저자들은 신부전이 동반된 당뇨병 환자에서 발생된 부비동형 모균증에 대하여 수술과 함께 liposomal amphotericin B 및 GM-CSF의 복합치료를 하여 성공적으로 치료한 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Mucormycosis (zygomycosis) primarily affects diabetic or immunocompromised patients and typically progresses rapidly, necessitating surgical excision and antifungal therapy with amphotericin B. Large doses of amphotericin B are needed for cure, but it has the risk of causing significant renal toxicity. The recent development of liposomal amphotericin B allows antifungal therapy to be administered with potentially improved efficacy and reduced nephrotoxicity. We have experienced a case of paranasal mucormycosis successfully treated with surgery, liposomal amphotericin B and GM-CSF. A 59-year-old male suffering from diabetes mellitus for 6 years was admitted with pain at left maxillary area. He was diagnosed as mucormycosis after cytologic exam on the necrotic nasal mucosa, which showed typical hyphae. He have had diabetic nephropathy with macroproteinuria and had rapidly rising serum creatinine levels with the amphotericin B treatment: creatinine levels reverted to basal level with the use of liposomal amphotericin B. Despite surgical excision and continued antifungal therapy, his infection was not effectively controlled. Therefore, GM-CSF was administered additionally to improve phagocytic activity of leukocytes. He was finally cured after receiving a combination of aggressive surgery, liposomal amphotericin B and GM-CSF. To our knowledge, this is the first detailed clinical description of the treatment of mucormycosis with liposomal amphotericin B in Korea.

This study was performed to evaluate the usefulness of the electrical analogue model in porous medium. Experimental values by the electrical analogue models were compared with those by the sand models and theoretical values by the fragment method. The results obtained are as follows: 1. The flow lines into the sand model were changed according to rising the upstream water level but those of the electrical analogue model weren't changed, and the equipotential lines were revealed to be almost same in the two kinds of models above. 2. The seepage quantities by the fragment method and the electrical analogue model were a little more than those by the sand model. 3. The exit gradients by the fragment method were steeper than those by the sand model and the electrical analogue model. 4. The seepage quantities were increased in proportion to the increment of the exit gradients. By means of the results above, it may be given as a conclusion that the electrical analogue model test is more useful than the sand model test for appling to seepage flow in the field condition, because the electrical analogue model test is more accurate and simpler. Then, the electrical analogue model test will be able to apply to the three-dimension seepage flow problems.

Applying Fishing-gear Simulation Software to Better Estimate Fished Space as Fishing Effort

Modeling fishing-gear systems is essential to better understand the factors affecting their movement and for devising strategies to control movement. In this study, we present a generalized mathematical modeling methodology to analyze fishing gear and its various components. Fishing gear can be divided into a finite number of elements that are connected with flexible lines. We use an algorithm to develop a numerical method that calculates precisely the shape and movement of the gear. Fishinggear mathematical models have been used to develop software tools that can design and simulate dynamic movement of novel fishing-gear systems. The tool allowed us to predict the shape and motion of the gear based on changes in operation and gear design parameters. Furthermore, the tool accurately calculated the swept volume of towed gear and the surrounding volume of purse-seine gear. We analyzed the fished volume for trawl and purse-seine gear and proposed a new definition of fishing effort, incorporating the concept of fished space. This method may be useful for quantitative fishery research, which requires a good understanding of the selectivity and efficiency of fishing gear used in surveys.
Drug eruption by antihistamine mistaken for chronic urticaria in a child

Although rare, antihistamines can cause adverse effects, including drug-induced eruptions or anaphylaxis. A 4-year-old child visited the pediatric department of a hospital for skin eruptions after administration of antihistamines, (e.g., ucerax [hydroxyzine] or leptizine [levocetirizine]), for cholinergic rashes; he did not have pruritus. Skin prick, intradermal, and drug provocation tests were performed to determine the relationship between the antihistamines and eruptions. Levocetirizine induced wheals in the skin prick test and a rash in the oral drug provocation test. In contrast, ketotifen induced no reaction in the skin prick test but showed a positive reaction in the oral provocation test. Our case report highlights that children can experience the same types of adverse reactions as seen in adults, and cross-reactivity between various antihistamines can occur.
