Open Thyroidectomy Using 8-Channel Surface Electromyography

        Purpose: Quantitative analysis of forearm muscle movements during actual surgery has not yet been performed. The aim of this study was to evaluate forearm muscle activity patterns during open thyroid lobectomy using 8-channel surface electromyography, specifically Myo gesture-control arm bands. Methods: We developed a raw-signal recording program based on output from a Myo device. Eight Myo electrodes were allocated to the following adjacent forearm muscles: pronator teres (PT), brachioradialis (BR), extensor carpi radialis & brevis (ECR), extensor digitorum (ED), extensor carpi ulnaris (ECU), flexor carpi ulnaris (FCU), palmaris longus (PL), and flexor carpi radialis (FCR). EMG signals were measured from the time of skin incision to the end of thyroid lobectomy. Root mean square (RMS) value was used to indicate muscle activity during surgery. Results: Total of 10 thyroid lobectomies (5 right and 5 left lobectomies) were performed with Myo devices. The right arm muscles had higher RMS values than the left arm muscles (mean RMS 632.427 versus 367.170, P<0.001). In the right arm, PT, BR, ECR, FCU, PL and FCR showed higher mean RMS values, which means that the right arm made a greater variety of movements than the left arm. Only ED and ECU showed higher mean RMS values in the left arm, which means that the left arm muscles acted to counteract the motion of the right arm. Conclusion: We are the first to quantitatively evaluate forearm muscle activity patterns during thyroid lobectomy. More detailed research will be needed to apply our results in other surgical fields.
