        Skin-sparing mastectomy with immediate nipple reconstruction during autologous latissimus dorsi breast reconstruction: A review of patient satisfaction

        Background Nipple-areolar complex (NAC) reconstruction following curative mastectomy is traditionally performed as a second-stage procedure several months after initial breast reconstruction. The recent literature has documented the increasing popularity of immediate nipple reconstruction carried out simultaneously during autologous reconstruction. The aim of this study was to evaluate the surgical outcomes and patient satisfaction with immediate breast and nipple reconstruction performed in a single stage after skin-sparing mastectomy. Methods All patients who underwent a skin-sparing mastectomy with immediate latissimus dorsi flap breast and NAC reconstruction as a single-stage procedure from 2007 to 2015 were included. Patient demographics, oncologic details, and surgical outcomes were recorded. The BREAST-Q questionnaire was administered to patients to assess the impact and effectiveness of this reconstructive strategy. Results During the study period, 34 breast and NAC reconstructions in 29 patients were performed at Cork University Hospital. The majority of our patient cohort were non-smokers (93.1%) and did not receive adjuvant radiotherapy. Postoperative complications were infrequent, with no cases of partial necrosis or complete loss of the nipple. The response rate to the BREAST-Q was 62% (n=18). Patients reported high levels of satisfaction with the reconstructed breast ($62{\pm}4$), nipple reconstruction ($61{\pm}4.8$), overall outcome ($74.3{\pm}5$), and psychosocial well-being ($77.7{\pm}3.2$). Conclusions Skin-sparing mastectomy with immediate nipple reconstruction during autologous latissimus dorsi reconstruction was demonstrated to be a safe and aesthetically reliable procedure in our cohort, yielding high levels of psychological and physical well-being. A single-stage procedure promotes psychosocial well-being involving issues that are intrinsically linked with breast cancer surgery.

      • <i>In situ</i>laser-based resonant ultrasound measurements of microstructure mediated mechanical property evolution

        <P>In situ laser-based resonant ultrasound spectroscopy is used to characterize the development of a recrystallized microstructure in a high purity copper sample. The modal shapes, used for mode identification, of several resonant modes are determined before and after annealing by raster scanning the laser interferometric probe. This information is used to isolate the motion of individual modes during high temperature annealing. The evolution of a particular mode during annealing is examined in detail. During recrystallization, the center frequency of this mode shifts by approximately 20% of the original value. Using electron backscatter data it is shown that the majority of this shift is due to changes in the polycrystal average elastic stiffness tensor, driven by changes in texture, and that changes in dislocation density and pinning length are secondary influences.</P>

        Linkages among US Interest Rates and East Asian Purchases of US Treasury Securities

        Recent increases in East Asian purchases of US treasury securities has led to growing concern over its impact on the US economy, particularly on the US long-term and shortterm interest rates. The vector error-correction model results revealed the presence of long-run causal relations among the federal funds rate, the 10-year Treasury rate and the East Asian demand in addition to a unidirectional short-run causal relation from the 10-year rate to the Federal funds rate. The variance decomposition and impulse response findings indicated that the Asian demand for US assets has a limited but negative impact on the 10-year US Treasury interest rate. The transmission of the East Asian demand shock on both interest rates was almost immediate. The Federal funds rate is found to have a significant negative impact on the long-term rate.

        Coherent Control of Optically Generated and Detected Picosecond Surface Acoustic Phonons

        Coherent control of electronic and phononic excitations in solids, as well as chemical and biological systems, on ultrafast time scales is of current research interest. In semiconductors, coherent control of phonons has been demonstrated for acoustic and optical phonons generated in superlattice structures. The bandwidth of these approaches is typically fully utilized by employing a 1-D geometry in which the laser spot size is much larger than the superlattice repeat length. In this article, we demonstrate coherent control of optically generated picosecond surface acoustic phonons using sub-optical wavelength absorption gratings. The generation and detection characteristics of two material systems are investigated (aluminum absorption gratings on Si and GaAs substrates). Constructive and complete destructive interference conditions are demonstrated using two pump pulses derived from a single Michelson interferometer.

        Lateral-looking Time-resolved Thermal Wave Microscopy

        Time-resolved thermal wave microscopy was used to measure lateral thermal transport in a thin metallic film on an insulating substrate. The basis of this approach is to decompose the reflectivity signal into a component that varies with delay time and a steady state component that varies with pump modulation frequency. The transient component is a summation of thermal waves at integral multiples of the pulse repetition frequency (76 MHz). The steady state component depends only on thermal waves at the pump chopping frequency (10 - 100 kHz). For long delays, the steady-state component is dominant and can be used to measure the thermal diffusivity.

        The Link between Fusobacteria and Colon Cancer: a Fulminant Example and Review of the Evidence

        Systemic infections due to Fusobacterium may originate in the tonsillar/internal jugular veins or from the abdomen. We encountered a patient who presented with bacteremia, fulminant septic shock, and extensive soft tissue pyogenic infection due to Fusobacterium necrophorum. In addition, there was widespread metastatic colon cancer with the unique finding of pre-mortem co-localization of F. necrophorum and cancer cells at a site distant from the colon. We reviewed the literature of the association of F. necrophorum and colon cancer, and discuss the evidence of how each of these 2 distinct entities may mutually augment the development or progression of the other.

        Primary sclerosing cholangitis (PSC) and inflammatory bowel disease (IBD): a condition exemplifying the crosstalk of the gut–liver axis

        The close relationship between primary sclerosing cholangitis (PSC) and inflammatory bowel disease (IBD) provides a good opportunity to comprehend the gut–liver axis. The gut and the liver have reciprocal interactions, including how gut inflammation influences the liver through immune cells and the microbiota and how the microbiota in the gut modifies bile acids, which are produced and secreted from the liver. PSC-IBD shows distinct clinical findings from classical IBD. In addition, a distinct genetic predisposition and unique microbiota composition suggest that PSC-IBD is an independent disease entity. Understanding the pathogenesis of PSC-IBD helps to develop novel and effective therapeutic agents. Given the high risk of malignancies associated with PSC-IBD, it is critical to identify patients at high risk and implement appropriate surveillance and monitoring strategies. In this review, we provide an overview of PSC-IBD, which exemplifies the gut–liver axis.
