Variations of arc thermal sprayed and arc welded Fe-Cr-B alloy coating with frictional loading and heat treatment were investigated by means of X-ray diffraction, optical microscopy, scanning electron microscopy, electron probe microanalysis and hardness test. The as-coated layers were shown to be crystalline state but peaks with the frictional loading largely disappeared in X-ray diffraction profiles. The splat is relatively wavy with the boundary in arc thermal spray coating. After heat treatment, the amount of porosity was found to be decreased and the intersplat distance was increased. As annealing temperature increased, interface layer for arc welding coating increased by the diffusion of Cr atoms. The wear type observed in arc thermal spray coatings is splat delamination. The hardness of the arc thermal spray coation layer was superior to that of the arc welding layer. Hardness of coated layer is reduced and microstructures become to be uniformed by the heat treatment.

Fourier's disease is an uncommon form of gangrene involving the scrotum and perineum with synergistic necrotizing fascitis. It was described first by Fournier, a French venerealogist and there are 3 findings characterizing the syndrome: 1) abrupt onset in a healthy male patient, 2) rapid progression to gangrene and 3) absence of a specific causative agent. Today, Fourniers gasngrene is recognized as a form of synergistic necrotizing fascitis secondary to a focus of perianal infection or urinary tract infection or local trauma as possible causal factors. Despite aggressive surgical and medical management Fourniers gangrene has a significatn mortality rate (7 to 50 percent in previous studies). Recently we have experienced 1 case of Fourniers gangrene in a 72 years old male patient who presented us with severe abdominal pain and dyspnea of several days duration. The patients abdominal condition was aggravated progressively, so we have explored the abdomen in emergency with the impression of panperitonitis of unknown origin and found severe inflammation in retroperitoneum, especially in pelvic cavity. Bacterial culture of the paritoneal fluid revealed as E. coli. The patients general condition was not improved significantly even after the operation, and the perianal fluctuation bacame definite. We drained the perianal abscess 12 days after the operation, then the patients condition was improved. The patient was discharged with healing stage of perianal abscess, but the anal sphincter function was still poor.

N/A The clinical study on 31 cases of acute pancreatitis were carried out during the period between October 1986 and December 1991. The male patient outnumbered female by the ratio of 2:1. The most cornmon etiology was alcoholism followed by biliary tract disease and trauma. The majority of the cases presented with chief complaints of abdominal pain, and less common symptoms were nausea, vomiting, fever and jaundice. Among 27 cases who had elevated serum amylase on admission, it turned to normal in 19 cases within 3-6 days of hospitalization. Bacterial cultures from peritoneal fiuid were done on 7 ceases during the laparotomy: Two case yielded positive culture. Assessment of the cases were made according the Ranson's criteria and it was found that the mortality rate was generally parallel to the number of signs each patient developed. Complications developed in 12 patients (40%), and the commonly seen were pleural effusion, pulmonary and renal insufficiency. Of the 10 cases who were operated because of aggravating condition, 4 cases died postoperatively. All mortality cases developed respiratory and renal insufficiency followed by multiple organ failure.

Purpose: This study investigated the correlation between non-mass-like enhancement (NME) observed on preoperative breast magnetic resonance imaging (MRI) and the actual pathological size of breast cancer. We further examined the effect of NME on the positive resection margins during partial mastectomy. Methods: We retrospectively collected data from breast cancer patients who underwent surgery between January 2018 and September 2020. Patients were divided into two groups based on their MRI findings: NME and no-NME (mass-like lesion only) groups. The medical records, including MRI findings and clinicopathological information of patients, were collected retrospectively, and correlations with pathologic results were analyzed. Propensity score matching was applied to develop comparable cohorts of the NME group and no-NME group. Results: This study included a total of 317 patients, with 66 and 251 patients in the NME and no-NME groups, respectively. The mean pathologic size of invasive lesion was significantly smaller than the mean lesion size in the NME group (1.55±1.39 cm vs. 3.45±1.81 cm, p<0.001). The mean pathologic size of ductal carcinoma in situ (DCIS) lesions was larger than that in the NME group but without statistical significance (3.91±2.67 cm vs. 3.50±1.79 cm, p=0.326). In the NME group, NME estimated DCIS size to within 1 cm in 20 patients (30.3%) and overestimated invasive lesion size by more than 1 cm in 31 patients (46.9%). NME (vs. no-NME; odds ratio [OR], 2.967; 95% confidence interval [CI], 0.878-10.025) showed a tendency to predict positive resection margins, but this was not statistically significant (p=0.080). Conclusion: NME findings on MRI showed a similar extent of DCIS lesions. NME findings on preoperative MRI should be considered an important factor for measuring the extent of tumors, especially in DCIS patients.

Three soluble enzyme fractions (F-Ⅰ, F-Ⅱand F-Ⅲ)that hydrolyze phophoinositides were separated from soybean sprouts by using Matrex green gel column chromatography. Among the three phosphatidylinositol (PI)-specific phopholipsase C (PLC) enzymes, only the third fraction (F-Ⅲ) was able to hydrolyze phosphatidylinositol 4,5-bisphosphate (PIP_(2)) as well as phosphatidylinositol (PI) and phosphatidylinositol phosphate(PIP) as substrates. The F-Ⅰand F-Ⅱ fraction only showed enzymatic activities for PI and PIP. The PIP_(2)-hydrolyzing PLC protein, F-Ⅲ, was partially purified using the chromatographic steps of the Matrex green gel, phenyl Toyopearl, Matrex orange gel, Mono S cation exchange, and superose 6 gel filtration columns. The molecular weight of the F-Ⅲ protein was estimated to be about 64 kDa on SDS-PAGE. The protein showed immunocross-reactivity with a polyclonal antibody that was prepared against the X and Y motifs of animal PLC enzymes, the conserved catalytic domains. Ca^(2+) ion critically affected the PIP_(2)-hydrolyzing PLC activity of F-Ⅲ protein, representing maximal activity at 10㎛ Ca^(2+) concentration. The PIP_(2)-hydrolyzing PLC activity of the protein was also significantly increased by sodium deoxycholate (SDC) from 0.05 to 0.08%. However, the activity was greatly reduced above the concentration, and no activity was detected at 0.3% SDC. In addition, the protein exhibited maximal PIP_(2)-hydrolyzing PLC activity at pH, in the range of 6.5-7.5.
