        TNF계 CD137L 및 RANKL의 파골세포와 T 세포에 대한 활성조절

        본 연구는 TNFR family인 CD137 및 RANK. 파골세포의 CD137L와 T 세포의 RANKL 간의 역신호에 의한 이들 세포의 역할을 알아보고자 하였다. 이에 RANKL 및 CD137L 자극으로 유도되는 역신호 전달에 의한 T 세포 활성과 파골세포분화에 미치는 영향을 규명하고자 웅성 생쥐의 골수세포와 T 세포를 공동배양하여 다음과 같은 결과를 얻었다. 1. 생쥐 단핵세포주 및 골수유도 단핵전구세포에서 CD137L이 발현되며, CD137L 단클론 항체로 자극을 주었을 경우 파 골세포 표지단백절인 TRAP 양성 파골세포의 형성이 억제되었다. 2. 활성화된 CD4^(+) 및 CD8^(+) T 세포에서 RANKL을 발현하였으며 RANKL의 유사 수용체인 OPG 재조합 단백질을 처리하여 CD4^(+) 및 CD8^(+) T 세포의 세포증식이 억제되었다. 이 연구의 결과는 CD137자극에 의한T세포활성 및 RANK 자극에 의한파골세포분화및 활성이 각각수용체에 결합하는 라이겐드의 역신호에 의해 억제되었는데, 이는 파골세포와 T 세포의 과도한 활성을 제어하는 생체의 항상성조절에 관여하는 기전으로 생각된다. Resorption of alveolar bone in periodontitis is due to excessive differentiation and activation of osteoclasts. Bacterial antigens causing periodontitis activates CD4 T cells, which leads to expressing RANK ligand (RANKL) on CD4 T cells. RANKL binds RANK on preosteoclasts or osteoclasts, and enhances the differentiation preosteoclasts into osteoclasts and the activation of mature osteoclasts. CD137, one of TNF receptor (TNFR) family, expressed on activated T cells binds with CD137 ligand (CD137L) on antigen presenting cells. Cross-linking of CD137 by CD137L acts as T cell co-stimulatory signals and, therefore, enhances the activation of T cell. In this study, I elucidated the biological responses of CD137L on (pre)osteoclasts and RANKL on T cells in the context of in vivo interaction between T cells and osteoclasts. RAW264.7. murine monocytic cells, constitutively express CD137L. Ligation of CD137L with anti-CD137L mAb inhibited RANKL-induced osteoclast formation in a dose-dependent manner. Bone marrow cells are expressed CD137L by the treatment with M-CSF. Cross-linking of CD137L abolished M-CSF/RANKL-evoked the formation of multi-nucleated osteoclasts. Both mouse CD4 and CD8 T cells are expressed RANKL following their activation. Ligation of RANKL with OPG, the decoy receptor for RANKL, inhibited both CD4 and CD8 T cell proliferation. These effects were attributed to RANKL-induced apoptosis. These data indicate that CD137L and RANKL on osteoclasts and T cells, respectively provide them with inhibitory signal.

        Hydroa vacciniforme is a rare, light sensitive dermatitis characterized by discrete vesicles followed by varioliform scar formation, which was first described by Bazin on 1862. Our case was a 10-years-old girl who had umblicated vesicles, crusts, varioliform scars on both cheeks, nose, dorsum of hands in symmetrical distribution with itching sensation, which had used to recur in every summer since last five years ago. Histologic examination showed necrosis and multilocular bullae due to reticular degeneration in the epidermis, extravasation of red blood cell and lymphohistiocytic infiltration in the dermis. On urine examination, porphyrin was negative. Diagnosis was established by clinical characteristics and laboratory findins and histologic pictures.

        韓國人에서 VNTR D1S80 遺傳座位의 遺傳的 多樣性 및 集團의 均質性 檢定

        Human genomic DNA prepared from whole blood sample was analyzed to estimate allelic frequencies and genetic diversity at a hypervariable variable number of tandem repeats(VNTR) D1S80 locus in 127 Korean. Target DNA fragments were amplified by polymerase chain reaction(PCR) and separated by polyacrylamide gel electrophoresis. Then, the following results were obtained. 1. A total of 23 alleles were identified at this locus, which is the biggest number of alleles observed at this locus so far(cf. 16 from U. S. Caucasians and 15 from Finnish). 2. Sixty-two genotypes, included 3 homozygotes and 59 heterozygotes, out of a total of 276 probable genotypes were also observed, which might be resulted from the small sample size compared to the number of alleles observed in this study. 3. General pattern of allelic distributions was concordant with those observed in previous studies from Finnish and U. S. Causccasians where two alleles, M18 and M23, were observed most frequently and the rest of them were observed rare(<10%) with the exception of M30 allele(14.6%) in Korean. The number of M30 allele is suspected to be increased in Korean at or after the divergence of Korean from the hypothetical ancestor probably caused by random genetic drift such as founder effect of bottleneck effect. 4. Higher level of heterozygosity was obtained in Korean(H=0.866 and H=0.881) than in Finnish(H=0.77 and H=0.79) and U. S. Cauccasians(H=0.808 and H=0.797). 5. Population homogeneity was tested to corroborate the obtained population genetic parameters calculated based on the assumption of Hardy-Weinberg equilibrium, which could be done by Chi-square test between the observed number of alleles and the expected number of alleles, because of the small sample size analyzed in this study which resulted in high proportion of undetected genotypes (78.5%) and several genotypes significantly deviated from hardy-Weinberg expectation in a sample population. The result suggests that samples were drawn from geneticically homogeneous population. 6. This study suggests that Korean is more diversified than Finnish and U. S. Cauccasians on the basis of the number of alleles and the level of heterozygosity, although Korean is known to be entirely composed of single race. Though we need more informations from other Asian countries, this result suggests frequent gene flow from neighbor countries, which is also expected based on the old Korean history of frequent invasions from Japan and China. 7. Based on the Hardy-Weinberg expectation, 216 out of 253 genotypes at D1S80 locus can be shared by less than 5 individuals among 1,000 unrelated individuals by chance. Our results suggest that the analysis of VNTR D1S80 locus can provide powerful, but imperfect by itself, DNA markers for forensic study, which can be corroborated by analyzing additional VNTR loci.

        Background: Potassium iodide(KI) has been used for various skin problems such as erythema nodosum, chronic vasculitis and deep fungal infections. However, the mechanisms of KI action for those diseases were unclear. Objectives: The aim of this study is to investigate the proliferative and migratory effects on KI in keratinocyte progenitor cells (KSC) and human dermal fibroblasts (HDF), and reveal the cellular alteration which involved in the KI activation and wound healing effect of the KI. Methods: We studied the effect of KI (0.01, 0.1, and 1mM) on cell proliferation, migration and hydrogen peroxideinduced apoptosis using a KSC and HDF cell lines. Animal murine model was used to investigate the effect of KI on excision wound. Results: KI showed stimulation effects on the cell proliferation and migration in HDF. KI reduced apoptotic cell death and increased cell survival in hydrogen peroxide treated HDF and KSC. Furthermore, the topical application of KI accelerates the healing of epidermal and dermal wounds in murine model. Conclusion: In our results, KI showed cell stimulation and protection effects, which induced fast wound recovery. We suggest these effects would be considered for one of mechanism of pharmacological action of KI.

        인진오령산 복용 후 간기능 혈액화학수치가 호전된 1례 보고

        Background: The purpose of this study is to report on patients who showed improvement in liver function test results following the administration of Injinoryeong-san. Methods: A woman visited seeking herbal medicine prescription for symptoms of numbness in her hands and feet. Abnormal elevation of ALT and AST was observed in the blood test conducted before prescribing herbal medicine, so I administered IJORS. Results: After two weeks of taking IJORS, the patient’s liver function test (LFT) returned to normal, and additional improvements were observed in symptoms such as indigestion and fatigue. Conclusion: LFTs are helpful in assessing the patient’s liver function before prescribing herbal medicine. IJORS can be prescribed for patients with abnormal high LFTs.

        Experimental Research Article : Effects of postconditioning with N,N,N`N`-tetrakis-[2-pyridylmethyl]-ethylenediamine in isolated rat hearts

        Background: It was reported that N,N,N`N`-tetrakis-[2-pyridylmethyl]-ethylenediamine (TPEN), a transition metal chelator, confers cardioprotection against myocardial ischemic injury. In this study, we investigated the effect of TPEN targeting reperfusion period in isolated rat hearts. Methods: Langendorff perfused rat hearts were subjected to 30 min of regional ischemia and 2 h of reperfusion. Hearts were randomly assigned to either control (n=9) or 10 μM of TPEN (n=8) groups. TPEN was perfused for a period of 5 min before and 30 min after reperfusion. Results: The ratio of infarct area/ischemic area (AN/AR) was significantly reduced in TPEN treated hearts (6.9±1.7%, P<0.001) compared to control hearts (29.5±3.2%). Recovery of left ventricular developed pressure (LVDP), rate-pressure product (RPP), +dP/dt(max), and -dP/dt(min) in the control group after reperfusion were 53.8±6.2%, 51.0±6.3%, 51.9±5.7%, and 51.4±5.7%, respectively, of the baseline levels. In the TPEN group, LVDP, RPP, +dP/dt(max), and -dP/dt(min) returned to 58.5±4.6%, 54.8±6.4%, 61.7±4.9%, and 53.4±3.9%, respectively, of the baseline levels. There were no significant differences in the cardiodynamic variables between the two groups (P>0.05). Conclusions: Pharmacological postconditioning with TPEN reduces myocardial infarction however, TPEN does not modify post-ischemic systolic dysfunction in isolated rat hearts.       • P223 : Clinical and histologic study of pigmented purpuric dermatoses

        Background: Pigmented purpuric dermatoses (PPD) are chronic and relapsing disorders characterized by asymmetrical rash of petechial and pigmentary macules confined to the lower limbs. Clinical and histopathological study of PPD has not been conducted in Korea. Objectives: The aim of this study was to investigate the clinical and histopathological features of PPD in Koreans. Methods: We reviewed all the medical records, photographs and histopathological slides of 35 patients who had been diagnosed with PPD at Kangbuk Samsung Hospital between January 2008 and June 2013. Results: The mean duration of disease was 26.5 months, and both legs were most frequently involved site (85.7%). Most of the patients were asymptomatic (74.2%). Patients were treated with topical or systemic steroid in 30 cases (85.7%) and 9 patients were added tranexamic acid but there was no difference in the clinical course according to the treatment. The identified etiologic factors were orthostatic pressure (17.1%) and exercise (5.7%). Patients without etiologic factors had more frequent incidence of medical problems (p<0.05). The major microscopic features of the lesions of PPD were perivascular lymphohistiocytic infiltration, extravasated erythrocytes and hemosiderin deposition. Conclusion: The results of this study may help clinicians to diagnose PPD and it should be included in differential diagnosis when cutaneous lesions especially show asymptomatic persisting rash on both lower legs.
